2. Scope of Work Community Utilities of Indiana, Inc. ("CUi i" or "Uti lity") requests cost proposals to provide Professional

Services related to the design, installation, and connection of two new water wells (Well# 12 and Well#

13) within our Twin Lakes system. These proposed new wells would service Water Treatment Plant #2.

The intent of this RFP process is to identify a successful Bidder by determining which proposal provides

the best overall value to CUii.

2.1. Existing situation

The Twin Lakes potable water system is entirely ground-water sourced and has two water treatment

plants (WTP #1 and #2) which perform iron filtration and chemica l treatment. The chemical treatment

includes disinfection, fluoridation, and polyphosphate add ition. Water treatment plant #2 has two

Tonka brand pressure filters, each with a treatment capacity of approximately 220 gpm, for a total

treatment capacity of approximately 440 gpm.

Historically WTP #2 has been served by two wells, Well #8 and Well #9, which were each installed with a

design capacity of 200 gpm. Well #8 was installed in 1987 and is located directly to the west of WTP #2,

within a permanent easement. Well #9 was installed in 1987 and is located to the east in Lions Park,

within a permanent easement. The treatment plant, the two wells, and the general vicinity are shown in

the below Figure.

The production capacity of each of the wells has declined and is currently 60 gpm for Well #8 and 70

gpm for Well #9. Several well rehabilitation procedures were conducted in Spring and Summer of 2018

to try and improve the yield from Well #9 but these attempts were unsuccessful. As a result we are

) .

evaluating the design and installation of two new wells that would either supplement, and/or replace,

existing Wells #8 and #9.

Our plan is to perform the following:

1. Install new well #12 near to existing wel l #8 and use both wells to provide raw water to the Twin Lakes system. The reason for this is to try and minimize the reduct ion in well capacity tha t occurs over the years.

2. Install new well #13 within the easement of existing well #9, but approximately 95 ft to the northwest,

and then abandon well #9. The reason for th is approach is to locate the new well such that, in the future,

additional wells can be placed within the existing easement. Unfortunately, this means that the new well

will be less than 100 feet away from the existing well and is thus not allowed per IDEM regulations.

2.2. New wells and raw water transmission mains

CUI desires to drill two (2) new water wells within the Twin Lakes water system to supplement and/or

replace existing Wells #8 and #9. Well# 9 (in Lions Park) will be abandoned due to insufficient capacity.

The proposed wells (Well Nos. 12 and 13) will be drilled on existing CUI right-of-way with one well

located near each of Well 8 and 9 sites. In order to prepare fai r and consistent bids, the follow

assumptions should be use for both wells:

1. Test well: Each well will have a test hole installed prior to drilling the production well.

a. Test hole will be instal led using sonic drilling method. b. Continuous samples will be collected and analyzed for screen sizing and length, well design.

c. Test hole will be advanced to bedrock (assume depth is 125 feet for bidding purposes)

2. Water samples a. Not required during the test hole construction

b. 24-hour pump t est and water samples required for the production well

c. For bidding purposes assume required testing parameters are: fluoride, N03, and coliform

3. Production well (12 inch) shall be bid using the Barber (dual-rotary) or dual tube reverse circulation

methods. Mud rotary drilling may be bid as an alternate. Cable tool drilling methods (e.g., percussion

drilling) are not permitted.

a. For bid purposes assume that the proposed wells will be 125 ft deep and have a design capacity

of approximately 200 gpm each, such that no modifications to the existing treatment facilities

will be required.

4. Well development: a. Assume 1 hour development per foot of screen, assume 20 ft of screen for bidding purposes

b. Development shall be tan k surging, followed by jet washing. Note modifications from this in your

bid. 5. Mechanical systems

a. Pumps shall be stainless steel, sized for 75 ft depth (to the inta ke screen of the pump) b. Motors shall be all stainless steel, 200 gpm, assume 10 ft friction (dynamic) head and 65 psi

needed for the pressure filters in the treatment facility

c. Down hole transducers or VFDs are not necessary

d. Well housing-shall be Baker Pitless (or equiva lent), sized for a 12 inch ID, with welded discharge

point, 6.5 ft bury depth. The Baker Pitless unit is to include an integrated sample tap (OEM}

e. Permanent sampling shall be via integrated sample tap. An external sample spigot may be bid as

an alternative. f. For determining water level include PVC sleeve, 3/4/ inch, tied into the Baker Pitless plug-in cap

g. Well down pipe (drop pipe) to be 4 inch PVC Certa-Lok, with a drilled-modified catch cable.

