When health begins to decline, the mental, spiritual and physical toll on an individual and their loved ones can be difficult to handle alone. Community Hospice is available to all who need assistance with the final stages of a person’s illness. When the goal of treatment begins to shift from curing the illness to providing comfort, it is time to consider hospice care. This may come well before a physician indicates that the patient’s life expectancy is limited. Unfortunately, many people wait until their final days to involve hospice care. By contacting hospice early, the patient and family are informed of their options and may choose the path that will have the most positive impact on their life. The best time to learn about hospice is before you need it. We are available to meet with you and your family to help determine the right time for services to begin. The Community Hospice philosophy affirms dignity and quality of life and is dedicated to preserving the integrity and personal choice of our patients. We work hard at listening to and understanding the needs of our patients and their families. Our Team is available 24 hours a day, 7 days a week, including holidays.• Physician and Certified Nurse
Practitioner: Specialists in hospice and palliative care medicine who care exclusively for Community Hospice patients along with coordinating care with your physician.
• Nurse: Provides hands-on care and education while offering options to best manage care.
• Social Worker: Identify patient and family resources and provide emotional support while assisting in achieving goals and wishes.
• Chaplain: Along with your own clergy, the chaplain will provide support to help you and your loved ones find spiritual comfort.
• Hospice Aide: Assists you with your personal care needs while providing companionship.
• Bereavement Counselor: Facilitates grief support groups and individualized counseling for those that have lost a loved one.
• Volunteer: Serves as a companion to you and your loved ones.
• Therapies: Reduces stress, anxiety, and agitation while providing comfort and support.

Our LegacyCommunity Hospice is a Medicare/Medicaid certified agency and voluntarily participates in the Community Health Accreditation Program (CHAP). By obtaining CHAP accreditation, we open ourselves to being reviewed through self-assessments and on-site surveys which will ensure that we continue to exceed current hospice standards of care. The Community Hospice Quality Team establishes consistent clinical practices and tracks various quality indicators to ensure that we are continuously improving patient and family care.
Experience, gained over time and diversity of cases, helps build both individual clinician expertise and organizational expertise. We have the experience and the depth of resources to assist with challenging or unusual circumstances.
Medicare, Medicaid and most private insurance providers cover all hospice care costs, including medications, equipment and supplies that relate to the terminal diagnosis. This also includes all levels of care required to provide comfort and support for your loved one.
Through community donations, we are able to care for all who need our services. No one is refused hospice care due to inability to pay.
At our state of the art inpatient facility, Community Hospice patients can receive short-term, acute care for serious or urgent symptoms, instead of being hospitalized. The peaceful, homelike setting of the Hospice House allows families to spend quality time with each other, while experienced clinicians provide around the clock care focusing on your loved one’s well-being and comfort.
Community Hospice has local roots in every county we serve and were started by the community to serve the community. As one of the most respected non-profit hospice programs in the state, Community Hospice has served our community for almost 30 years and served more than 10,000 patients and families.
Over the years we have expanded our service area and added satellite offices as well as a Hospice House to better serve our patients and families.
Our doctors, nurses and other key staff members work with physicians throughout the community to provide exceptional care to patients. Community Hospice is a health care provider with 150 employees and over 550 volunteers, all dedicated to providing comprehensive, compassionate service to seriously ill people and those who love them.
