Film Program Questionnaire Fall 2015 Please create one document that includes the following and save it as a PDF. 1. Your name, telephone number, address and email, SFU student number or application reference number 2. Complete the questionnaire: a. Are you currently attending SFU? If yes, what program are you studying? If no, what is your highest education level and where did you attend? b. Describe any background in filmmaking, related artistic pursuits, academic study or other experiences that have prepared you for a universitylevel filmmaking program. How do you hope to build on this experience at SFU? (250 words) c. Is there a particular aspect of film/video (e.g. writing, producing, directing, cinematography, editing, criticism, etc.) that you might pursue in the future? What are your goals five years from now? Fifteen years? How do you expect to achieve these goals? (250 words) d. Describe a fiveminute 16mm. film you would like to make in your first year of the program. Your film will not have synchronized dialogue, but can include music, narration and sound effects (no music videos please). (500 words) e. Briefly analyze a film you have seen recently. Explain what you found interesting about the way it was made. Would you have made it differently? (250 words) f. Discuss a critical, historical or theoretical book or article that you have read recently, and explain how it has influenced your view of what cinema is or could be? (250 words) g. What do you feel you can contribute to your community and/or society at large? (250 words) 3. Your unofficial transcript The PDF can be made in any software you want and can be done in something as simple as Word. The goal is to have a single document combining the answers and the unofficial transcript, for the committee to read and refer to during the interview rather than several separate images and documents.