h. Well casing sha ll be standard steel, well screen to be stainless steel, size of the screen to be

determined by the test well, assume 20 ft of screen for bidding purposes

i. Well head protection cage shall be a Baker "Protective Well Enclosure" or equivalent, mesh not

acceptable, mounted on a poured concrete pad

j. Poured concrete pad to be 2'x2' by 8 inches thick, may be machine-mixed onsite

6. Subcontracting: Well driller will be responsib le for subtract ing:

a. Surface water piping

b. All electrical work

7. Permitting: Well driller will be responsible for all permitting, including but not limited to:

a. IDEM site survey

b. Construction permit

c. Permit t o install

8. Landscaping:

a. Assume $800 standard seeding/grading b. Add itional shrubbery/landscaping will be handled directly by CUii

Clearly note any exceptions to the above in your bid.

2.3. Proposed new Well #12 (near existing Well #8 and WTP #2)

The location of we ll is as shown in the sketch below.

Specific details of items to be installed by the contractor:

1. Water meter, meter vault suitable for operator ent ry, 8 ft bury 2. Raw water piping connected via tee to exist ing 6 in raw water main, via a 15 ft pipe run.

3. New isolation valves to be added to both the new raw water main and the existing raw water main to

allow for independent operation

4. New electrical controls, assume 100 ft electrical run from new well location to water treatment plant.

2.4. Proposed new Well #13 (near existing Well #9 in Lions Park)

The location of well is as show n in the sketch below.

Specific details of items to be installed by the contractor:

1. Contractor to reuse the electrical controls from the existing well #9 (which will be abandoned)

2. Add new safety electrical disconnect at the well locat ion (mounted on a post 7 ft away from the well head)

3. Safety electrical disconnect to be lockable, stainless steel, waterproof mounted on a 4x4 wooden post, post painted brown

4. Assume 95 feet of raw water main, to be connected to the existing raw water main currently serving well

#9

5. Single isolation valve to be included, on the new raw water main, between new wel l #13 and location of

exist ing well #9, to be loca ted approximately 10 ft from the new well

2.5. Project duration and timelines

We anticipate that NTP will be given by October 18 and work sha ll be completed by May 15, 2019.

2.6. Bid scope

Please provide four separate bids, as fo llows:

1. Bid for new we ll #12 2. Bid for new well #13

3. Bid for abandoning existing well #9 (In Lions Park)

4. Bid for the ongoing well rehabilitation and maintenance for the wells at:

a. Twin Lakes, 2 wells per year (7 wells total), ongoing b. Water Service Corporation, Inc, 2 wells, once every five years

A combined bid for doing both well nos. 12 and 13 may be provided, if desired.

3.2. Questions and Clarifications

Bidders are responsible for satisfying themselves as to the requirements of this RFP. All quest ions or

requests for clarification must be directed in writ ing to the primary contact person identified above.

Bidders should include the RFP name in the email subject line.

Inqui ries wi ll be received until the time indicated in Section 4.2 below . If determined necessary, CUii will

issue an addendum and circulate responses to all Bidders.

3.3. Bid Evaluation

3.3.1 Prequalification

CUii fo llows a prequalification procedure, w hereby we invit e Proposals from qualified bidders. Bidders

must meet our prequalification standards in order to have their Proposals fully considered.

3.3.2 Bid evaluation

After the Closing Time, CUii wi ll review and evaluate all Proposals received in accordance with the

submission requirements of this RFP. Bids w ill not be opened publicly. Proposals w i ll be evaluated based

on the overall best va lue to CUii.

3.3.3 Bids Response

At any time after the Closing Time and before award of the Contract, CUii has in it s sole discretion, the

right:

a. to invite some or all the of th e Bidders to appear before Utility; b. to request any additiona l informat ion, details or materials from a Bidder that CUii requires to

sat isfy itself that the Bidder will be able to carry out the Work; and/or c. to negotiate changes to the scope of Work or any other condition with any one or more of the

Bidders without having any duty or obligation to advise any other Bidder. Uti lity shall have no liability to any Bidder because of such negotiations or modificat ions.

