SECTION I – GENERAL INFORMATION
INTRODUCTION
The Ocala Housing Authority is requesting proposal for Property Coverage; General Liability
and Automobile Liability/Physical Damage. Insurance is to be effective January 1, 2020.
It is requested that proposers show a premium breakdown for each line of coverage proposed
and the replacement value for the Mobile Equipment.
All proposers, Agents and companies must be duly licensed for business in the State of Florida
by the Florida Department of Insurance. Insurance company must have a Policyholder’s Rating
of A- or better. Self Insurance funds are acceptable, but must attach the latest financial statement
to their proposal.
Proposers should indicate the carrier’s most favorable terms of premium payment for all policies
quoted.
The current property coverage is written with Housing Authority Insurance Group. Scheduled
building/contents values are $16,352,386 with additional values for inland marine coverage.
Due care and diligence have been exercised in preparing these specifications, and all information
is believed to be substantially correct. However, the responsibility for determining the full extent
of the exposures to risk and verification of all information rest solely with the proposer. Neither
the Ocala Housing Authority nor any of either entity’s representatives shall be responsible for
any error or omission in these specifications, nor for the failure on the part of the proposer to
understand the full extent of the exposure. Should you need additional information, please

UNINTENTIONAL OMISSION OF EXPOSURE – In accord with the above paragraph, all due
care and diligence have been exercised in preparing these specifications; however, in the event
that an exposure has been unintentionally omitted, the proposing carrier should include an
endorsement protecting the Ocala Housing Authority of Marion County from denial of coverage
in the event of a loss to that omitted item. This endorsement should provide coverage for
unintentionally omitted items as well as providing that premiums must be paid retroactively to
the effective date of the policy, or date of addition of any item of exposure, whichever is later.
NAMED INSURED – The Ocala Housing Authority of Marion County shall be the named
insured of the accepted policies. However, the named insured should be endorsed to cover
elected officials; boards; commissions; authorities or agencies; etc. and employees and
volunteers of the Ocala Housing Authority of Marion County, as applicable.

PROPOSAL RETURN DATE – Sealed proposals must be received either by mail or hand
delivery no later than 3:00 PM, November 22, 2019. Four (4) copies of the proposal must be
received. Completed proposals should be delivered to the Ocala Housing Authority, 1629 NW
4th Street, Ocala, FL 34475, Attention: Dr. Gwendolyn B. Dawson, Chief Executive Officer.
Section III of the RFP must be returned with your proposals (information indicated on pages 24-
25).
CONTRACT TERMS – The effective date of the proposed coverage will be January 1, 2020.
Rates should be guaranteed for a minimum of 12 months. Longer guarantee periods are
solicited. Subsequent renewal terms of this contract will be based upon satisfactory service
along with acceptability of cost and financial stability of the carrier. Any rate increase or intent
to terminate coverage must be preceded by a minimum of 90 days notice to the Ocala Housing
Authority.
NON-WAIVER OF SOVEREIGN IMMUNITY – In accord with Florida Statute 768.28,
which preserves the Authority’s sovereign immunity, insurers shall not be permitted to endorse
policies to waive the Authority’s sovereign immunity above $100,000 per claim or
judgement/$200,000 per incident or occurrence.
CONFLICT OF INTEREST STATEMENT – Proposers shall abide by Florida Statute 112
and disclose within their proposal any officer, director or agent of their organization who is also
an employee of the Ocala Housing Authority. Also, the name of any employee of the Ocala
Housing Authority who owns more than 5 percent of the proposing organization must be
disclosed.
PUBLIC ENTITY CRIMES STATEMENT – The attached form 7068 “Florida Public Entity
Crimes Statement” must be signed, notarized and returned with your proposal. Form 7068 is
located in Section VIII.
DRUG-FREE WORKPLACE STATEMENT – Please submit the attached Drug-Free
Workplace Statement as applicable to the proposer and/or insurer. This form is found in Section
VIII. It is not mandatory that the proposer and/or insurer have a Drug-Free Workplace in order
to respond to the RFP.

