SCHOLARSHIP PROGRAM 2016-17

Attached is the list of various scholarships which are available at this time. Please note the following

procedures as you select those for which you wish to apply.

1. Scholarships listed in this packet are available to all Milton High School seniors. Unless otherwise

noted, applications are available on the school district website at www.milton.k12.wi.us. Click on

the counseling link.

2. BEWARE OF DEADLINES. Scholarships are due in the counseling office by deadlines listed in

this packet, NOT deadlines printed on the scholarship. We have to allow time for processing and

mailing. LATE SCHOLARSHIPS WILL NOT BE ACCEPTED—NO

EXCEPTIONS! SCHOLARSHIPS ARE DUE BY 3:15 P.M.

3. The scholarship packet is updated frequently. Check emails for announcements, stop in the counseling

office or check the counseling website for updated lists.

4. Some scholarships require essays, photographs, letters of recommendation, etc. and it is THE

STUDENT’S responsibility to take care of this. Don't expect letters of recommendation to be

completed in a few days. Contact people for letters of recommendation well in advance so they have

adequate time to write the letter. Be sure you have all supporting documents asked for by the

sponsor before turning in your applications.

5. All scholarships are numbered. Please reference that number when requesting applications in the

counseling office. They are numbered in order in which they are received and if they are standard or

individual and not in numerical order on the page.

DIRECTIONS FOR INDIVIDUAL APPLICATIONS (All scholarships that follow require individual applications)

Unless otherwise noted, individual applications will be found online or a paper copy can be picked up in the counseling office if noted. Some scholarships may require additional materials such as essays, letters of recommendation, a photograph, etc. Make sure you read each scholarship’s criteria carefully. 11. MHS DRAMA SCHOLARSHIP DEADLINE: MARCH 17, 2017

AWARD: One of $250 Eligibility criteria: 1. Graduating senior planning to attend a four year college or university. 2. Must have participated in at least one theater production. 3. Must maintain at least a 2.0 GPA during the first semester to collect the scholarship award. 4. Application for this scholarship needs to be picked up in the counseling office. 12. EDWIN A. KAPEK SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: Six of $500 Eligibility criteria: 1. Graduating senior with at least 6 completed seasons of sports. 2. Must maintain a GPA of 2.75 or higher and be planning to attend a 2 or 4 year college. 3. Application for this scholarship needs to be picked up in the counseling office.

22. BOWER CITY GARDEN CLUB GREEN SCHOLARSHIP DEADLINE: MARCH 17, 2017

AWARD: Two of $1,000 Eligibility criteria: 1. Graduating senior pursuing a career in one of the following fields: horticulture, environmental studies, conservation, botany, resource management, forestry, landscape design, or other garden related field. 2. Letter of recommendation required. 3. Selection based on past performance and recommendations. 4. Pick up application in counseling office. 28. ST. MARY KNIGHTS OF COLUMBUS SCHOLARSHIPS DEADLINE: MARCH 15, 2017

AWARD: Four of $400 Eligibility criteria: 1. Graduating senior and member of St. Mary parish who will be attending a college or vocational school. 2. Selection based on scholastic achievement; need; character; and involvement in church, school, and community. 3. Application for this scholarship needs to be picked up in the counseling office.

31. HARRY H. BLISS MEMORIAL SCHOLARSHIP DEADLINE: MARCH 30, 2017 AWARD: Two of $1,000 Eligibility criteria: 1. Graduating senior who is in the top half of the class and preparing for a career in communications or journalism. Preference will be given to those who express special interest in news writing and reporting, editorial writing, editing, news photography, advertising, broadcast announcing and production, broadcast engineering, or other areas having application in communications media. 2. Essay, one letter of recommendation, photograph, transcript, and ACT score report are required. 3. Application for this scholarship needs to be picked up in the counseling office. 33. MILTON MARLIN SWIM CLUB SCHOLARSHIPS DEADLINE: MARCH 16, 2017 AWARD: Three of $200 Eligibility criteria: 1. Graduating senior pursuing any post high school program who is a present or former Marlin swimmer and a member of the high school swim team for all four years. 2. Application for this scholarship needs to be picked up in the counseling office.