3.3.4 Bid acceptance and rejection

Notwithstanding any other provis ion of t he RFP, the Utilit y has in its sole discretion, the right to:

a. accept or reject any Proposal; b. reject all Proposals; c. accept a Proposal that is not the lowest Proposal; d. accept a Proposal that deviates from the requirements of this RFP;

e. reject a Proposal even if it is the only Proposal received by CUii;

f. accept all or any part of a Proposal; and

g. split the requirements of the RFP between one or more Bidders.

The Utility may elect to disqualify any Bid w hich fails to meet any of t he requirements of t his RFP.

However, the Ut ility in its sole discretion may waive any non-compliance with this RFP and may elect to

retain for consideration Bids which are non-conforming, which do not, for example, contain the content

or form required by the RFP, or which have not complied with the process for submission set out herein.

3.4. Contract award

The Contract is awarded when an authorized representative of the Utility notifies the selected Bidder via

email of its acceptance of its Bid, including any mutually agreed modifications, clarifications or

negotiated changes (if any).

Fa ilure of the selected Bidder to execute and deliver the Contract, together with any such other

documents as is required by the RFP or the Contract, within the t imeline noted above, will be considered

just cause for cancel ing the award. CU ii may then commence discussions with one or more of the other

Bidders.

The RFP and the submission of a Proposa l do not, nor does the RFP process in and of itself, or t he

select ion or notification of a Bidder, create a legally binding contract. It is only upon the award and

execution of the Contract by an authorized representative of CUii that CU II and selected Bidder become

legally bound to the other to perform the obligations under such Contract.

3.5. Confidentiality

All informat ion provided by or obtained from CUii in any form about the RFP process, including

Information about CU ii and its business activities, is the sole property of CU ii and must be treated as

confidential. Such information is not to be used for any purpose other than enabling each Bidder to

submit a Proposal. It must not be copied, disclosed or released without prior written approval from CUii

and shall be returned by Bidders to CUii promptly upon the request from CUii. Each Bidder agrees that

this obligation of confidentiality will survive the termination of the RFP process and/or the Contract.

4. Instruction to Bidders

4.1. Definitions

In this Invitation to Bid :

a. "Bidder" means the individual, partnership, corporation or combination thereof, including joint

ventures, providing or submitting a cost proposal in response to this RFP.

b. "Contract" means the cont ract to be executed between CUii and the Successful Bidder that will

govern the Work.

c. "RFP" means this document, including all addenda issued by CUii.

d. "Successful Bidder" means the Bidder that enters the Contract with CUii.

e. "Work" means the scope of services to be provided by the Successful Bidder t o CUii pursuant to the Contract. Such services are described in Section 2 of this RFP.

4.2. Proposal Submission and Due Date

Questions wi ll be received at the above email address until two (2) days before the due date, at 9pm

(Centra l Time). CUii staff will reply to written questions to the extent possible.

4.3. Submission Requirements

Please include the following documents, in order, saved together as one single RFP document.

4.3.1 Timing Proposals will be received up until the Closing Time. It is solely the responsibility of the Bidder to ensure

that their Proposal is received by the Closing Time. Proposals received after the Closing Time may not

be accepted and may be returned to the Bidder unopened.

4.3.2 Proposal information Proposals should include the following information :

1. Experience and t rack record with similar work

2. Experience with private uti lity companies and/or small municipal syste ms and/or experience with Utili t ies, Inc.

3. Proximity of bidder's offices t o CUii's utility systems 4. Project Approach and Understanding

5. Technical Proposal and Price

a. Unit and equipment costs as relevant

b. Equipment to be used for the project

c. Materials consumed and used during the project d. Line items for costs, such as mobilization, demobilization, special testing and/or inspections

6. Clearly denote excluded items, such as permits, specialty parts, and similar good and services, which are NOT part of your proposal.

As necessary, please include project understanding, tasks, team/resumes, roles, timeline(s), and other

details. Only include those resumes of personnel who will be substantially involved with performing the

project tasks.

Proposals shall be emailed as one complete PDF file containing all proposal elements, and should be

written in a format appropriate to describe the overall proposal.

4.3.3 Bid Form.

Attached as Section 5 to this RFP.

4.3.4 Legal signing authority

Proposals shall be executed by an officer or employee having authority to bind the Bidder to the terms

and conditions of this RFP and the Contract.

