It is very important for gynecologists and fertility specialists to know, how to interpret the report of semen analysis and to check the authenticity of the reporting lab and andrologist/technician. Abstinence: �e required abstinence for semen collection is 2-7 days.

Semen collection: �e semen sample should be collected in a private room near laboratory. �e man should follow clear instructions for semen collection.

Pass urine if bladder is fullWash his hands and penis with soap, to reduce the risk of contamination of the specimen with commensal organisms from the skinRinse away the soapDry hands and penis with a fresh disposable towelEjaculate into a sterile, wide-mouthed and well labeled container�e sample should be obtained preferably by masturbation �e sample should be complete and any loss of fraction to be reported

Time intervals of importance: �e time from semen collection to the reporting laboratory should be 30-60 min. �e time from semen collection to start of analysis should be less than 3 hours. �e maintenance of temperature between 200C–370C during the time interval is very important.

Reference range: According to the latest WHO laboratory manual[1] for examination and processing of human semen, the lower reference limits (5th centiles and their 95% con�dence intervals) for semen characteristics are given in Table 1.

Terminology: While it is very important to know what is normal, equally important is to know what is abnormal and what the clinical implications are. Table 2 represents the terminology to describe the semen samples with values lying outside the reference range.

Semen characteristics are highly variable both within and among men, and are not the sole determinants of a couple’s fertility; the ranges therefore provide only a guide to a man’s fertility status.

Semen parameters that lie within the 95% reference interval do not guarantee fertility and men whose semen charac-teristics fall below the lower limits given here are not necessarily infertile.

�ere may be regional di�erences in semen quality, or di�erences between laboratories.

What to do a�er abnormal semen report?

�e process of spermatogenesis in male genital tract takes approximately 74 days to complete. Hence any abnormality in the semen analysis test to be noted and repeat semen analysis should be ordered at the interval of 4 weeks. Any abnormal semen report should be interpreted in concordance with clinical picture. If the abnormality is persistent in repeat semen samples then only the report should be labeled as abnormal and further testing such as advanced semen analysis, sperm function test, hormonal pro�le or imaging studies as applicable may be o�ered.
