• Identify who in your organization will have access to the PSO Database to: – Add, edit, delete events – View events – Generate and manage reports – Manage users – Submit to PSO

Accessing the NCQC PSO Web-Portal
• After completion of Survey Monkey: – NCQC PSO review users and notify Verge – User will receive email from Verge with unique
user name (email address) and link to create their own password
– Users with Admin privileges can edit users, unlock and delete users
– If privileges need changing just email the NCQC PSO staff

What Gets Reported?
• Incidents: patient safety events that reached the patient, whether or not there was harm involved.
• Near misses (or close calls): patient safety events that did not reach the patient.
• Unsafe conditions: circumstances that increase the probability of a patient safety event occurring.

Documents – Patient Safety Work Product, includes any discussions or documentation used for quality and patient safety improvement • Root Cause Analysis or other internal reviews • Committee Minutes • Investigation notes, including interview notes • Peer review documents • Other Patient Safety Work Product (PSWP) IMPORTANT: NCQC PSO does not recommend including Action Plans in documents submitted to the PSO! Root Cause Analysis or other event investigation should be submitted with the action plan redacted.