4.3.5 Exceptions Each Bidder must clearly state in their Proposal any proposed exceptions it may have to the

requirements of this RFP. For each proposed exception, Bidder sha ll outline the applicable document,

section, the reason for the exception, and specific alternate wording. CUii will, in its sole discretion,

determine what constitutes an acceptable exception. Any exceptions that are not acceptable to CUii

may be cause for rejection of the Bidder's Proposal. If no exceptions are indicated in the Proposal, Ut ility

will be entitled to interpret that the Bidder offers to perform the Work in full compliance with the

requirements of this RFP.

4.3.6 Ownership of proposal submissions Proposals submitted to CUii in response to this RFP will become the sole property of CUil. Bidders

should anticipate that their proposal submissions will be used to support utility costs and expenses to

the applicable State Utility Commission(s) and will thus be available for public viewing and review. At no

time should Bidders expect or request "trade secrecy" or similar protections for intellectual content.

4.3. 7 Withdraw of proposal Proposals may be withdrawn by written notice only, provided such notice is received prior to the Closing

Time by the designated email addresses. A Bidder who has withdrawn their proposal may submit a new

proposal, but only in accordance with the terms and conditions of this RFP.

4.4. Bidder costs

All costs associated with the preparation and submission of a Proposa l, including any costs incurred after

the Closing Time, will be borne solely by the Bidder. By submitting a Proposal, each Bidder is

acknowledging and agreeing that CUii will not be responsible in any way for any costs, expenses, losses,

damages (including damages for loss of anticipated profit) or liabilities whatsoever incurred or suffered

by the Bidder as a result of submitting its Proposal, taking part in meetings, or otherwise in any way

connected with this RFP, including CUii 's acceptance or non-acceptance of any Proposal or any contract

award not made in accordance with the express or implied terms of th is RFP.

4.5. Duty to inform

4.5.1 Limitations on information provided

It is the sole responsibility of each Bidder before submitting a Proposal to examine, inspect and fully

inform itself as to all aspects of the Work under the Contract including the requirements set out in the

RFP. CUii makes neither representation nor warranty in relation to any information supplied about this

RFP, its adequacy, accuracy or suitability for any purpose. By submitting a Proposal, Bidder

acknowledges and understands that CUii is not liable for any loss or damage arising from the use of the

information, however caused.

4.5.2 Understanding of Site Conditions By submitting a Proposal, each Bidder will be deemed to have satisfied itself as to all matters relative to

the location of the Work, including, without limitation, the form and nature of the location of the Work,

details and levels of existing Work, the quantities and nature of the Work and materia ls necessary for

the completion of the Work, the means of access to the site, and in general to have itself obtained all

necessary information as to risks, and other climatic, geological, hydrological and natural conditions or

such other contingencies which may influence or affect a Bidder's Proposa l. No claim arising from

Bidder's failure to adequately inspect and examine the location of the Work will be considered.

4.5.3 Amendments or cancellation by CUI/.

Utility may in its discretion at any time and for whatever reason:

a. Clarify, modify, amend or otherwise change the RFP by issuing a written addendum. Addenda

issued by CUii will form an integral part of the RFP.

b. Alter or cancel the RFP process by issuing a written notice to Bidders.

MWP wishes to outline our understanding of the scope of work. Before reviewing the project details, MWP would first like to stress our commitment to providing Community Utilities of Indiana, Inc. (CUII) with a company committed to making Health & Safety our number one priority on this project. We at MWP stress safety on every job. MWP as of today has worked 1,496 days without a lost time accident. MWP has broken down the proposal into the following units:

II. Key Equipment to be supplied to the Project MWP intends to mobilize the following equipment to the Twin Lakes site:

The primary drilling rig will be one of two Foremost Industries DR24HD rigs. The DR24HD has 200,000 lbs. of pull back and 2,500,000 in-lbs. of torque. The DR-24 rig will be supplied with a crane truck to assist in rod handling and moving equipment on site.

The rig will be equipped with up to 200-ft of appropriately sized drill pipe. MWP will utilize 18” tricone bits. MWP has assumed a support truck for pipe and miscellaneous parts to complete the work.

The drilling package includes a two-man drilling crew working 10-12 hours per day on a 5 day per week schedule. Fridays are typically half days.

MWP will provide a pump rig for development, test pumping and permanent pump installation activities.

The equipment listed above will be dedicated to this project on a full-time basis and all the equipment is up to date with annual inspections and certifications. Further, the equipment has (or will be) in compliance with completion requirements.

