REWARDS FOR DISTINGUISHED AND MERITORIOUS SERYICE.

The following has been communicated by the Director-General A.M.S. :-, As there appears to be a certain amount of misconception both as regards the dis­

tribution and conditions which govern the allotment of the above rewards, the following is published for general information :-

1. The sum of £1,400 is provided annually in Army Estimates towards rewards for distinguished service for officers of the Army Medical Service. Since 1908, only those on the Retired List are eligible.

2. The ajlove amount is distributed with the approval of the Army Council as follows :-

(a) For Surgeon-Generals and Surgeon-Major-Generals, ten rewards of £100 each .. .. .. .. .. .. .. .. .. £1,000

NOTES FROM LONDON.-Quartermaster-Serjeant J. M. Maxwell writes:-" No. 35 Company sent a stretcher squad to the Royal Army Medical Corps, Aldershot,

Annual Athletic Meeting last month to compete for the Corps Challenge Shield, open to Royal Army Medical Corps, any station. A very keen and demonstrative contest took place, with the result that the Company were victorious with 84 points, second and third teams gaining 71il and 64 points respectively. In all six teams entered. Members of the winning team were CorporalR. V. V. Egan, Privates S. J. Weeks, W.E. Dunnage and J. C. Watts. The result of the above proved sufficient excuse for holding a smoking concert in honour of the occasion. The function was taken in hand by the winners on Wednesday, July 24, and a very enjoyable time was spent by those who participated. Serjeant.Major R. Cox occupied the Chair and wielded the' hammer' to the entire satisfaction of all present. After the usual and loyal toast, 'The King,' a most entertaining programme was negotiated which caused the proceedings to go with a swing only known to such' Smokers.'

"The principal contributor to the excellent programme was Mr. 'vV. Hewitt who rendered in his appreciative tenor' Take a Pair of Sparkling Eyes,' and encored with 'Sincerity.' Later in"the evening he contributed' The Last Watch,' and a coon song 'Do you want any Coal?' which was received with hearty applause. Lance-Corporal Floyd was very amusing in 'I've never been so happy for a long, long time,' given in effective style. He was noisily encored to' We had a little Drop.' Private Phillips, 'Down the Vale,' to his own accompaniment. Lance-Corporal Thurger, 'I£.the Missus wants to Go.' Private Tunnicliff, 'Thora.' Private Ferdinando, , Kitty Dear.' (2) Oontrast the symptoms, physical signs, and pathology of mitTal stenosis with those of obstTuctive disease of the aOTtic valves. What are the indications for treatment in either case, and how are they to be met?

(3) Oompare acute peripheml neuritis with acute ante1'ior poliomyelitis with respect to etiology, symptoms, course and treatment.

(4) Mention the characteristics of diabetes mellitus. What circumstances would suggest a bad prognosis? Give the treatment.

(5) Discuss briefly the differential diagnosis, and the appropriate treatment in each case, of coma occurring in a tropical resident.

EXAMINATION FOR COMMISSION IN THE ROYAL ARMY MEDICAL CORPS.

Medicine. Case for Commentary.-Wednesday, J!Jly 24,1912. Commencing 10 a.m. (Time allowed, 1~ hours). Read your instructions.

A married woman, aged 22, who had been short of breath all her life, and had had rheumatic fever at the age of 12, was admitted to hospital on April 22, 1912. For two and a-half months her breathing had been growing worse, and one month before admission labour was indnced two weeks before term. Since then she has had a congh and attacks of dyspnCBa. She was a thin, pale, fragile-looking girl with a swinging temperature, nearly always normal-occasionally subnormal-in the morning, and running up to 103° F. or higber later in the day: the pulse-rate was rapid, averaging 128 in the morning and 160 in the evening. The respiration rate averaged 34. On seven days out of the thirty that she was in hospital she had rigors. The urine was uratic, always contained albumin, though in varying amounts, and on several occasions blood, leucocytesand casts. A blood-culture made on April 30 showed chains of minute Gram-positive cocci. There was pain in the right side of the chest followed by friction, but never any expectoration. In the left third intercostal space there was diffuse pnlsation and a systolic thrill. The cardiac dulness began at the level of the third rib and extended 1 in. to the right of the sternum. The apex beat was in the mid-clavicular line in the fifth intercostal space. There was a harsh systolic murmur all over the precordia, most intense in the pulmonary area. The liver and spleen were not enlarged. The patient went down !:till rapidly. On May 12 large hmmorrhages appeared under the skin in various parts of the body. She became semi-comatose and died suddenly on May 21.

Discuss the diagnosis and treatment of this case.

Surgery.-Case for commentary. Wednesday, July 24, 1912. Commencing 11.40 a.m. (Time allowed, 1~ hours). Read your instructions.

A healthy-looking man, aged 48, came under observation complaining of attacks of pain which commenced in childhood, but had only become urgent of late years. Whilst a boy he often suffered from bad "stomach ache," but had no special treat­ment for this; he passed through the usual infections of childhood without trouble, but has never had enteric fever. The family history was good. His more serious abdominal symptoms date back about fifteen years, and during this period he has experienced much trouble, varying in character from moderately severe colic to temporary obstruction. The former milder attacks came on gradually, and consisted of intermittent pain, referred mainly to the umbilical region, with considerable dis­tension and much rumbling of wind; they were not un frequently determined by eating a heavy or indigestible meal when cold or over tired; they did not result from con­stipation, and were occasionally warded off by a dose of castor oil, mixed with laudanum and brandy. Four or five of these attacks were of a graver type, and did not yield to the dose of castor oil; constipation became absolute, neither fmces nor flatus passing for days; vomiting appeared, and in the worse attacks became of a fmcal character; in tbe worst seizure the patient vomited for four days. On one occasion there was slight but distinct jaundice, with tenderness over the liver on deep pressure. During the last seizure the pain was less marked than formerly. ' The usual signs of collapse manifested themselves during the attacks, the patient looking pinched and drawn, and his pulse becoming rapid and feeble. The abdomen becomes symmetricalJy distended, and this is more marked in the centre than in the flanks; coils of dilated intestine could be seen, and peristalsis was sometimes visible.

Treatment for the attacks consisted in complete sta~vation, persistent enemata, and morphia as required. After vomiting for some days the symptoms yielded, tbough on one occasion peritonitic troubles lasted for a few days longer. At the time of observation the patient's condition was good, and physical examination revealed nothing abnormal.

Discuss in detail the nature of this case, and indicate the possible ,cause of the symptoms. Discuss also tbe treatment that would be required, and state how it might have to be modified according to whether the patient was in the throes of an acute attack or in a quiescent interval.
