□ Passport: Please submit your passport with enough blank pages for visa and entry/exit stamps. **Passport must be valid for 6 months after the date of departure** Passport must be signed on the 3rd page.

□ 1 Application Form: On-line application form. All fields are required to be filled out. Please, use the following link: http://evisa.kdmid.ru to go to Russian government visa on-line application form. The on-line form cannot be edited once it has been printed. After completing on-line form, print it out and sign in black. Currently, to use on-line form you must have your Russian visa support information ready with you. Attention! In the field for "visa issuing offices" you must choose: "Russian Consulate in Toronto" (for residents of ON, AB, MB, SK) or "Russian Embassy in Ottawa" (if you reside in the Capital Region, BC or Maritimes). Please include your ID Number and password assigned to you by Russian visa application site for further visa processing.

□ 1 Passport photo: Canadian color passport photo (3.5cm x 4.5cm) on white background no older than 6

months. Front view including top of the shoulders. On white background. Neutral face expression required.

Please do not staple pictures to the application form.

□ Business Invitation: Please provide us with the original invitation document issued by the Russian Ministry of Foreign or Internal Affairs only. Visaconnection can obtain this document on your behalf. Please submit the application form available below. The visa cannot be processed without this requirement. **Please note that the original document is required to process the visa.

□ Business Support Letter: Please provide a letter signed and dated by the applicant stating the purpose of the business trip to Russia and the intended dates of travel. Address the letter too:

□ Proof of Legal Statues in Canada: Proof of Legal Status in Canada is required. Please include a copy of your Permanent Resident Card, Work Permit or Student Visa must be submitted. Please contact our office to verify whether the original document or a photocopy is required.

□ Proof of Non Russian citizenship: Applicants who used to be citizens of the USSR or of the Russian Federation and then emigrated from the USSR or from Russia must submit one of the following (copies accepted): previous Soviet passport, proof of nationality after 1992 (i.e. Kazakh passport), birth certificate. The burden of proof rests with the applicant to demonstrate they are not Russian citizens.

□ MANDATORY WAIT PERIOD: Please note, tourists may only enter Russia five business days after the application is submitted to the Consulate for processing.

□ Client Information Sheet and Authorization form: These forms should be completed in full, signed and submitted with your visa application.