III. Permit Submittal We have included an allowance of $500 per well for well construction permit fees.

IV. Technical Approach MWP is providing the following summary of our technical approach regarding expected geology and drilling plan. The estimated geology is expected to be sand & gravel to 125-feet.

The DR24HD will be mobilized to the site and set up to drill the new potable water supply well. The associated cuttings

tank will be arranged accordingly. All drilling derived waste will be containerized or stockpiled for disposal by MWP. MWP is not a hazardous waste hauler and has not included any such costs in our final disposal costs. We do not anticipate any issues; however, we have to mention it.

MWP will perform daily safety meetings (approx. ¼- Hr.) to review the activities of the upcoming day and deal with any previous issues.

Well #12 & #13 Construction

Well Drilling (each well)

Sonic test well drilling is not included and would be by others. Unfortunately, the company that we team with for sonic drilling backed out last minute and as such we are unable to provide a number for this portion of work.

Upon completion of the test well by others we would mobilize our equipment to the site. The DR24HD will advance 18-inch OD temporary steel well casing using the dual rotary (Barber) drilling method

to a depth of approximately 125-feet with tri-cone bit and Barber drill shoe on the 18-inch pipe. During all drilling operations we will maintain a chlorine residual in the well. Install 12-inch pipe size x 20-ft long 304 ss well screen from a depth of 125 feet to a depth of 105 feet welded to

18” OD SCH 40 steel well casing from 105 feet to approximately 2 feet above surface. Well screen to be ss plate bottom by weld ring top, Johnson, 600-ft construction, HiQ design with slot size TBD based on test well drilling. The well screen will be gravel packed between the 12” screen and the 18” hole as the 18” temporary pipe is pulled back to a depth of approximately 102 feet. A 2-ft sand/bentonite seal will be placed on top of the pack after which we will pressure grout the casing with neat cement in the annular seal. The 18” OD temporary casing will be removed during the grouting process. Casing, screen, gravel pack and neat cement to be provided by MWP. Allow 72-hours for grout to set.

Prior to grouting the well will be developed for 20 hours as specified. Gravel pack will be added to the well if necessary due to settlement during development.

MWP will perform a plumbness and alignment test on the 12” casing and screen and provide report to CUII. Collect drilling samples and submit to CUII. Install temporary test pump designed for at least 200-GPM and an anticipated setting depth of 75 feet. Electric

power provided by generator supplied by MWP. Perform 24-hour continuous pump test. Collect water samples for analysis per specifications (Fluoride, NO3 &

coliform only at this time). Remove test pump equipment and perform final disinfection. Results of all testing to CUII.

Televise the completed well and provide report and DVD copy to CUII.

Well Pump Installation

Furnish & install Baker pitless unit Model 6.5PS1214WBWE04MS4EPH with 6.5’ bury depth, 4” NPT drop pipe connection, 4” MJ discharge, sampler valve and 1” probe tube. Concrete pad and Baker Model PWE121636 or similar protective enclosure included.

Furnish & install Grundfos stainless steel submersible well pump designed for 200 GPM @ 235’ TDH (estimated) @ 3600 RPM with 20 HP, 3 PH, 230V motor. It is understood that after test pumping, the design of the pumping equipment may change. Pump to be set to a depth of 75-ft on 4” PVC Certa-Lok drop pipe with appropriately sized cable (#6 assumed).

The horsepower of the motor is proposed to be 20 HP The new well pump will be proposed to be set to 75-feet. This will also be the setting for the test pump. Water main work from each well is not included and would be by others. As with the test well drilling, our

subcontractor for all the underground work decided not to quote at the last minute. Furnish & install new motor starter and approximately 115 feet of new buried wire from the new well to the panel

in the water treatment plant – WELL #12 only. Furnish & install new post mounted disconnect and approximately 95 feet of new buried wire from the new well

to the disconnect (existing Well #9 controls to be reused) – WELL #13 only. Provide two sets of complete operation and maintenance manuals for CUII.

V. Project Schedule

Project to be completed by May 15, 2019 as per specifications.

VI. Project Management Plan

MWP will work in conjunction with CUII to modify the schedule and scope of work to address timing issues or construction issues which may arise during the performance of the scope of work. Upon completing negotiation of a written contract, the mobilization of the DR24 rotary rig would be scheduled. During

this period, the field performance plan, site specific Health and Safety Plan, Activity Hazard Analysis and initial submittals would be completed as required by MWP, plus any submittal of forms and well permits required.

