Auburn Mountainview High School SLC/Project Plan Designed to Prepare Lions to successfully pursue their individual goals. The Portfolio is a graduation requirement of Auburn Mountainview High School and meets the State of Washington High School & Beyond plan requirements. Portfolio activities and products at each grade level are designed sequentially to help students produce the final culminating project portfolio. Students shall have the following components, approved by their advisor, and placed in their portfolio by the end of each year. Senior Portfolio: Table of Contents Learner Box Sheet- Sheet Typed, Completed Boxes Checked off and Printed. Best Works- Three: Typed, Printed and Signed. Artifacts Attached. Academic History- Transcript: Senior Year 1st Sem, Test Scores: WASL, ACT, SAT Citizen Exploratory Record- Complete 12+ hours. All forms signed. Transition Resume- Final Edit Typed and Printed. Letter of Recommendation Form- Sheet Typed and Printed. (No letters needed) Post HS Action Plan- Sheet Typed and Printed. (Pick only one of four sheets) Portfolio TOC- All boxes checked, Form Printed. Autobiographical Writing- Final Edit Typed and Printed. Title Page- Name/Grade Date/Adviser, Typed. (May be on front cover of binder) Evaluation Portfolio Rubric- Print. (Advisor will fill this out) Power Point- Completed. E-mailed to Advisor. Printed (Six slides per page) Presentation Rubric- (Advisor will complete and place form after your presentation) 4-Year Plan- Sheet Typed Printed.