38. MILTON AREA EMERGENCY SERVICES ASSOCIATION SCHOLARSHIPS AWARD: Two of $500 DEADLINE: MARCH 17, 2017 Eligibility criteria: 1. One scholarship for students pursuing a career in emergency services (police, fire, EMS), healthcare, or corrections. 2. One scholarship for children or grandchildren of current or past members of the Milton Fire Department or Milton Emergency Services. 3. Two letters of reference required. 4. Application for this scholarship needs to be picked up in the counseling office.

39. JANESVILLE MORNING ROTARY “SERVICE ABOVE SELF” SCHOLARSHIP AWARD: One of $1,250 DEADLINE: MARCH 15, 2017 Eligibility criteria: 1. Graduating senior who has been involved in community service and shows financial need. 2. Recipient should not be the child of a Rotarian. 3. Application for this scholarship needs to be picked up in the counseling office.

44. SFM FOUNDATION SCHOLARSHIP DEADLINE: MARCH 28, 2017 AWARD: Range between $1,000 and $10,000 (may receive annually through graduation – may be used at a two- or four-year technical school, college, or university) Eligibility criteria: 1. Graduating senior who is the natural, adopted, step-child, or full dependent of a worker injured or killed in a work-related accident during the course and scope of employment with a Minnesota- or Wisconsin-based employer and entitled to receive benefits under the Minnesota Workers’ Compensation Act or Worker’s Compensation Act of Wisconsin. 2. Must provide the following: transcript, verification of college acceptance, estimate of school costs, one- page explanation letter (educational goals and need for financial assistance), copy of your recent school identification card, two original letters of recommendation, brief description of nature and extent of parent or legal guardian’s injury, death certification of parent or legal guardian (if applicable), and first report of injury. 3. MEA EDUCATIONAL SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: One of $1,000 Eligibility criteria: 1. Graduating senior entering the field of education 2. Must maintain 12 credits with 2.0 GPA for the first semester. 3. Preference given to child of MEA member. 4. Must include two letters of recommendation. 5. Application for this scholarship needs to be picked up in the counseling office.

57. VIETNAM VETERANS OF AMERICA AND ASSOCIATES ROCK RIVER CHAPTER #236 SCHOLARSHIPS DEADLINE: MARCH 15, 2017

AWARD: Seven of $300 (Vietnam Veterans of America Scholarship) Two of $300 (Tim Kettle Memorial Scholarship Fund) One of $300 (Albert H. Kath Memorial Scholarship Fund) Eligibility Criteria: 1. The applicant’s father, mother, or grandparent must be Vietnam-Era Veterans who were on active duty between January 1, 1959 and May 7, 1975. Please note: service did not have to be in the Republic of Vietnam. 2. Application must be accompanied by proof of service dates (DD-214 or equivalent document).

58. J.P. CULLEN & SONS, INC. SCHOLARSHIP DEADLINE: MARCH 28, 2017 AWARD: One of $1,000 Eligibility criteria: 1. Must be entering the construction field (Bachelor of Science in Engineering/Construction or Technical College Associates Degree). 2. Photograph and brief essay required.

