Four (4) original duly accomplished Report of Marriage Form typed or printed legibly in black or blue ink in blockletters, bearing the original signature of both husband and wife. Wives should use their maiden name as the last name.

One (1) original Marriage Certificate, duly authenticated by the Ministry of Foreign Affairs (MFA) of the country where the marriage took place and three (3) photocopies

Four (4) photocopies each of the data page of the passport of husband and wife

English translation of non English documents

Joint Affidavit of Delayed Registration of Marriage if the Report of Marriage is filed after 12 months of the date of marriage.

One (1) original and three (3) photocopies of the Philippine Statistics Authority ( formerly NSO) - issued AnnotatedMarriage Certificate ( which indicates the details of the annulment) and/or DFA authenticated Judicial Decree ofAnnulment with Certificate of Finality.One (1) original and three (3) photocopies of the Philippine Statistics Authority (formerly NSO) issued BirthCertificate of Filipino wife if Filipino wife had a previous Philippine marriage annuled.
