T = Exam Table/Scale or Medical Equipment

In a medical office, the exam table moves up and down and the scale is accessible with handrails to assist people with wheelchairs and scooters. The weight scale is able to accommodate a wheelchair. In other service locations, there is a height adjustable chair and tables. The mammography machine can accommodate a wheelchair. Lift equipment is available to assist with transfers.

Choosing a Primary Care Provider

Now that you have joined Santa Clara Family Health Plan (SCFHP), one of the first things to do is to choose a doctor or clinic for each health plan member in your family. The doctor or clinic you choose will be the Primary Care Provider (PCP) for yourself or your child. Each person in your family who is a member of SCFHP may have his or her own PCP. It does not need to be the same doctor or clinic for your whole family.

Your PCP is the one who takes care of you and is the first person you see when you need care, if it is not an emergency. Your PCP will also refer you to a specialist when needed. If you are seeing a specialist, ask your specialist for their advice in choosing a PCP.

Each PCP works with certain specialists and hospitals. When you choose a PCP you are also choosing to use the specialists and the hospital(s) with whom the doctor works. We call this a network. You will find each doctors network listed under his or her name in the full listing by city.

SCFHP has several networks: VHP Network and their affiliated clinics and Community Clinics; Physicians Medical Group of San Jose; Premier Care of Northern California; Palo Alto Medical Foundation; and a number of independent physicians who work directly with SCFHP.

How to Select the PCP Who is Right for You

One of the best ways to make sure you get excellent health care is to have a PCP with whom you can build a long-term relationship. Your PCP is someone who knows your medical background and understands whats important to you. You can choose a PCP who is a:

Family practice physician (usually treats all ages) Pediatrician (treats infants and children) Obstetrician/gynecologist (OB/GYN) (treats women) who also does primary care General internal medicine physician (treats adults) Clinic, including a Federally Qualified Health Center (FQHC), Community Clinic or an

Indian Health Service facility in Santa Clara County

You may receive care from a nurse practitioner (NP), physician assistant (PA) or certified nurse midwife (CNM) to provide your PCP services. The NP, PA or CNM must work in your PCPs office and be supervised by your PCP. If you are pregnant or you are planning to become pregnant, you also have the right to select an out-of-plan certified nurse midwife (CNM).

When you have a PCP you know and trust, you feel comfortable talking about anything. And, all your basic care, including routine exams, preventive care and treatment for illnesses or injuries, goes more smoothly. Your PCP also knows when its best to refer you to a specialist.

When choosing a PCP, there's a lot to think about. Here are some questions to ask yourself when choosing a PCP.

1. Do you want a PCP who is close to home, school or work? Keep in mind that most appointments are scheduled during the day, Monday through Friday. Consider where you are during these times. At work? At home? Helping out at your childrens school? This will help you find a PCP who is the most convenient for you.

2. Are you more comfortable with a male or a female PCP? Discussing health issues and treatments can be a very personal experience. Its important that you are comfortable talking to your PCP about any medical situation that may come up. Deciding whether you prefer a male or female PCP can ensure that you are as comfortable as possible.

3. Do you have any language requirements? Medical treatments and diagnoses can be complex regardless of what language you speak. Thats why its important for you and your PCP to communicate without any language barriers. Choosing a PCP who speaks your language will help ensure good communication.

4. Is there a hospital close to your home, school or work that you want to use? You might want to find a PCP who is affiliated with, or has privileges at that hospital.

5. Do you want to be able to access your PCPs office online? Many PCPs now offer convenient online services, so patients can use the web to view test results, renew prescriptions, request appointments and e-mail their PCP. If online services are important to you, you should call the provider to ask about their online services.

California law requires health plans to provide timely access to care. This means that there are limits on how long you have to wait to get health care appointments and telephone advice.

Cmo elegir a un proveedor de atencin primaria (PCP)

Ahora que se ha unido a Santa Clara Family Health Plan (SCFHP), una de las primeras cosas que debe hacer es elegir un mdico o una clnica para cada miembro de su familia que participe en el plan de salud. El mdico o clnica que elija ser el mdico de atencin primaria (Primary Care Provider, PCP) de usted o de su hijo. Cada miembro de su familia afiliado a SCFHP puede tener su propio PCP. No es necesario que toda la familia utilice el mismo mdico o la misma clnica.

Su PCP es quien cuida de usted y es la primera persona a quien recurre cuando necesita atencin, si no es una emergencia. Su PCP tambin lo remitir a un especialista cuando sea necesario. Si visita a un especialista, pdale que lo ayude a elegir un PCP.

Usted puede recibir atencin por parte de un enfermero practicante (nurse practitioner, NP), un auxiliar mdico (physician assistant, PA) o una enfermera partera (certified nurse midwife, CNM), que le brindarn los servicios de PCP. El NP, el PA o la CNM deben trabajar en el consultorio de su PCP y estar bajo la supervisin de su PCP.