62. CHEWNING LEGAL, LLC SCHOLARSHIP DEADLINE: APRIL 30, 2017 AWARD: One of $500 Eligibility Criteria: 1. Must have a minimum 3.5 GPA. 2. Scholarship is for graduating high school seniors in Wisconsin. 3. Must be attending a 4-year college or university. 4. A transcript with counselor's or school representative's signature must be submitted for consideration. 5. A 1,000 word minimum/2,000 word maximum essay explaining the manner in which the applicant’s parent’s disability has impacted their life, education and goals. 6. Proof that parent(s) currently receives Social Security Disability or SSI benefits (e.g., a copy of a letter or notice from Social Security regarding the benefits). 7. Winner will be notified via mail and announced on our Facebook page and our blog page. 8. 63.ROCK COUNTY ASSOCIATION FOR HOME AND COMMUNITY EDUCATION SCHOLARSHIPS AWARD: One of $500 DEADLINE: MARCH 28, 2017 Eligibility criteria: 1. Graduating senior who is interested in going into a community service field (education, extension, nursing, police science, fire safety, social work, etc.). 2. Transcript and three letters of recommendation are required. 3. Application for this scholarship needs to be picked up in the counseling office.

65. PREMIER ORAL & MAXILLOFACIAL DEADLINE: APRIL 19, 2017 AWARD: One of $1,000 Eligibility criteria: 1. Graduating senior who lives in the WCLO-AM listening area and is entering the medical or dental field. 2. Must have a minimum 3.0 GPA. 3. Transcript and 300 words or less essay & transcript required. 4. Application for this scholarship needs to be picked up in the counseling office.

74. HELEN GRANDT BLUE NOTES SCHOLARSHIP DEADLINE: MARCH 29, 2017 AWARD: One of $500 Eligibility criteria: 1. Graduating senior who intends to pursue a career in the field of vocal music. 2. Applicant must show involvement in the community. 3. Letter of recommendation from a teacher is required. 4. Application for this scholarship needs to be picked up in the counseling office.

79. CARRIE CRYSTAL STUCKERT MEMORIAL ART SCHOLARSHIPS DEADLINE: APRIL 20, 2017

AWARD: Two of $2,000 Two of $1,000 Eligibility criteria: 1. Graduating senior who will be an Art Major (including Art Therapy programs) at an accredited institution of higher education in the U.S. 2. Must possess extracurricular accomplishments and inspirational goals. 3. Autobiography and two letters of recommendation (one from an art instructor) are required. 4. 80. BLACKHAWK COMMUNITY CREDIT UNION FOUNDATION SCHOLARSHIPS AWARD: Two of $2,000 (Presidential) DEADLINE: MARCH 13, 2017 Eleven of $1,000 Eligibility criteria: 1. You or your parents must be a current member of Blackhawk Community Credit Union. 2. Must be planning to attend an undergraduate program at an accredited public or private college, community college, or technical college. 3. Transcript and essay required. 4. 81. AB NICHOLAS SCHOLARSHIP DEADLINE: APRIL 30, 2017 AWARD: Up to 60 of $10,000 for four-year programs – renewable or $ 5,000 for two-year programs – renewable Eligibility criteria: 1. Live in Wisconsin and attend a Wisconsin public high school. 2. Participate your senior year in your school’s boys or girls basketball program, as a player or manager. 3. Must plan to attend a University of Wisconsin System school. 4. Based on financial need. 5. Apply online at www.abnicholasscholars.org. 85. JESSICA JOY ARNOLD MEMORIAL SCHOLARSHIP DEADLINE: APRIL 7, 2017 AWARD: One or more of approximately $2,000 Eligibility criteria: 1. Graduating senior who has been accepted to an accredited college or university. 2. Must have GPA of 3.0 or better. 3. Must show leadership (preference will be given to those who have been actively involved in 4-H, Future Farmers of America, or the Arabian Horse Association). 4. Include at least two letters of recommendation, one from a high school teacher and one preferably from a youth activity director or employer. 5. Must submit transcripts, ACT score report, and a copy of college/university admission letter. 6. AWARD: One of $1,000 DEADLINE: JUNE 30, 2017 Eligibility criteria: 1. Senior pursuing a career in nursing. 2. Selection is based upon academic excellence, particularly in science and mathematics. 3. WISCONSIN TOWNS ASSOCIATION SCHOLARSHIP DEADLINE: MAY 24, 2017 AWARD: Two of $1,000 Four of $1,000 One of $1,000 Eligibility criteria: 1. Graduating senior who is planning to attend a Wisconsin public or private college or vocational technical institute. 2. Must include 500-1,000 word double spaced typed essay addressing the following topic: “A federal republic

