List of manufacturer-owned facilities that have responsibility for manufacturing or assembly of the registered product(s)

OR Statement that the manufacturer does not directly

manufacturer or assemble registered products

ANDIf manufacturer facilities are self-declared as conformant: Documentation of a management system which

covers the listed facilities

If manufacturer facilities are certified: Copies of certification(s) to ANSI/AIHA/ASSE Z10 or

OSHAS 18001 for listed facilities

List must include all facilities that perform manufacturing or assembly functions for the registered product(s)

Statement must be properly dated and signed by an official of the company

Documentation must demonstrate that a management system is in effect which conforms to ANSI/AIHA/ASSE Z10 or OSHAS 180011 for ALL manufacturer-owned facilities included in the facilities list. Examples of evidence may include audit reports to ANSI/AIHA/ASSE Z10 or OSHAS 18001 or copies of management system documentation indexed to ANSI/AIHA/ASSE Z10 or OSHAS 18001

Certifications must be current and name the facility and certification type

This is a required corporate criterion; consequences of nonconformance may include removal of all manufacturer’s products from the registry

Somewhat difficult to

demonstrate conformance

1 ANSI/AIHA/ASSE Z10 or OSHAS 18001 are standards for occupational health and safety management systems and are available online.

List of the manufacturer’s top six suppliers which supply the principle storage device(s), principle semiconductor device(s) and printed circuit boards

For 3 of the top 6 suppliers for each component, provide the following:

Evidence of conformance to ANSI/AIHA/ASSE Z10 or OSHAS 18001

OR

Copies of certificates to to ANSI/AIHA/ASSE Z10 or OSHAS 18001

AND

If fewer than 3 suppliers provide the listed components, Statement from the manufacturer that confirms the

number of suppliers for those components

List must include the top six suppliers that supply these three components, defined either by annual fiscal or annual calendar spend

For example, management system documentation (similar to 12.4.1), server manufacturer audit report of the supplier, or third party audit report of supplier may be considered acceptable forms of evidence

Certificates must be awarded by a certification body that is accredited by an International Accreditation Forum (IAF) member association body and must clearly identify the supplier and contain a current date range. In lieu of a certificate, a URL linked to an online copy of the certificate may be provided

Statement must be appropriately signed and dated by a company official, and indicate the number of suppliers of the component if fewer than 3. Signing the provided list of suppliers satisfies this requirement

This is a corporate criterion If the manufacturer has fewer

than three suppliers for any of the listed component types, evidence must be provided for every supplier of the component type

Somewhat difficult to

demonstrate conformance

Evidence of conformance to ANSI/AIHA/ASSE Z10 or OSHAS 18001

NSF-ANSI 426: 12.4.2

Documentation of a compliance management system which demonstrates conformance with ANSI/AIHA/ASSE Z10 or OSHAS 18001 may be one acceptable piece of evidence for this requirement. As in criterion 12.4.1, if this method is used to demonstrate conformance it is helpful to provide documentation that demonstrates how the supplier’s management system documentation maps to the requirements outlined in ANSI/AIHA/ASSE Z10 or OSHAS 18001.

For 3 of the top 6 suppliers for each component, provide the following:

Copies of certifications to SA8000

OR

Copies of most current EICC1 VAP audit report

Statement from manufacturer confirming number of suppliers for listed components if fewer than 3

List must include top six suppliers that supply these three components, defined either by annual fiscal or annual calendar spend

SA8000 certification must clearly identify the facility or company and must be no older than 3 years. They must be issued by certification bodies which have an accreditation to perform certifications to SA80002

Audit reports must be issued by RBA VAP Operations Management Team 2 years prior to the product’s EPEAT registration or verification. Certificate must confirm that no major non-conformances were identified in the audit report, or that any non-conformances were addressed according to the corrective action procedures outlined in the VAP

Statement must confirm number of suppliers for the listed components and be signed by an official of the company. Signing the provided list of suppliers satisfies this requirement

This is a corporate criterion Currently the RBA does not issue

certificates as specified in this standard. Therefore, audit reports are acceptable evidence as well

Somewhat difficult to
