Confirmation Candidate Form should be
attached to the saint report.

DUE DATE for SPONSOR FORM IS
NOVEMBER 18. This needs to be filled out
by the chosen sponsor and both SIGNED
and SEALED by the sponsor’s parish
church. THE SPONSOR CANNOT BE THE
PARENT. A SPONSOR MUST BE AT
LEAST 16 YEARS OLD AND MUST
FAITHFULLY WORSHIP AND LIVE THE
CATHOLIC FAITH.

Comprehensive Assessment: RELIGIOUS EDUCATION Feb. 20th, 21st, 22nd, 24th
ST. DENIS-ST. COLUMBA : Archdiocesan Exam
Confirmation Name and Saint Report: Each candidate needs a saint’s name or a derivative of a saint’s name. The name given at Baptism can be used, assuming it is a saint’s name. Due date was first week in November. Final date for all Confirmation requirements is on or before Dec. 1st.
SACRAMENT OF CONFIRMATION
Rehearsal: One Hour Long (check Group for day & specific time)
Robes and Dress Code: Students will bemeasured for Confirmation robe (Wk of Dec 4th).
Robes will be distributed at the rehearsal. You are purchasing the robe and therefore it is yours to keep. The following should be worn beneath the robes:BOYS: Dress shirt, pants, and shoes; NO jeans or sneakers, or casual footwear. GIRLS: Dress, skirt and blouse, or dress pants and blouse, with appropriate dress shoes and hose; NO jeans, sneakers or casual footwear.

Robes and Dress Code (continued):Sponsors: Business attire that is modestby Catholic standards and expresses
Christian dignity is appropriate attire. The sponsor should be present for the rehearsal. If not possible, a
parent substitutes and informs sponsor of responsibilities expected during rite of Confirmation including appropriate dress.
Planning celebration afterwards:Expect approximately one hour-and-a-half to two hours for the celebration of the Sacrament of Confirmation and photo taking.