96. ZONTA INTERNATIONAL YOUNG WOMEN IN PUBLIC AFFAIRS DEADLINE: MARCH 17, 2017 AWARD: $300 Eligibility criteria: 1. Young women planning on enrolling at a college/university. 2. Applicant must show active commitment to volunteerism. 3. Provide evidence of volunteer leadership achievements, as well as a dedication to advancing the status of women worldwide. 4. Experience in local government, student government, or workplace leadership (paid or unpaid). 5. Letter of employment (if applicable) and two letters of recommendation. 6. Application available at www.zonta.org. Applications must be submitted via email to lreinhar@charter.net by the deadline.

97. HANDY ART VISUAL ARTS S SCHOLARSHIP DEADLINE: APRIL 3, 2017 AWARD: One of $2,500 Eligibility criteria: 1. Graduating senior who has been accepted to a technical college, community college or four-year college. 2. Must major in the visual arts (drawing, painting, sculpture, jewelry, ceramics, photography, computer graphics, web design, animation, game design, printing, art education, art history, etc.). 3. A minimum GPA of 3.0 4. A minimum of 4 semesters of art class at Milton High School. 5. A minimum of 2 years participation with art club or National Art Honor Society and Must be an active member of art club or National Art Honor Society during senior year. 6. Physical art portfolio demonstrating a sample of your best work must be submitted (A minimum of 10 examples). 7. One letter of recommendation required. 8. Pick up application in the counseling office.

98. SCWBA SCHOLARSHIP DEADLINE: APRIL 3, 2017 AWARD: Two of $1,000 Eligibility criteria: 1. Graduating senior who is planning on pursuing a degree in a building industry related field. 2. Minimum GPA of 2.5 3. Pick up application in the counseling office.

DIRECTIONS FOR STANDARD APPLICATION

(All scholarships that follow require the standard application) The standard application form can be found on the Milton High School website under the counseling tab. There are a few formats available depending on your computer. Fill this out and save it on your computer, this allows you to change or print multiple copies for various scholarships. Some scholarships may require additional materials such as essays, letters of recommendation, a photograph, etc. Make sure you read each scholarship’s criteria carefully. If an addendum is required please pick that up in the counseling office, it may be an essay question, financial information or something extra that is needed for that particular scholarship. It will be listed below. 9. MHS HISTORY CLUB SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: Two of $100 Eligibility criteria: 1. Graduating senior who will attend a 4 year college in the field of education, preferably majoring social studies/history. 2. Must have been a member of MHS History Club. 10. MHS FORENSICS TEAM SCHOLARSHIPS DEADLINE: MARCH 17, 2017 AWARD: One-Three of $100 Eligibility criteria: 1. Graduating senior currently active in Forensics. 13. BADGER STATE ETHANOL SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: One of $1,000 Eligibility criteria: 1. Graduating senior going into an agricultural related field. 2. May be used at any post-secondary school in Wisconsin. 3. The scholarship will be paid upon successful completion of first semester and proof of enrollment for second semester. 4. Three questions essay required, essay form needs to be picked up in the counseling office. 14. HAMMER CHIROPRACTIC SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: One of $300 Eligibility criteria: 1. Graduating senior who is planning to enroll in a full-time course of study at an accredited two- or four-year college, university or vocational technical school. Eligible majors or programs include those that prepare students for a career in the health care profession. 2. Monies will be paid after successful completion of first semester. 3. Attachment required. Pick up form in counseling office. 4. Photograph required.

