Introduction The incidence of invasive fungal disease (IFD) is increasing in all parts of the world. The etiology for this ongoing epidemiological development is not completely understood. However, major contributing factors are the increasing number of transplantation procedures undertaken around the world (estimated at 500,000 per year), a widening of the indications for intensive chemotherapy, and the growing number of other clinical conditions requiring immunosuppressive treatment. Therapeutic standards have been developed for the most frequent IFD, i.e. candidiasis, aspergillosis and cryptococcosis. However, the so called “emerging fungi” are also a reason for the increased number of IFDs. Thus, clinicians are now facing infections due to a variety of different fungi without any reliable treatment recommendations and therapeutic decision making is not evidence-based. Prevalence of “emerging fungi” is virtually unknown and there is an additional knowledge gap in pathogen biology, since isolates have not been collected systematically. These shortcomings have been described in detail in the recent Clinical Practice Guidelines of the European Fungal Infection Study Group (ESCMID-EFISG) and the European Confederation of Medical Mycology (ECMM). In order to help alleviate the lack of knowledge on epidemiology, clinical course, biology and pathomechanisms, and finally to aid in facilitating an evidence based diagnostic-therapeutic integrated approach of IFD caused by “emerging fungi”, Fungiscope™ - A Global Emerging Fungal Infection Registry has been created in 2003. FungiScope™ uses a web-based electronic case form accessible via www.fungiscope.net. For inclusion into the registry cultural, histopathological, serological or molecular evidence of IFD and associated clinical symptoms and signs are required. Anonymized clinical data on underlying disorder and IFD are entered through a web-based interface. Ideally, the pathogen isolated are collected, examined and stored in a bio-bank called FungiThek. The clinical and biological data are subsequently evaluated and analysed. Figure 1a and b present the data / sample flow and overall project structure, respectively. The clinical database and the biobank are connected, and anonymized data are easily accessable via a web-based search engine - FungiQuest (Figure 2). In the following report we introduce the most current results and achievements. We also discuss future goals and ongoing developments of the project.

Results Until now, 538 cases have been documented and considered valid in the FungiScope registry. The most frequently registered pathogens were mucorales (n=240; 45%), Fusarium spp. (n=80; 15%), yeasts (n=71; 13%), and dematiaceae (n=55; 10%) followed by Scedosporium spp. (n=29; 5 %) and Penicillium spp./Paecilomyces spp. (n=26; 5%) (Figure 3). The contribution of cases by country is shown in Figure 4. The majority of documented patients were Caucasian (71%) followed by Asian (22%) and North/South American Indian origins (2%) (Figure 5).

Sites of infection differ among pathogens (Figure 6). In patients with invasive mucormycosis, the lungs were the most commonly involved organs with an incidence of 68.3%. Other involved sites were the paranasal sinuses (21.3%), deep soft tissues (16.3%) and the central nervous system (13%). Fusarium spp. infections most frequently presented as blood stream infections (51%) or skin/deep soft tissue infections (51%). Other commonly affected sites were lungs (40%), Eyes and Paranasal sinuses (15%). Yeasts were most often diagnosed from blood cultures (71.8%) followed by pulmonary infections (32.4%). Infections with Dematiaceae mostly involved the skin, lungs, deep soft tissues, eyes, and paranasal sinuses (range from 14.5% to 27.3%).

Most frequent risk factors for the development of IFD are shown in Figure 7 for most common pathogens. The most common risk factors for developing an infection with Mucorales and Fusarium spp. were chemotherapy (56.7% and 60%, respectively) or haematopoetic stem cell transplantation (HSCT) (28.8% and 36.3%, respectively) followed by Intensive Care Unit (ICU) stay (23.4% and 16.3%, respectively) and diabetes mellitus (17.1% and 16.3%, respectively).

In patients presenting with yeast infections chemotherapy was also the most common risk factor (54.9%). One third of patients were treated in the ICU. Other risk factors were HSCT (23%), diabetes mellitus (14.1%), chronic pulmonary disease (9.9%), and solid organ transplantation, chronic renal disease or chronic liver disease (7%). By contrast, patients with a dematiaceae infection presented most frequently with diabetes mellitus (38.5%) and/or chronic renal disease (13%).

All-cause-mortality, mortality due to fungal infection and response to treatment differed among individual pathogens and are shown in Figure 8. The highest all-cause-mortality was observed in patients with yeast (59.2%), followed by patients with mucoromycosis (56.5%) and Fusarium spp. infections (51.3%). Mortality due to the fungal infection, the attributable mortality, was highest in infections caused by mucorales (42.1%) followed by yeast (40.6%) and Fusarium spp. (37.5%). By contrast, the lowest all-cause-mortality was revealed in patients with dematiaceae, equaling only 18.2%. Just 5.5% of these deaths were primarily attributable to the fungal infection.

Favorable response to antifungal therapy was achieved in 77% of patients with an infection caused by dematiaceae, 59% with Fusariosis and 47.8% with yeast infection. In those cases complete and partial response is considered favorable outcome, whereas for mucormycosis stable disease is favorable, too. For mucormycosis favorable response was reported for 49.8% of the cases.

In conclusion, the clinical relevance of invasive fungal infections by emerging fungi is increasing. Over the past 10 years a large number of cases from several countries world-wide were documented and showed a broad spectrum of pathogens. Investigators and coordinators are encouraged to contribute to FungiScope™ and by this significantely contribute to the development of improved treatment and clinical management strategies for these rare diseases.
