Health

Called the FlexChoice Program, Scottsdale Healthcare’s flexible healthcareprogram allows you to choose from a variety of insurance options to customize the plan that meets your family’s healthcare needs and budget.

Medical PlansCo-Pay Advantage Plan - pays benefits if you use any provider, but offers afinancial advantage for using network providers. Levels of coverage varyfrom 100% to 80%.Consumer Choice Plan - is a pay-as-you-go plan that offers lower premiumsin exchange for higher co-pays for services when you use them. Health Reimbursement Arrangement Plan - is a low-premium plan with ahigh annual deductible. To offset a substantial part of the annualdeductible, you have a tax-advantaged fund contributed by ScottsdaleHealthcare from which to pay your initial covered expenses.

Wellness BenefitAll three medical plans have a comprehensive wellness benefit that pays100% of covered annual wellness visits. Plus staff members and spousesreceive an annual benefit to pay for wellness education programs offered atScottsdale Healthcare.

Health & Wellness ProgramGet healthy and stay healthy with the professionals of ScottsdaleHealthcare’s Occupational Health program. Free to all staff members, thisprogram provides health assessment tests, personalized plans, coaching,workshops and follow-up calls on a schedule you design.

Prescription Drug PlansTwo prescription plans offer flexibility and choice. Both plans offer discounts on mail-order and 90-day prescription services.Premier Plan - is a three-tier plan that offers lower co-pays for generic prescription drugs and higher co-pays for non-formulary drugs. Standard Plan - in exchange for lower premiums, this plan allows you to payas you go with an annual deductible before the plan’s three co-pay optionsbegin.

Fitness CentersScottsdale Healthcare offers staff members access to its CardiacRehabilitation exercise facility and to the Essential Touch fitness center. Aportion of the low monthly fees is reimbursable to staff under the medicalplan’s Wellness Benefit.

Dental PlansComprehensive Dental Plan - offers full coverage, including orthodontia. Preventive Dental Plan - offers basic coverage at a lower cost to staff mem-bers. Both plans carry an annual deductible and offer limited coverage forout-of-network services.

Vision PlanWith the option to use two nationwide networks of vision centers, you willreceive significant discounts on vision services, glasses and contact lenses. Acredit toward LASIK surgery is even included.

Medical Flex Spending AccountYour Medical Flex Spending Account allows you to pay for out-of-pocketmedical, dental, vision, and pharmacy expenses with pre-tax dollars throughthe convenience of a debit card.

Family Resource ProgramReceive free referrals for childcare, adult/elder care and pet care services.This program also provides free adoption information and referrals toadoption services.

Adoption Assistance BenefitScottsdale Healthcare’s Adoption Assistance benefit reimburses you up to$3,000 for expenses related to adopting a child.

Sick Childcare • Kids Care. When your child wakes up sick and cannot go to school, but

you absolutely must go to work, Kids Care is there to help. On-campuscare for your mildly-ill child costs only $12 per day. Limits apply.

• Sick Childcare Services. Through the free referral service, you can locatea qualified provider care for your ill child in your home for $12 per day.

Back-Up Childcare and Adult/Elder Care You have help when you have to go to work and the regular childcare orelder/adult care provider is vacationing, sick, closed or otherwise unavailable.This service provides back-up childcare and elder care subsidized in part byScottsdale Healthcare. The cost to staff members is only $12 per day.

Childcare CenterThe new Scottsdale Healthcare Childcare Center will provide care for staff members’ children from infant to kindergarten age and is centrally locatedon the Shea campus.

Survivor Financial Counseling BenefitThe loss of a spouse or parent can place a complex burden of responsibili-ties on the survivors. Full and part-time staff members may use a financialcounselor to get help with their short-term and long-term concerns.Scottsdale Healthcare will reimburse staff members up to $3,000 for thecost of a financial counselor after the death of a parent or spouse.

Pet Care Referral ServiceAre you looking for information on veterinarians, pet grooming and boardingservices? Do you ever need a pet sitter while you are vacationing? Receive freeinformation and referrals on pet services of all types.

Family Medical Leave / Medical Leave / Military Leave / Personal LeaveWhether unforeseen or planned, you may need to take time off to attend tomedical, military or family matters. Scottsdale Healthcare offers a generousleave policy - in some cases paid leave - that gives you peace of mind.

Dependent Care Flex Spending AccountIf you pay for childcare while you work, you can reduce your taxes whilepaying for qualified childcare with tax-free dollars. Through easy payrolldeduction, you can set aside dependent care costs into your Flex SpendingAccount and reimburse yourself from this account when you pay your caregiver. Finances

The 403(b) Retirement Security PlanSave for retirement on a tax-deferred basis. Save as little as 2% or as muchas 50% of your base earnings, up to the maximum set by the IRS.Scottsdale Healthcare matches your savings up to the first 4% of eligiblecompensation after 90 days of service (must be scheduled to work 40 hoursa pay period). Choose from a wide variety of investment funds.

529 College Savings PlanThis is a tax-saving way to help you save for your family’s college education expenses. Earnings on this plan are non-taxable at distribution time, pro-vided the money is used for tuition.

Savings BondsU.S. Savings Bonds offer a way to invest for the future through easy payroll deduction.

Credit UnionBank where you work at convenient Desert Medical Federal Credit Unionlocations on each campus. Members enjoy competitive rates on savingsplans and a variety of other services. Any staff member can open a savingsaccount. The bank card can be used at any credit union ATM without a fee.

Take Charge AmericaFinancial stress affects all aspects of life. This service provides assistance inthe form of financial education, confidential counseling and debt manage-ment. The goal is to help staff members gain control of their finances, payoff their consumer debt, save for the future and be debt free.

Direct Deposit AdvantageStaff members are entitled to free checking with no minimum balancewhen signing up for direct deposit at any of these banks: Bank of America,Chase Bank, Compass Bank, Wells Fargo, AmTrust Bank and WashingtonMutual. Several of these institutions offer free on-line services, financialeducation and other services. Visit the cafeteria during any third full weekof the month to speak with a bank representative.

Short Term Disability and Long Term Disability CoverageAfter one year of service, you may be eligible for salary replacement in theevent you are unable to work due to a non-work-related injury, illness, orcondition. Disability payments begin on the eighth day of your disability orthe first day of inpatient hospitalization or surgery.

Worker’s Compensation - Plus PayWorker’s compensation laws require employers to replace a portion of yoursalary if you are injured on the job and cannot work. However, if yoursalary exceeds the statutory maximum, your benefit will lag behind.Scottsdale Healthcare’s PlusPay program ensures you will receive 60% ofyour average monthly wage for up to 12 weeks.

Basic Life and Voluntary Life InsuranceAs a full-time staff member, Scottsdale Healthcare provides you with basic lifeand accidental death and dismemberment (AD&D) insurance equal to twotimes your annual salary at no cost to you. Part-time staff members receive a$10,000 policy. Full time staff members may purchase Voluntary LifeInsurance in addition to basic coverage, and may insure their dependents.