Notice is hereby given that the Ocala Housing Authority will accept Proposal for Insurance until November 22, 2019; 3:00 p.m. Each proposal shall be sealed and endorsed with the name of the proposer and plainly marked “Proposal for Insurance: Ocala Housing Authority.” Four copies of your Proposal should be forwarded to the OHA at the following address:

The OHA reserves the right to reject any or all Proposals, waive formalities, negotiate terms and conditions, and to select the agent and service options that best meet the needs of the OHA and its employees. During the evaluation process, the OHA reserves the right, where it may serve the OHA best interest, to request additional information or clarifications from vendors, or to allow corrections of errors or omissions. At the discretion of the OHA, firms submitting proposals may be requested to make oral presentations as part of the evaluation process. CONDITIONS AND STIPULATIONS You are invited to submit your Proposal for the administration of the indicated insurance plans based on the information contained in this Request for Proposal. Firms may choose to be considered for any or all of the following services: Current Coverage Amount General Liability $1,000,000 Automobile Liability See Schedule of Value Automobile Physical Damage See Schedule of Value Administrative Office Property – Building & Contents See Schedule of Value Blanket All Property $16,352,386 Mobile Equipment See Schedule of Value Boiler & Machinery $50,000,000 Unless a specific note is made to the contrary in your Proposal or a subsequent contract, we will assume that your Proposal conforms to the OHA’s Specifications and an award to you will bind you to comply fully with all of the following Conditions, Stipulations, and Specifications. General Conditions and Stipulations The OHA reserves the right to accept or reject any or all proposals and to waive formalities and select the agent(s) that best meet the needs of the OHA and its employees. The OHA’s objective is to select an agent(s) who will provide the best possible service at the best possible cost while meeting the Request for Proposal specifications. The OHA is not obligated to award the contract based on cost alone. The OHA reserves the right to designate more than one agent of record for the different coverage options. Agent(s) that are awarded the business shall submit an action plan and timetable for assuming responsibilities to the OHA within thirty (30) days from when the Council approves the selected agent(s) of record. A servicing representative must be available to the OHA on an on going basis. Representatives must be available at the all renewal meetings during the contract period to explain the plan. Representatives must also be available for all plan design and cost containment planning meetings. The agent(s) who is selected must be recognized as an agent or apply to be an agent for the Benefit Carriers used by the OHA.

Rejection of submittal: The OHA reserves the right to reject any and all submittals received in response to the RFP; and to waive any minor technicalities or irregularities as determined to be in the best interest of the OHA. The OHA reserves the right to award the contract in the best interest of the OHA.
This service is being assisted by funds from the U.S. Department of Housing & Urban Development (HUD). Accordingly, any contract(s) awarded by the OHA will be subject to all special requirements which are applicable to HUD-assisted contracts, including: Equal Employment Opportunity and Section 3 Employment and Contracting requirements. In addition, this project is subject to HUD requirements for contracting with small and minority firms and women’s business enterprises.
AUDITS
The U. S. Department of Housing and Urban Development, the Comptroller General of United States, or any of their duly authorized representatives, shall have access to any books, documents, papers and records of Vendor, which are directly pertinent to this contract for the purpose of making audits, examinations, excerpt and transcriptions.
DISCRIMINATION PROHIBITION
No person shall, on the grounds of race, sex, age, handicap, familial status, religion creed, color or national origin, be refused the benefits of, or be otherwise subjected to discrimination under any activities resulting from this RFP.
PUBLIC ENTITY CRIME INFORMATION STATEMENT
Section 287.133 (2)(a), Florida Statutes, contains the following provisions: A person or affiliate who has been placed on the convicted vendor list following a conviction for public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, may not submit a bid on a contract with a public entity for the construction or repair of a public building or public work, may not submit bids on leases of real property to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, and may not transact business with any public entity in excess of the threshold amount provided in Section 287.017, for CATEGORY TWO for a period of 36 months from the date of being placed on the convicted vendor list.

