There are two types of cholesterol, one that can make the arteries less flexible and therefore make a person more susceptible to heart attack or stroke; and the other that actually protects against heart attack. Cholesterol cannot dissolve in the blood, which means it has to be transported to and from the cells by carriers known as lipoproteins. Low-density lipoproteins, or LDL, are considered “bad” cholesterol. When too much bad cholesterol is circulating in the blood, LDL can slowly build up on the inner walls of the arteries, possibly even teaming with other substances to form plaque, a thick, hard deposit that narrows the arteries and makes them less flexible. High-density lipoprotein, or HDL, is what’s considered “good” cholesterol. That’s because medical experts feel HDL carries cholesterol away from the arteries and back to the liver, where cholesterol is then passed from the body. In addition, some experts also feel HDL removes excess cholesterol from arterialplaque, slowing its buildup as a result. And though cholesterol is commonly, and falsely, considered a dirty word, low levels of HDL actually increase a person’s risk of heart disease. A simple blood test can help men and women learn their HDL and LDL levels, and if they need to make any changes to get on the right track. TF112384

Tuberculosis Concerns Put to Rest With Test DEAR DR. ROACH: I am 22, and I am a security guard at a local store. Just recently, I detained a suspect for shoplifting. We were in a room no bigger than a small office, and she was coughing up a storm. I was in and out of the room for a half- hour or so. When the local police department showed up, she revealed that she has active tuberculosis. I did my research, and it is contagious when it’s airborne. How much exposure is necessary to catch it? What are the symptoms of TB? Is it curable? Please, I’m in need of answers. -- E.A.M. ANSWER: Tuberculosis is an infection caused by the bacteria Mycobacterium tuberculosis. The most common body location is the lungs, but tuberculosis can infect the kidneys, lining of the brain, bones and lining of the heart, among others. It is contagious by air only when in the lungs, as you said. The major symptom is cough, but weight loss also is frequent in advanced stages, giving TB its old name, consumption. The likelihood of getting infected after exposure depends on many factors, including some things we don’t know in this case, such as whether she is being treated and whether she has cavities in her lungs. The good news is that you would be considered a low-risk contact, having been in a room with her only for a relatively short period. The bad news is that it is possible to have acquired an infection. Your doctor or a public health official should evaluate you for evidence of prior infection with a skin test, which should be repeated in eight to 10 weeks. Tuberculosis is almost 100 percent curable. People with active infection usually are treated with several medications for six months. *** DEAR DR. ROACH: My wife drinks in excess of 3 liters of diet soda a day, sometimes even more. She has colitis, for which she takes Lialda, and on her last visit to her primary care physician, her blood work revealed borderline bad levels in her liver. The doctor seems to think she needs to stop “drinking alcohol,” but I have been married to her for more than 30 years, and I know it’s not drinking that is causing this. We both consume only moderate amounts of alcohol. Could it be the chemicals in diet soda? -- F.J.R. ANSWER: Three liters a day is a lot of diet soda, and that much caffeine probably isn’t good for her. However, I don’t think it is likely to be affecting her liver. Mesalamine (Lialda and others) rarely can cause abnormal liver tests -- specifically, high levels of the same enzymes that might lead a doctor to suspect alcohol use. You didn’t tell me what kind of colitis your wife has, but ulcerative colitis, a form of inflammatory bowel disease, is associated with several kinds of liver problems, including fatty liver and primary sclerosing cholangitis, which is potentially severe. I don’t think either alcohol or diet soda should be blamed until she has had a more careful workup. Walk to End Alzheimer’s Returns to Four Rochester Area Communities

The Alzheimer’s Association Rochester & Finger Lakes Region will host the Walk to End Alzheimer’s in five Rochester-area communities in the coming weeks.

The Walk to End Alzheimer’s is the nation’s largest event to raise awareness and funds for local Alzheimer’s care, support and research. Each Walk begins at 10 a.m. with registration and check- in at 9 a.m.

September 26: More than 1,000 people are expected to participate at the Walk in Rochester. The Walk begins and ends at Monroe Community Hospital, 435 E. Henrietta Road. It features a 1.1-mile and 3.5 mile route along the canal.

October 17: More than 400 Walkers are expected to participate at the Walk in Canandaigua. The Walk begins and ends at the Granger Homestead & Carriage Museum, 295 N. Main St. It features a 1.1 mile walk through the city’s historic neighborhoods and a 2.2-mile walk that extends into Sonnenberg Gardens and Mansion State Historic Park.

October 24: More than 250 Walkers are expected to participate at the Walk in Brockport. It begins and ends at the College at Brockport’s Alumni House, 142 Utica St. It includes a 1-mile route around the campus and a 2-mile route that extends into Main Street Brockport.

