FIRE MAIN ISOLATING VALVE

● Isolating valve is installed to transfer seawater from the Em’cy Fire pump to the hydrant

properly when using Em’cy Fire pump.

● The isolating valve is installed at previous point, where the pipe derived from the main fire pump

meets the pipe derived from the Em’cy Fire pump, and the isolating valve must be installed out-

side of the engine room.

● Em’cy Fire pump would be used when the main Fire pump is not available due to fire in the en-

gine room or damage to the fire pipe. If the isolating valve is not installed, seawater from the

Em’cy Fire pump may flow into the engine room and fail to supply the seawater with suffi-

cient flow rate & pressure to the fire hydrant in need of seawater. In addition, the isolating

valve must be installed outside the engine room because unavailability of the Main Fire pump

means it is most likely in situations that access to inside of the engine room is impossible.

1. Role of Main Isolating Valve

■ Disassembled the valve and cleaned BODY, DISC and VALVE STEM THREAD

■ After re-assemble, checked that the valve fully closed without leakage (During Fire pump running,

pressure decreased when valve was closed.)

2. Deficiency

■ After operating the Main Fire pump, the pressure in the fire line was 7 bar. The Main Isolating

valve in Foam room was closed, but the 7 bar was sighted on the upstream side of the

valve(pressure gauge).

⇒ Main isolating valve separating the Engine room and Deck failed to close. (Leakage)

3. Corrective Action (See photos in following page)

■ Remark of Isolating valve leakage check items will be added on M&I Checklist.

Significant Near-miss Investigation Report

MAIN ENGINE STARTING FAIL DURING BERTHING

1) Summary

■ During entering port of Pusan, M/E No.5 Cyl. Starting Air Pilot V/V Loop Fail Alarm occurred and failed

to start the engine for Dead Slow Astern in W/H.

■ When operated the engine as “Air run” by switching M/E control to engine Local station, engine RPM

did not change and confirmed that starting air was not supplied for No.5 Cyl.

■ After conduct M/E Turing about 90 degree, Dead Slow Ahead / Astern Test was done successfully and

M/E operated normally and changed Engine Control Station : Local → E.C.R → W/H

■ After alongside the pier, found that the cable which connected from E95-45 J/Box to M/E No.5 Cyl.

Starting Air Pilot V/V was damaged, so renewed the cable.

2) Root Causes

◇Damaged cable which connected from E95-45 J/Box to M/E No.5 Cyl. Starting Air Pilot V/V

3) Lesson learned

◇ In the vessel(M/E : WARTSILA 14RT-Flex 96C-B), M/E starting fail may occur if 14 cylinders are

fitted but no starting air is injected even with one cylinder.

◇ Starting air injection to cylinder is ordered each FCM(Flex control mod-

ule) received crank angle sensor. According to crank angle position, en-

gine starting is possible without problem. However, if starting air failed

cylinder is TDC position, engine starting could be failed.

▶ While cylinder cover overhauling, extremely have caution for damage

of electric cables.

▶ If starting air failed for one cylinder, vessel have to take action im-

mediately, engaging turning gear and turning crankg angle about 30

deg. With ahead(or astern) direction for starting.

▶ Take action immediately and report with RM-Record for below items.

1) Check protection tubes and electric cables for all cylinder starting air pilot

cable. If found cable or tube damage, repair ASAP.

2) Do visual inspection for cables near M/E cylinder cover and repair cable

if need.

3) If Starting Air Pilot V/V loop fail occurs intermittently during main engine

starting, check root cause (cable damage, tightening of pilot solenoid coil

plug DF93627) without delay.

Marine Incident Cases

As a result of observation after discovering a fishing boat approaching from the starboard of this vessel

which was navigating, a situation of a fishing boat passing to the stern of this vessel was confirmed to

at one mile from the CPA(Closest Point of Approach). After that, as the fishing boat tried to pass the bow

of his vessel by increasing speed, it was mutually agreed to pass the stern of this vessel by reducing

speed from working on fishing as as result of verifying the intention with VHF after checking the AIS

(Automatic Identification System) information. Although the precaution was not taken by confirming the

reduced speed of vessel, the radar CPA alarm sounded as the fishing boat approached by increasing

speed once again. When the reason for not keeping the promise with VHF after passing to the stern of

fishing boat by suddenly making a significant angle change to the starboard was asked, it was notified

that the promise was broken unavoidably because they were fishing.

● After mutually agreeing on the evacuation method, the fishing boat does not keep the promise

In winter, there is a high possibility of fire due to increased use & short circuits of electric

heaters in homes, offices, and ships. Please refer to the following safety regulations and take

caution for using electric heater on board.

Prohibition of Personal Electric Heater use on board

▣ SAFETY RULES FOR THE USE OF ELECTRIC HEATERS

○ Do not use personal electric heaters on board

(Unauthorized heaters, personal electric blanket/pad/sitting mat, electric iron, defective

personal electric heating device, etc.)

⇒ For unavoidable reason, use only after obtaining the Master’s permission in advance, only

provided that automatic power cut-off is attached

○ Do not use defective electric socket, plug, cable

○ When leaving your cabin for Watch/Daywork or other reasons, turn off electric devices and

pull out power plug

○ Do not insert multiple electrical appliances into one outlet or socket.

○ Periodically remove dust from electrical outlets (short circuit caused by dust increases fire

risk)

○ Do not put other electronic devices, such as cell phones, on the electric heater

(Accumulated battery heat increases the possibility of ignition)

Acute appendicitis

◆ Acute appendicitis

▶ The pocket-shaped organs located on area con-

nected from the small intestine to the large intes-

tine is called appendix. The appendicitis that we

call common is inflammation of the thin, long

vermiform appendix at the end of the appendix,

and the exact name is acute appendicitis.

◆ Causes

▶ The acute appendicitis is caused by clogging of

appendix for any reason. There are three main

reasons why the appendix is clogged.

< Causes of appendix clogging>

When the appendix is clogged, bacteria increase

inside the appendix, and by secreting toxic sub-

stances, the appendix becomes inflamed, causing

internal mucous membrane damage and ulcera-

tion.

There have been occasional cases of disembarkiation due to acute appendicitis on board. Please refer to below

information on the causes and major symptoms of acute appendicitis for personal health care.

◆ Symptoms

■ Mobile pain that not stay in one place.

■ Initially appetite decreases and nausea occur

■ Vomiting

■ Feeling bloated like indigestion and pain around so-

lar plexus

■ Over time, pain around solar plexus passes navel and

changes into pain in the right lower abdomen.

■ Pain when press the right lower abdomen with hand

■ Oppressive pain when pressing McBurney’s point

■ Slight fever and chill

■ If the appendix bursts due to hole, the pain becomes

more severe and the pain area spreads to the entire

abdomen.

■ High fever may appear, heart rate increases

▶ If neglect for more than 3 days after the symp-

toms start, the appendix will burst. This leads to

the peritonitis that pus inside the appendix spreads

through the abdomen and could lead to serious

complications such as sepsis.

Acute appendicitis has no special precautions and

it is important to maintain regular lifestyle and

proper eating habits. Also, the only treatment is

surgery, so don’t overlook the initial symptoms of

sense on abdomen distension, nauseam etc. and

make sure that your right lower abdomen

doesn’t get sick for a day. It’s important to visit

the hospital for early detection and treatment.