SUMMARY OF COVERAGE OPTIONS
BACKGROUND INFORMATION The OHA of Ocala located in Marion County, State of Florida is managed under the Board of Commissioners format with a relatively small but fast growing employee base. The OHA currently employees 24 full time employees, which are made up of diverse employment positions and demographics. The OHA employs such personnel as Executive Managers, Maintenance Mechanics, Inspectors and administrative specialist. The OHA is divided into four (4) departments: Public Housing, Section 8, Accounting and Administration. For the purpose of generating accurate costs for insurance coverage, please find a current list of OHA automobiles below, and property on the following page, which should the basis for all coverage quotes. All information is protected and can only be used for express purposes outlined in these documents.

REQUIRED CONTENTS OF PROPOSAL
The purpose of the Proposal is to demonstrate the qualifications, service level, and cost for services, competence and capacity of the firms seeking to become an agent of record for the OHA of Marion County. The proposal should address all the points outlined here as required. A. Which insurance coverage(s) your firm is bidding to be an agent of record for. B. Firm History and Experience:
1. Brief history of firm including size and any specialty areas. 2. Background company data, including financial references. 3. Particular expertise or involvement in the insurance industry. 4. Municipality, nonprofit, HUD experience. 5. List of providers the firm is associated with. 6. Expected communication responsibilities.
C. Qualifications:
1. Description of service philosophy. 2. Conceptual program structure and pricing. 3. An introduction of the account team, by name with specific roles,
qualifications and experience, and distribution of responsibilities including support capabilities.
4. Current use of technology, especially capability for computerized legal/benefit design research and for sharing and editing documents electronically.
5. Action plan and timetable for assuming responsibilities as well as future design/cost containment plan.
6. Detail of services that will be provided to the OHA. 7. Indicate current responsibilities of person designated to serve as lead
contact for the OHA. D. References:
1. List new clients and the clients that have left you within the last three years. Provide the contact names and telephone numbers.
2. List of cities you currently represent and for what type of service. Provide the contact names and telephone numbers.
3. Provide the contact names and telephone numbers of five (5) clients in the State of Florida with whom you have had a working relationship, as a reference for the OHA. (Preferably, the references should be governmental units.) Include two groups that recently terminated coverage.

E. Conflict of Interest:

1. List of previous engagements with the OHA of Marion County. 2. Describe what lead to the end of the engagement.
G. Proposed Fee for Services:
1. Explanation of compensation plans for your firm under this proposal including all services to be included in that fee.
2. The proposal should contain the proposed fee for services either in a fixed dollar amount or as a percentage of premiums paid for coverage. Fees are to be maintained at the proposed level unless approval is given by the OHA Council for a change in the fee structure or level.
3. List any additional service options as well as the fee structure involved. 4. Indicate any alternate billing arrangements you would be willing to
consider and under what circumstances they would be most appropriate. H. Copies of the following items:
1. Errors and omissions insurance coverage 2. Business license 3. Statement of compliance with federal and state laws. 4. Description of the firm’s view of their responsibilities to the OHA in the
provision of insurance coverage brokerage services. The OHA anticipates a change in providers and will use the following the criteria for supplemental evaluation. I. Insurance Provider Information, Plans and Cost:

PROPOSAL EVALUATION A. Criteria The OHA will evaluate proposals based on the needs of the OHA. The following criteria will be used in evaluating each of the Agent/Broker responses:
1. Compliance with specifications. 2. Ability to provide strong administrative support and member services to
the OHA. 3. Compliance with applicable State and Federal laws and regulations. 4. Financial position of the agency. 5. Cost (both premium and broker services). 6. Experience and Qualifications.
B. Review of Proposals The OHA will evaluate the proposals based upon the factors listed above and may make a recommendation to the OHA Board of Commissioners if services proposed are deemed better than existing services and rates. The successful candidate will be required to enter into a written agreement with the OHA of Marion County that will include service agreements and compensation for all coverage(s). This agreement may last for a period of 2 years and will contain a review process and termination provisions. The OHA of Marion County reserves the right to retain all proposals submitted and use any idea in a proposal regardless of whether that proposal is selected. The OHA will choose the proposal(s) that best fits its needs. The OHA is not obligated to award the contract based on cost alone, nor is the OHA obligated to award only one agent of record.