October 24: More than 550 Walkers are expected to participate at the Walk in Geneseo. The Walk begins and ends at Village Park on Main Street. It features a 1-mile route through town and a 3-mile route that includes the campus of the State University of New York at Geneseo.

How to make informed

healthcare decisions The typical healthcare patient has changed in recent years.

No longer are individuals putting all of their healthcare decisions into the hands of nurses and doctors. Patients are more informed than ever before and are interested in taking a more active role in their own care.

Thanks to the Internet, people are able to access information that, in the past, was not easily available. A few decades ago, medical data may have been elusive and filled with confusing jargon the layperson might not have understood without a crash course in biology. However, today there are different Web sites that clearly spell out information about certain illnesses and diseases. This means that patients may no longer be walking blindly into appointments with doctors.

Facing an illness is never easy, and the decision-making abilities could be hampered by emotions and the desire to improve as quickly as possible. Patients who do not have all of the facts may have to rely solely on the expertise of caregivers and physicians when making important healthcare decisions. By knowing the avenues of information, a patient can find assistance with careful decision-making.

While learning about an ailment is important, there are some things to consider.

• The Internet isn’t foolproof. Many online medical sites are very reliable and offer a wealth of pertinent information. They can be good starting points when seeking out information on a particular condition. It is in your best interest to visit sites that are well-known and monitored by respected

medical affiliations. Other sites may be peppered with exaggerated claims or misinformation. Therefore, do not go by online information alone.

• Don’t self-diagnose. It can be easy to use the Internet as a means to narrowing down symptoms and making assumptions about what ailments you may have. Instead of using the Internet to self-diagnose your condition, leave the diagnosis up to your doctor and rely on online information after you are diagnosed. This can improve your understanding of the condition and any potential treatment options.

• Seek other avenues of information. You should never hesitate to seek a second opinion or go to a published medical journal to find out more about a condition. You have rights as a patient to be comfortable with the advice doctors give and be as involved in your treatment as you want to be.

• Online forums could be more harm than help. Many people turn to online forums and blogs to gain more insight into particular diseases. While these forums may be good sources of support, information published on these sites could be misleading, inaccurate or unsafe. Before trying any proposed treatment, it is best to consult with your doctor.

Having a general knowledge of a medical condition can enable healthcare consumers to make more informed decisions about their situations.

Atrial fibrillation is the most common type of arrhythmia, w hich is a problem w ith the rate or rhythm of the heartbeat during w hich the heart can beat too fast, too slow or w ith an irregular rhythm. Atrial fibrillation, or AF, occurs w hen rapid, disorganized electrical signals cause the atria, w hich consists of the heart’s tw o upper chambers, to contract very fast and irregularly. According to the National Heart, Lung and Blood Institute, w hen a person is suffering from AF, blood begins to pool in his or her atria. W hile that blood pools in the atria, it is not being pumped completely into the heart’s tw o low er chambers, and this prevents the low er and upper chambers from w orking together effectively. Though AF does not alw ays produce noticeable symptoms, some people w ith AF experience chest pain or heart failure, espec ially w hen the heart rhythm is rapid. AF can increase a person’s risk of stroke, and it may surface periodically or become an ongoing problem that lasts for years.

Bought 10 face cord of wood from a local farm market farmer to use in my wood stove. I should have asked dimension prior to buying. (I have a feeling he would have lied anyway). I assumed it would be cut to fit in my side loader wood stove (like all the other wood I have bought in the past 20 years). After starting to stack I called and asked if by chance you gave me wood for a wood furnace because of how large some of the pieces were; you replied no and said you never had a complaint before. I highly doubt that. We had to get a wood splitter and split down over 2 face cord. You should be ashamed of yourself for taking advantage of people like that. You just dump and take the money. Don’t worry, every person I come in contact with will know your name.

For the one day that I actually get off from work to enjoy the outside, that I can’t because you took it upon yourself to think we in the neighborhood wanted to listen all day to your loud music, outside and inside our houses. How clueless could you have been when your buddies were screaming to talk to someone, as to how loud it was? I would like to personally thank you, for being so inconsiderate & clueless as to think it being a holiday none of your other neighbors wanted to enjoy the day also. Thanks.

It’s nice that local gas prices are going down, but I still don’t understand why you can drive 10 to 15 miles and find gas up to 30 cents per gallon cheaper. Maybe our local stations should buy gas from their suppliers.

Public school busing is a state mandated service paid for by tax dollars. It is not a ‘privilege’, as recently claimed by our suburban district. You fail to remove violent children year after year, yet you threaten students who are accused by a 5 year old of using language she doesn’t even understand. The bigger question is why do you transport 5 year olds and high school students ON THE SAME BUS? Instruct your drivers to keep babies at the front of the bus, pick them up separately, or be prepared to explain why you threaten students based on the unsubstantiated accusations of Kindergarten students.