15. PEGGIE SERSCH MEMORIAL SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: One of $1,000 Eligibility criteria: 1. Graduating senior who is pursuing any 2- or 4-year postsecondary degree. 2. Candidates must demonstrate a passion for helping and serving others through service to school and/or community, strong character, and selflessness. 3. Candidate must complete three questions listed in addendum. 4. Financial need is a consideration. 5. Essay Attachment Required. Pick up form in the counseling office

16. PAUL EDWARD CAMPION MEMORIAL SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: Four to Five of $3,000 Eligibility criteria: 1. Graduating senior who is planning to enroll at a college and is in need of financial assistance. 2. Candidate should be in the top half of their graduating class. 3. Should have been active in extracurricular activities such as athletics and community service. 4. Candidate will be selected based on the application, financial need, and their future goals and objectives. 5. Must include financial income information. Pick up form in counseling office. 17. JANESVILLE AREA CRIMESTOPPERS SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: One of $500 Eligibility criteria: 1. Graduating senior planning to attend a 2- or 4-year college or technical school and pursue a career in law enforcement. This would include but not be limited to police officers, game wardens, probation officers, FBI agents, jail administration, forensic science, etc. 18. MILTON OPTIMIST CLUB SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: Six of $300 Eligibility criteria: 1. Senior furthering education at a college or vocational school as a full time student. 2. Must have a 3.0 GPA or better. 3. Selection based on extracurricular involvement, including community service and school activities, and goals and purposes. 4. Transcript required. 5. Attachment required. Pick up form in counseling office. 19. JANESVILLE AREA RETIRED EDUCATORS (JAREA) SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: One of $500 Eligibility criteria: 1. Graduating senior who plans to pursue education/teaching. 2. Priority will be given to children or grandchildren of former educators who might otherwise not receive a scholarship. 3. Attachment required. Pick up form in counseling office.

21. MHS FRENCH SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: Two of $200 Eligibility criteria: 1. Graduating senior who is currently a French student and has been an active member of either the French Club or the French Honor Society. 2. Attachment required. Pick up form in counseling office. 24. EDGERTON HOSPITAL AUXILIARY SCHOLARSHIPS DEADLINE: MARCH 9, 2017 AWARD: Undetermined number of $500 Eligibility criteria: 1. Graduating senior planning to enroll in a program leading to a degree or certification in nursing, medicine, dentistry, pharmacy, therapy, etc. 2. The criteria focuses first on need, second on community and church activities, and finally on your GPA. 3. Transcript, a copy of your acceptance letter from your college, university, or vocational school, and three letters of reference required. 4. This scholarship may be renewable. 25. KEVIN EKEDAHL CANCER SURVIVOR/CAREGIVER SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: Two of $500 Eligibility criteria: 1. Graduating senior who is a survivor of cancer or a caregiver for an immediate family member (mother, father, or sibling) affected by cancer. 2. Must be planning to pursue further education/training at a college or technical college or serve in the military armed forces beginning in the fall of 2017. 3. Essay, physician verification form, transcript, and two letters of recommendation are required. 4. Pick up form in counseling office. 26. ROBERT WENHAM MEMORIAL AGRICULTURAL SCHOLARSHIP DEADLINE: MARCH 16, 2017 AWARD: One of $500 Eligibility criteria: 1. Graduating senior in good standing at Milton High School who has participated in at least three agricultural classes. 2. Transcript and photograph required. 27. ROBERT WENHAM MEMORIAL VOCAL MUSIC SCHOLARSHIP DEADLINE: MARCH 16, 2017 AWARD: One of $500 Eligibility criteria: 1. Graduating senior in good standing at Milton High School who has participated in at least three vocal music classes. 2. Transcript and photograph required.

