Checklist Introduction This risk assessment checklist has been designed for professionals to assess the underlying factors that may increase the risk of young people becoming teenage parents or developing other sexual health issues. It assists professionals in enabling young people to recognise these risk factors and actions they can take to reduce these risks. This checklist is to be used with the aid of the teenage pregnancy Resource Pack. Completing the form will provide: An indication of the overall level of risk that a young person is at The support needs of the young person A plan to meet the needs of the young person. The assessment is designed to be used by a professional who has a concern that a young person may be at risk of early sexual activity leading to an unplanned pregnancy, contracting a sexually transmitted infection or other sexual health issues. This form is to be completed by professionals who already know a young person and build on information/concerns they already have. This form should be completed with the young person in all cases.

HOW IT WORKS

You have concerns about a young person you work with

Arrange a one-to-one meeting to go through the form together

Explain that questions concern all areas of their life, as many things affect a young person’s ability to keep themselves safe. Some of the questions could bring up difficult

feelings for the young person.

Explain the purpose of the checklist to the young person & your organisations policy on confidentiality and due regard to Child Protection Protocols

Go through each of the sections of the form encourage the young person to discuss each section in-depth, but respect decisions not to

For every question that applies put the score in the last column

Total up the score

Discuss what the score indicates and how the young person feels about this

Go back through the form and discuss how the risk factors could be reduced. Decide which risk factors the young person wants to reduce

You can suggest factors that the young person could reduce, but for the young person to make changes they need to feel the decision is theirs

If score is above 30 contact allocated Social Worker or youth offending worker share the concerns and send a copy of the score sheet that outlines the risk factors.

If the young person is not known to a statutory agency in agreement with the young

person complete a CAF refer to Integrated Support Teams via CAF Administrator.

If the score is between 20-29 contact allocated Social Worker or youth offending worker share the concerns and send a copy of the score sheet that outlines the risk factors.

If the young person is not known to a statutory agency in agreement with the young

person complete a CAF, refer to another agency for support and log the CAF with CAF Administrator.

If the score is below 20 help the young person set realistic and achievable targets for themselves.

Complete the Action planning section and set a date and time to review progress.