During this time, a Pre-construction conference will be scheduled and site modifications for access would be discussed. Utility clearances and site access verified by owner. Mobilization of drilling equipment. Mobilization of the well materials must be staged at appropriate time to meet the site activities. It should be noted that construction noise will be heard. Work hours will be approximately 7am to 7pm.

Once actual field work begins, MWP’s on-site project managers will insure the following activities are completed: Installation of orange construction fence and necessary signage for safety. Chain link fence is not figured at this time. Installation of silt fencing as required. Daily equipment inspections, safety audits will be obtained and displayed. Daily communication with on-site personnel on daily on-site progress and to make sure concerns are addressed

immediately. There will be occasions that our plans or procedures will have to be modified to meet drilling conditions, weather conditions or unforeseen external factors. These changes will be documented in a Change Document. MWP and the CUII will have to mutually define this process to critical procedural changes vs. more mundane changes. If a critical activity is discovered to require a major change, no work will proceed without a mutually agreeable course of action has been determined.

VII. Relevant Experience MWP has extensive experience utilizing DR24HD rigs and is a Midwest leader in water supply drilling. MWP is providing the following list of several DR24HD projects completed: See Appendix B for current well completion list. MWP can provide more detailed project summaries as required by CUII. Our corporate resume is attached in Appendix B.

VIII. Unit Cost Breakdown MWP is providing the unit price estimate attached. Units are as determined by the specifications and drawings. Please note that we have modified our pricing based on the items bid and notes in this letter. Clarifications:

The equipment package quoted herein. MWP has included all fuel costs. The Drilling Derived Waste assumed to be non-hazardous waste and will be hauled and disposed of by MWP. No test

fees are included. Disposal “on-property” is anticipated. CUII to provide water for drilling as needed. We will not affect the water supply to residents. The bid is a complete package and cannot be modified without prior written authorization from MWP. Also, this is

an estimate and the final invoice will reflect actual quantities utilized in the field which are properly documented multiplied by the unit rates quoted.

A mutually agreeable written contract will be negotiated prior to mobilization. MWP is providing a two-man crew to maximize efficiency and safe work conditions. MWP has assumed working 10 hours on a 5 day a week schedule. Based on geology encountered or phase of work,

work times may need to be extended. Payment terms of net 45 days. Restoration allowance of $800 included. We are only bidding on the construction of Wells #12 & #13 at this time. We are not providing a bid for the

abandonment of Well #9 nor the maintenance of the existing wells. Underground/water main work from the new wells is not included. No prevailing wages included. No American Iron & Steel included. No builder’s risk insurance included. No ingress/egress included. IDEM site survey not included. We have included a $500 allowance for permit fees.

History - MUNICIPAL WELL & PUMP was established as a corporation in the State of Wisconsin in July 1984. In 1992 Milaeger Well & Pump joined forces with the Municipal Well & Pump to provide additional expertise going back to 1916. The company was purchased by Midwest Well Services, Inc. in September, 2000, but continues to operate under the Municipal Well & Pump name. In 2005, the company purchased Pumpstation Professionals which expanded our services in the golf course and pump station controls market. Services - The firm primarily engages in well construction, new well pumps and installation, well pump repair, well testing, well rehabilitation, and Geophysical Exploration. Municipalities and large industries throughout Illinois & Wisconsin comprise the majority of Municipal Well & Pump customers, with some customers in Indiana, Iowa, Minnesota and Michigan as well.

Crews - Municipal Well & Pump runs 8 to 9 full-time crews pump crews and 1 to 2 full-time drilling crews. • Our Pump crews are led by Andy Klemme (23 years), Tom Piscitello (22 years), Robert Kooima (12 years) and Wes

Derksen (11 years). All or our crews have received hands on training in all aspects of well and pump servicing. Well pumps of up to 1,000 horsepower and 1,200 feet in depth have been installed and serviced by our crews.

• Our Drilling crews are led by Darwin Schulz (32 years) and Mason Rens (14 years). In addition to drilling large diameter wells with depths up to 1,500 feet, they are also experienced with reverse circulation, “barber drilling” and open hole mud drilling. Safety – Municipal Well & Pump takes safety seriously and strives for “zero accidents” at all times. We are an active member of ISNetWorld, BROWZ and Avetta which monitors all our OSHA records, EMR results and Safety policies for various clients. Our Drug and Alcohol program is managed by DISA for both FMCSA and DCCHA consortium.

