    Thissectionpresentsthe findingsofthe Finalknowledge, attitude andcoverage surveythatwasconductedinthe Konni District, Niger. Findingsare presentedunderthe following

    categories; Demographicscharacteristics ,Maternalandnewbornhealth, childspacing, breastfeeding, vitamin A supplementation, childimmunization, malaria, controlofdiarrhea, Acute

    RespiratoryInfections, waterandsanitationand Anthropometrics. Thissectionalsocomparesbaseline with endline findings.

    60% ofdeliveriestake place inthe Healthcenters. Itistwice the baseline line (29%). While

    there has been modestincrease inthe extensionof Health facilitycoverage between 2007 (52

    Healthpost) and 2011 (60 Healthpost) inthe Konni Districtingeneral. Inthe surveyed 60

    villages, the numberof HP and Primary Health Care ( CSI) Centershas evenremained

    unchanged. Improved Healthcare deliveryand Community Mobilization inprojectarea may

    have contributed .

    There almost 40% ofdeliverystillattended by TBA (Matrons) eventhoughtheyare

    notconsideredasskilledpersonnel. The projecthasdevotedconsiderable time inthesensitizationtoteachto TBA intheirnew role of companiontodelivery .

    when we cross where dothe Birthassisted by matronstook, we findthatonlyhalfof

    them were at Home, 50% ofthese Births assisted by TBA( 75/142) occurredin

    Health Centers. Thisis wellknow practicesparticularlyin CSIand District Hospital

    where matronsare stillused fornightshiftunderMidwife supervision. Officially

    matronsare expected eveninthose centerstoonlyaccompany Parturientto Maternity

    andhelpthe womeninpostpartum wards. Butthe realityisthat matronscontinue to

    assistdelivery whenthe Midwife actually wenttorestduringnightguards.

    88% (178/201) of mothers whodeliveredin Health Centershadapostnatalcheckwithinthefirst weekand 80% ofthem hadthe checkwithinthe dayafterdelivery. The totalnumberof

    the respondents matches with 213 whodeliveredin Health Center.

    Only 29 mothers were able toidentifythe Healthpersonnel whoperformedthe Check. Halfof

    them (51%) were done by Healthpost Worker. 4 checksdone by midwifesand 2 by Doctors.

    Itappearsclearlythat Doctorand Midwife are mostrecognizable ormay be more

    communicative (?) Than Nursesand Health Post worker, since the numberofdeliveries

    assisted by Midwife and Doctorsare the same forthe postnatalchecks.

    94% ofthe newborn were breastfedImmediately ( with 1 hour) and same proportion

    were givencolostrumsduringthe first 3 daysafterbirthand 84% ofthe newborn were

    notgivenanyotherfeedduringthe sameperiod. Thisistwice (42% at baseline)

    higherthan baseline rate.

    1.7 Malaria:

    59% (212/358) ofthe childrenhad feverinthe last 2 weekspriorthe surveyand 68% ofthem

    were treated withappropriate anti malarial ( ACT, Fansidar, Chloroandamodiaquine) within24hofthe onsetofthe fever. Thisisa moderate decrease intheprevalence of malaria from

    2008 KPC (64%) butaccesstothe treatmenthas beensignificantlyimproved from 17.5% in

    2008. Coexistence ofreportedhighprevalence of malariacasesand ITNS allegedusedin

    surveyina Nationalpatternknownin Nigerby Bothsmallscale project andthe National

    Malaria Program. Possible reasontolookatin furtherinvestigationare the time people start

    usingthe ITN atnighttime andissuespertainingtodrugresistance because eventhoughthe

    Nationalpolicyhasadoptedsince 2008 use ofcombined Artemisindrugs, Choloroquine,

    amodiaquine are still be usedas firstline therapy mostly by Ambulant Pharmacist.

    1.8 Discussions and Recommendations

    Ingeneral, findingsofthe endline surveyin Konni Districthave shown 2 majorTrends:

    improvementsin mostofthe indicatorsin Maternalandnewborncare, Preventionand

    Treatmentof infantdisease, Immunizationand Watsanandratherdeteriorationof Nutritional

    statusofchildreninthe projectareathat will be discussed more extensively.

    1.8.1 Maternal and Newborn

    On MotherChild Protectionagainst Tetanosindicator, the endline findingshasshownthatahigherproportion (81% vs 28.8% at baseline) of mothers with children 0-23 whoreceived

    atleast 2 dosesof Tetanus Toxoidvaccine before the birthoftheir youngestchild. This ishigherthanthe 62.1% found bythe 2010 Child Survival National Survey[1] forthe Region

    of Tahoua. (Statistique, Juin 2010).Increase in TT isgenerallyrelatedinimprovedattendance of Antenatal Consultation by

    women. Despite severalstockoutofvaccinesrecordedduringthe fouryearimplementation

    ofthe childsurvivalproject, the Policyof Free care accessto Motherandchildcare decreedbythe GON in 2008 andcommunitysensitizationdone byprojectcould be contributingfactor.

