On the following page you will provide information for the six subject areas that are part of the 16 high school core competency coursework

requirements: English, Math, Lab Science, Social Science, Second Language, and Fine Arts or Career & Technical Education (CTE). For more details on the

16 high school competency requirements refer to http://admissions.arizona.edu/how-to-apply/abor-course-competencies.

Indicate the coursework you have completed and grades you have received. Also, list the courses you plan to complete during your senior year of high

school. Include any college credit(s) you have completed or will complete by the time you graduate from high school. You must obtain a copy of your

high school transcript and use it to complete the self-reported form. Your self-reported grades should match those displayed on your transcript. For

example, if your transcript only lists final grades, then you must report final grades, even if you know your semester grades. The official transcripts will

be used to verify the information you self-report. If you provided inaccurate information, your admission may be revoked or your enrollment canceled.

You are required to indicate at least which courses you have in progress for any of these subject areas.

If you provide us with each of the 16 high school core competency coursework requirements in this section, your admission decision will be based upon this information. You will not need to submit a transcript until after your graduation from high school. If your final high school transcript indicates that you do not meet the minimum standard for admission to the University of Arizona, your admission may be rescinded.

If you only provide your senior year coursework or do not provide complete information in this section, you will need to send us an official transcript.

If you fill out this section completely you will receive a faster admissions decision, and the self-reported grades you provide will replace official high school transcripts during the initial admission review process. Key to filling out High School Coursework Table:

Grade Level – What year in High School did you or will you complete this course? Options include: Pre-High School, 9th grade, 10th grade,

11th grade, 12th grade.

Course Rigor – Indicate whether the course is Standard, AP (Advanced Placement), IB (International Baccalaureate), Advanced/Accelerated,

Honors, Dual Enrollment/College Credit, Remedial.

Term Schedule – Indicate the term schedule reported on your transcript. Options include: Final Grade Only, Block, Quarter, Semester, Trimester.

Grade – Please fill out the appropriate number of grade boxes according to your term schedule. For example, if your Term Schedule is Quarter,fill out Grade 1, Grade 2, Grade 3, and Grade 4. If you are currently taking the course, then mark it as IP (In-Progress) for each term you will be

enrolled in the course. If your high school utilizes a non-standard grading scale (such as numerical grades) or a non-standard semester schedule

(such as trimester or block schedules) please see the FAQ page http://admissions.arizona.edu/faqs/freshman#srg.

The number of units/years required per subject are: English (4 years), Math (4 years), Lab Science (3 years), Social Science (2 years), Second

Language (2 years), Fine Arts or Career and Technical Education (CTE) (1 year). Please include all courses in each subject with at least the number

of required units/years. You can include more than the minimum of required units. If you did not take any courses in a subject, then in the course

title write in the following: no courses taken.