Bonding - Municipal Well & Pump is fully bondable for individual projects up to $4,000,000 and total projects to $8,000,000. Complete insurance coverage is in full force with limits of $2,000,000 Liability with a $9,000,000 Umbrella.

Municipal Well & Pump Resume of Key Employees

Donald W. Rens - President Don joined Municipal Well and Pump in 2001 as the VP of Operations. He was an Owner and Senior Project Manager for Westra Construction from 1984 to 2001. Prior to that, he was an Underground Superintendent. Don has more than 30 years’ experience in water and wastewater projects.

Dick Milaeger - Vice President / Senior Project Manager Dick is a third generation well contractor with extensive field knowledge in well drilling and pump service. He has been a Sales Engineer for Municipal Well & Pump since 1992 with expertise in well drilling, rehabilitation, pump engineering, water treatment and water systems design. Dick was also the Owner & President of Milaeger Well & Pump from 1983 to 1992 and was Director of Sales from 1976 to 1983. Dick is a Graduate of University of Wisconsin with a Bachelor of Science in Mining Engineering. Dick is a licensed Pump Installer in Wisconsin & Illinois; he is also a licensed Well Driller in Illinois.

Tracy Greenfield - Vice President / Senior Project Manager Tracy has been a Project Manager and Estimator for Municipal Well & Pump since 1987. He has expertise in well construction, vertical turbine and submersible pump installations and well rehabilitation. He was also a Driller & Pump Installer for Bloyer Well Company from 1978 to 1987. He graduated from the University of Wisconsin with a Bachelor of Science in Civil Engineering. Tracy is a licensed Pump Installer and Well Driller in Wisconsin & Illinois.

Marty Van Ells – Vice President / Senior Project Manger Marty began working for Municipal Well and Pump in 1986 as an Operator. In 1996, he was promoted to Field Operations Manager. Then in 1998, Marty decided to take a Plant Manager position with AJS & Associates in Random Lake, Wisconsin. In February, 2003, after 5 years, Marty decided to return to MWP for a Sales / Project Manager position. Marty has extensive experience with Well rehabilitations, vertical turbine pumps, submersible pumps and horizontal pumps.

Patrick Harrington - Sales / Project Manger Patrick joined Municipal Well and Pump in December 2012 after working for Wisconsin Rural Water Association. Patrick has worked closely with water utilities since 2004, first as a source water specialist then a trainer with WRWA. He has experience and expertise in groundwater and wells. In 1994, Patrick received a Bachelor of Science degree in Water Resources and Soil Science from the University of Wisconsin – Stevens Point and is currently working on a Master of Science degree in Environmental Science and Analysis at the University of Wisconsin – Green Bay.

John Jensen – Sales / Senior Project Manger John began working for Municipal Well and Pump in January 2017. He previously worked for the Layne Christensen Company for 20+ years as Branch Manager/Account Manager in Layne’s Wausau WI location. John has extensive experience in well rehabilitation and pump repair projects and has participated in several water exploration and well construction projects over the course of his career. John has a Communications/Public Relations degree from the University of Wisconsin-Milwaukee. He looks forward to continuing to service municipal and industrial clients in Central and Northern Wisconsin as a member of the Municipal Well and Pump team.

Brian Brodersen, P.E. – Senior Project Engineer Brian joined Municipal Well and Pump in December 2016 after nearly 23 years at Layne Christensen Company as a Design Engineer and Project Manager. He has extensive professional experience in the water industry that includes well rehabilitation, well construction, pump design, installation and repair. Brian has Bachelor of Science and Master of Science degrees in Civil Engineering from the University of Wisconsin-Milwaukee. He is a licensed Professional Engineer in Wisconsin and also holds a Wisconsin Pump Installer license and a Wisconsin Small System Operator license.

Scott De Young – CFO, Treasurer / Secretary Scott joined Municipal Well and Pump in 2002. Most recently, Scott worked for the Barnabas Foundation in Orland Park, IL as the Director of Finance and Administration. Prior to 2000, he was the Vice President of Finance and Operations for 2 different printing companies in Chicago since 1989. He also worked for 2 years in public accounting. Scott graduated from Trinity Christian College, Palos Heights, IL in 1987 with a B.S. in Account. Scott is a Certified Public Accountant.
