ECC operator will:
Find out who is calling.
Obtain the sequence of events, including
Status of victims: symptoms, how many injured, hospitalized
Was building evacuated?
Is hazmat on scene?
Find out who is in charge of the scene.
Email the on call list of personnel (SDO, EPI, PHP
Coordinator, Regional Director, EH Manager).
Log all event information in ESS
SDO will then:
Notify lab on-call person of the incident
Determine if FBI/Arkansas State Police thinks it is a credible
threat or if it has a potential health risk. If so, ASP will
arrange transportation to ADH (ASP may coordinate with
FBI).
Ensure that chain of custody has been maintained.
Begin Notification System.
Determine when the sample will be arriving.
NOTE: We CANNOT transport.
Get final lab results and begin reverse notification process.

Be sure to follow up until the incident is closed.
ECC Operator will find out:
What chemical/fuel was spilled?
How much chemical/fuel was spilled?
What is the location (physical address) where the chemical/fuel was
spilled?
Has anyone been evacuated?
Is there a fire?
Who is responding? i.e. ADEQ, Hazmat
Are there any deaths?
Find out who is in charge of the scene. (On-Scene Incident
Commander info)
Critical infrastructures nearby?
Near schools, hospitals, at-risk populations?
Is the chemical/fuel going into a waterway?
Has the hazmat team been called?
What waterways are involved?
If waterways are affected: Call engineering.
Email the on call list of personnel (SDO, EPI, PHP Coordinator,
Regional Director, EH Manager, Shirley Louie).
SDO will then:
If site is contained- no further action.
If site is not contained- maintain contact with engineering person on-
call until incident is contained
Once incident is contained begin the reverse notification process
If meets the notification threshold, initiate Notification System.

Hazmat Event is contained
No action required. ECC operator will document information in ESS.
If nothing needs to be done by ADH. (No waterways affected, someone else is cleaning the spill or its contained etc.) The ECC Operator will send an FYI Email to EPI On-Call, Shirley Louie, Regional Preparedness Coordinator, Regional Directors and Regional Environmental Health Manager in the region where the incident occurred.

Find out what disease is suspected/diagnosed.
Determine many patients are suspected/diagnosed with the disease.
Report any deaths or patients in the hospital.
Report which hospital.
What is the phone number of the hospital?
Find out who the physician in charge is and their contact information
Email the on call list of personnel (SDO, EPI, PHP Coordinator,
Regional Director, and Physician).
SDO will:
Determine if this is a reportable disease? Check the reportable disease
policy in this manual. (page 10)
Call the Physician On-Call.
o Physician will call/advise calling the EPI On-Call Person
Make sure the Communicable Disease program receives the
information so they can follow up.
Email information to Linda Gladden and Dr. Dirk Haselow
If samples are going to arrive in the lab - Call Lab On-Call person.
Initiate the Notification System. If a reportable communicable disease
occurrence or outbreak involves Anthrax, Plaque, Viral Hemorrhagic Fevers,
SARS, Emerging Threat Agents, and Smallpox. (page 5)

Anthrax Threat:Guidelines for Arkansas Law Enforcement
• The Public Health Lab is the sole facility certified to conduct biological testing for anthrax.
• The FBI and the Public Health Lab are involved only when there is a credible or implied threat.
• If having concerns over a package, place in plastic bag and throw away.
• Hoaxes aggressively investigated by FBI in violation of Federal statutes.

• Level One Threat:– is an incident in which there is a credible or implied
public health threat of anthrax.
• Level Two Threat:– requires consultation with and a threat assessment
from the FBI in order to determine if the threat is deemed credible. ASP or FBI will have the discretion to transport the suspect items to the Public Health Lab.