29. HAL AND MARTHA DEAL MEMORIAL SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: Two of $500 Eligibility criteria: 1. Senior furthering education at a college or vocational school as a full time student. 2. Selection criteria will include goals, purpose, engagement in school, extracurricular activities, and community service. 3. Short essay – “What would it mean to me to receive this scholarship?” 30. MILTON BAND PARENTS ASSOCIATION SCHOLARSHIPS DEADLINE: MARCH 16, 2017 AWARD: Ten of $200 Eligibility criteria: 1. Must be a band member in good standing and be pursuing a post-high school education. 2. Based on commitment to the MHS band. 3. Attachment required. Pick up form in counseling office. 34. MILTON GARDEN GUILD SCHOLARSHIP DEADLINE: MARCH 16, 2017 AWARD: One of $250 Eligibility criteria: 1. Graduating senior who will be attending a college or vocational school. 2. Must have a passion for the plant sciences and be pursuing a career in botany, horticulture, or other garden related field. 3. One letter of recommendation required. 36. MHS STUDENT COUNCIL SCHOLARSHIP DEADLINE: MARCH 17, 2017 AWARD: Five of $200 Eligibility criteria: 1. Student Council membership is not mandatory to receive this scholarship. 2. Graduating senior planning to further his/her education at a college or vocational school. 3. Based on leadership and active participation in student body functions. 37. VOLLEYBALL BOOSTERS SCHOLARSHIP DEADLINE: MARCH 16, 2017 AWARD: Three of $200 Eligibility criteria: 1. Senior furthering education at a college or vocational school as a full time student. 2. Applicant must have been part of the MHS Girls Volleyball team all 4 years.

40. MILTON HOUSE SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: One of $350 Eligibility criteria: 1. Graduating senior with a minimum GPA of 2.0 planning to enroll in a full-time course of study at an accredited two- or four-year college, university or vocational technical school. 2. Applicants should have demonstrated commitment to the community in Milton. 3. Applicants who are members of the historical society or perform volunteer service for the society will receive preference.

Rankings are given based on: 1. Volunteer of the Milton House 2. Community Service (outside of high school) 3. Extra-curricular activities at the high school 4. Awards and Honors 5. GPA

4. Letter of Recommendation is required. 47. MILTON SHOW CHOIR SCHOLARSHIP DEADLINE: MARCH 16, 2017 AWARD: Undetermined of $200 Eligibility criteria: 1. Senior furthering education at a college or vocational school as a full time student. 2. Applicant must have been part of show choir for a least one year. 49. JOSH PELTON MEMORIAL SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: Three at $500 Eligibility criteria: 1. Graduating senior who has been active in multiple extra-curricular school activities and has held at least a part-time job. 50. MILTON BASKETBALL BOOSTERS SCHOLARSHIP DEADLINE: MARCH 15, 2017 AWARD: Four at $250 Eligibility criteria: 1. Graduating senior who has been active in basketball for at least 2 years. 2. Essay required. Pick up form in counseling office. 67. JESSICA SZERLONG “BRIGHT FUTURE” SCHOLARSHIP DEADLINE: MARCH 16, 2017 AWARD: One at $250 Eligibility criteria: 1. Graduating female senior who has been involved in at least 3 years of soccer, cheerleading or choir. 2. Student with a strong work ethic, who values team work and will use those characteristics to make an impact on what they do beyond high school. 3. 500 word or less essay required. Essay should be about an event in their life that has helped them realize the value of team work and working hard to achieve a goal, and how that will help them in their future endeavors.

76. TERRY MCCANN ALUMNI SCHOLARSHIP DEADLINE: APRIL 7, 2017 AWARD: Five of $300 Eligibility criteria: 1. Graduating senior. Two of the scholarships will go to students entering the field of education. 2. Attachment required. Pick up form in counseling office. 77. AMERICAN ASSOCIATION OF UNIVERSITY WOMEN SCHOLARSHIP AWARD: One or Two of $500 DEADLINE: MARCH 20, 2017 Eligibility criteria: 1. Graduating senior who is planning to attend either a college or a technical school. 2. Photograph required. 3. Attachment required. Pick up form in counseling office. 82.