The following contract clauses are required in contracts pursuant to 24 CFR 85.36(i) and Section 6002 of the Solid Waste Disposal Act, as amended by the Resource Conservation and Recovery Act. HUD is permitted to require changes, remedies, changed conditions, access and records retention, suspension of work, and other clauses approved by the Office of Federal Procurement Policy. The PHA and contractor is also subject to other Federal laws including the U.S. Housing Act of 1937, as amended, Federal regulations, and state law and regulations. Examination and Retention of Contractor’s Records. The PHA, HUD, or Comptroller General of the United States, or any of their duly authorized representatives shall, until three years after final payment under this contract, have access to and the right to examine any of the Contractor’s directly pertinent books, documents, papers, or other records involving transactions related to this contract for the purpose of making audit, examination, excerpts, and transcriptions. Right in Data and Patent Rights (Ownership and Proprietary Interest). The PHA shall have exclusive ownership of, all proprietary interest in, and the right to full and exclusive possession of all information, materials, and documents discovered or produced by Contractor pursuant to the terms of this Contract, including, but not limited to, reports, memoranda or letters concerning the research and reporting tasks of the Contract. Energy Efficiency. The Contractor shall comply with all mandatory standards and policies relating to energy efficiency which are contained in the energy conservation plan issued in compliance with the Energy Policy and Conservation Act (Pub.L. 94-163) for the State in which the work under this contract is performed. Procurement of Recovered Materials (a) In accordance with Section 6002 of the Solid Waste Disposal Act, as amended by the Resource Conservation and Recovery Act, the Contractor shall procure items designated in guidelines of the Environmental Protection Agency (EPA) at 40 CFR Part 247 that contain the highest percentage of recovered materials practicable, consistent with maintaining a satisfactory level of competition. The Contractor shall procure items designated in the EPA guidelines that contain the highest percentage of recovered materials practicable unless the Contractor determines that such items: (1) are not reasonably available in a reasonable period of time; (2) fail to meet reasonable performance standards, which shall be determined on the basis of the guidelines of the National Institute of Standards and Technology, if applicable to the item; or (3) are only available at an unreasonable price. (b) Paragraph (a) of this clause shall apply to items purchased under this contract where: (1) the Contractor purchases in excess of $10,000 of the item under this contract; or (2) during the preceding Federal fiscal year, the Contractor: (i) purchased any amount of the items for use under a contract that was funded

with Federal appropriations and was with a Federal agency or a State agency or agency of a political subdivision of a State; and (ii) purchased a total of in excess of $10,000 of the item both under and outside that contract. Termination for Cause and for Convenience (contracts of $10,000 or more). (a) The PHA may terminate this contract in whole, or from time to time in part, for the PHA’s convenience or the failure of the Contractor to fulfill the contract obligations (cause/default). The PHA shall terminate by delivering to the Contractor a written Notice of Termination specifying the nature, extent, and effective date of the termination. Upon receipt of the notice, the Contractor shall: (1) immediately discontinue all services affected (unless the notice directs otherwise), and (2) deliver to the PHA all information, reports, papers, and other materials accumulated or generated in performing the contract, whether completed or in process. (b) If the termination is for the convenience of the PHA, the PHA shall be liable only for payment for services rendered before the effective date of the termination. (c) If the termination is due to the failure of the Contractor to fulfill its obligations under the contract (cause/default), the PHA may (1) require the Contractor to deliver to it, in the manner and to the extent directed by the PHA, any work described in the Notice of Termination; (2) take over the work and prosecute the same to completion by contract of otherwise, and the Contractor shall be liable for any additional cost incurred by the PHA; and (3) withhold any payments to the Contractor, for the purpose of set-off or partial payment, as the case may be, of amounts owned by the PHA by the Contractor. In the event of termination for cause/default, the PHA shall be liable to the Contractor for reasonable costs incurred by the Contractor before the effective date of the termination. Any dispute shall be decided by the Contracting Officer.