    The increase in TT vaccine hasalso beencorrelated withproportionate increase inaccessto

    maternal Healthservice asshown by 73% of motherpossessinga Health/vaccinationcards

    and 60% ofpregnant womenthatgive birthin Health Center. The projectareashoweda

    bettercorrelationofcompletionof ANC/TT visitsand birthinhealth facilitythanthe National

    Child Surveyof 2010 whoshowedthatdespite asignificantincrease of Antenatalvisits (55%

    in Tahoua Region where the projectislocated), deliveriesin Health facilityremainedlow

    (31%). Amongthe 60% ofpregnant women whogave birthin Health Centers, 52% ofthe

    deliveries were attended byskilledpersonnelagainst 26.4% atthe projectstartup.

    MOH indicatordonotaccountthoughamongskilledpersonnel Healthpost Workerwhoare

    not Nurse. Thisaccounts foralmost 60% ofallthe Health workersinthe 60 HP of Konni.76% ofthe deliveriesperformedinthese facilitiesusedaclean Birth Kitcomparedto 20%reportedinthe Initial KPC.

    The rate ofpost-partum checkhasnotchangedandhave evenslightlydecreased both for

    mothers ( 88% vs. 92% at baseline) andchildren (11% vs. 13% ).

    While thisresults maytranslate arealstagnationofthe postnatalcheck-up, itshould be noted

    that the responsesrate inthe Final KPC islow. Only 29 mothers were able toidentify whoassistedherinthe delivery.

    The endline surveyhasalsonotedimprovementsinthe knowledge ofdangersignsduring

    pregnancycomparedtothe baseline. The proportionof mothers withchildrenaged 0-23

    months whosoughthealth facilitycare whentheyhaddangersignsduringpregnancyhasalso

    improved from baseline (from 97% to 99%).

    The proportionof mothers withchildrenaged 0-23 months whoreceivedirontabletsdeclined

    from 96% during baseline to 92% during endline. Itistherefore recommendedthatthe

    provisionofirontabletstopregnant mothers be intensified.

    The findingsofthe endline surveyshowsimprovementintheproportionof mothers whose

    births were attended byskilledpersonal (84% from 79% during baseline). The surveynoted

    animprovementinthe proportionof mothers withchildrenaged 0-23 months whogave birthatahealth facility from the baseline (85% from 79% during baseline).

    The rate ofpost-partum check-upshasimproved between baseline and endline forallofthe

    indicators formothers (41% to 58% formothers whohadhealth facilitydeliveryand 44% to72% ofthe children whohadahealth facility birth). Similarly, the post-partum indicatorhas

    improved betweenand endline survey forchildren who were bornoutside ahealth facilityfrom 8% at baseline to 29% at endline. Theproportionof mothersandchildren whoreceived

    post-partum care from skilledhealth workerhasimproved from baseline. However, the

    proportionsare stilllow. Itisrecommendedthatall mothersandnewbornsshouldhave

    postnatalchecksatleastthree daysafterdeliveryand birth, respectively.

    The surveyhasnotedahigherproportionof (95%) childrenage 0-5 months who were

    exclusively breastfedduringthe last 24 hours from the baseline to endline survey.

    Use ofvitamin A supplementationamongchildrenaged 6-23 monthshasimproved from

    baseline and endline survey. Thisimprovementisgood forthe childrenasvitamin A is

    essential fortheirgrowth.

    The endline surveynotedanimprovementinthe proportionofchildren whoreceived measles

    vaccinationascomparedto baseline, howeverthe ratesof DPT1 and DPT3 immunizationhasstagnated. Itistherefore beingrecommendedthatchildimmunizationcampaignsare

    enhancedand expanded.

    The use of bednetsisrecommendedtoprevent malariaamongchildrenandpregnant mothers.

    The surveynotedimprovementinthe use ofinsecticide treated bednetamongchildren 0-23

    months between baseline surveyand endline survey. Proportionofchildren who were treated

    with effective anti malarialdrug withinthe 24 hoursafterthe feverbeganalsoimproved from

    baseline findings. Suchimprovementisgoodasitpreventdeathasaresultof malaria.

    Diarrheahas beenone ofthe majorcausesof morbidityand mortalityamongchildrenandtheuse of ORS isone treatmentusedto manage the disease. The endline surveynoteda big

    improvementinthe use of ORS amongchildrenaged 0-23 monthstocontroldiarrhea.

    The endline surveyidentifiedanimprovementinhealthseeking behavioramong mothers with

    childrenaged 0-23 monthsas evidenced byincreaseduse ofappropriate healthprovidertomanage coughand fastand/ordifficult breathingofchildrenaged 0-23 months from baselinefindings.

    Use ofclean waterandgoodsanitationpreventschildren from having waterborne diseases.

    The studynotedimprovementsinhouseholds withchildrenaged 0-23 months whichtreat

    watereffectivelyanduse soap forhand washingcomparedto baseline findings. The national

    nutritionpolicy forMalawiindicatesthat 21% ofchildrenunderfive yearsoldare

    underweight. The baseline findingsnotedthat 28% were underweight while the endline

    surveynotedthat 16% were underweight. Thisshowsthatthe nutritionstatus forchildrenhas

    improvedinthe district.
