3.1. Seniors Health Community Seniors Addiction Mental Health (CSAMH)

Updates provided on activities underway across CSAMH Seniors Health:
Home Care: policy work underway on Home Care Client Charges Guideline; Home Care Services Policy; Self-managed Care Policy; Added Care/Extraordinary Care Policy.
Supportive Housing & Residential Living: Integrated Health and Housing Services Strategy draft report completed, will come to SCC PAC Fall 2020.
Recording and Surveillance Devices Guideline under review by AH Privacy.
Continuing Care Technology & Innovation Working Group making recommendations regarding past pilots and push/pull technologies.
Medical Equipment and Supplies with AHS Clinical Ethics, final report and recommendations expected by December 2019.
Access to Designated Living Option Policy and Procedure launched June 2019. Significant increase in assessments in community, seeking to complete one-year review regarding process and client experience.
Continuing Care Facility Directory has been publically available for three months; great response from clients and families,

operators. Continuing Care Health Service Standards (CCHSS) audit results will be posted by AH with a link from the Director to AH page when publically available. Next steps include addition of addiction/mental health (AMH) residential services, hospice.
Conversations Matter Guidebook available, included in Green Sleeve package provided to clients and families. Walks individuals through the process of advance care planning and provides information about Goals of Care in layman’s terms. Being translated into seven different languages.
Patient’s Death in the Home Setting Guideline one-year evaluation complete.
Continuing Care Connection website launched March 31, 2019 to very positive feedback.
AHS Connect Care Wave One launches November 2019, much education required for staff and physicians. Electronic medical record (EMR) will improve transitions with care plans carrying across settings (from acute care to community) and information will flow to primary care physicians. Connect Care will roll out in nine (9) waves through Summer 2023.
Recommendation from Council to translate into Cree, other Indigenous languages as locally appropriate.

3.2. Seniors Health Strategic Clinical Network™ (SCN™)

Quality Improvement Project to Address Pain Management and Depressive Moods in Long Term Care and Designated Supportive Living three-year initiative seeking to improve quality of life for residents.

Primary Health Care Integrated Community Geriatric Service Initiative has held nine local events in eight rural communities seeking to education physicians to better support older people in community.
Elder-Friendly Care workshops held in Edmonton and Calgary; provincial strategy will be complete in the Fall of 2019.
Community-based Innovations in Dementia Care Grants 2019 new $1M grant is in-place to support implementation of community-based innovations in dementia care. Della Robertson is on the project evaluation/ advisory committee.
Research activities underway include identification of Quality Indicators to monitor/report on SH SCN™ activities that impact it’s Transformational Roadmap. Completed efforts include Priority Setting Partnerships and Alberta Dementia Research Futures.
Appropriate Prescribing and Medication Use for Older Albertans see 3.8 for Council member participants’ update.

Creating recommendations specific to congregate living; linking with Continuing Care Technology Innovations (CCTI). Pilots undertaken with GPS and call-assist devices. Reviewing previous reports and evidence – not much practice change since then. Looking at technology to alert client, also at technology for the client to interact with.
3.5. Continuing Care Policy Review & Implementation Panel

Review of AHS Safe Bathing Temperatures and Frequency Procedure for Provincial Continuing Care underway. Seek to meet safety requirements while satisfying resident preference.

Self-managed Care Policy link to survey circulated 20 Sep 2019; SMC Policy Revisions document circulated 25 Sep 2019 for review and discussion at 05 Oct 2019 SCC PAC meeting.
Self-Managed Care (SMC) is a funding option for clients with stable health care needs that may not be met by locally-available Home Care service providers and for whom traditional continuing care congregate living options are not an ideal solution. Discussion and feedback will be provided to CSAMH Seniors Health.
Recommend strengthening definition of ‘Accountable Leader’ within Policy.

Crystal Stewart reported Community Engagement Social Media is shifting focus to local stories, either geographic or area of focus. Members are encouraged to share ideas for Social Media posting.
Additional ideas can be forwarded for consideration. Content will be vetted and not all submitted will be posted.
Members are asked to provide a brief caption describing the event/activity when providing link or photo.

3.8. Appropriate Prescribing and Medication Use Strategy for Older Albertans Advisory Committee

Appropriate Prescribing Medication Use Strategy for Older Adults presented to SCC PAC Spring 2019; feedback to Discussion Paper incorporated.
High volume of medication use by seniors identified; more prescriptions mean more side effects, interactions, hospitalizations and cost.
Physicians identified potential work in acute care, primary care and long-term care/DSL. Implementation is dependent on funding.

John Erkelens and Susan Sommerfeldt participated in education sessions including self-assessment checklist and staff training/just-in-time education that utilized design thinking posing ‘how might we’ questions to stimulate conversation. Discussion around use of Game Theory that looks at how players collaborate and navigate.

4.1. Advance Care Planning Alberta – pan-Alberta meeting to co-create a public action plan

18 Sep 2019 at Edmonton Federal Building – seeking to integrated Green Sleeve for all Albertans. Representatives attended from across the province, seeking to develop Action Plan – funder for future work remains to be confirmed. Discussion around opportunities to share Green Sleeves recommended focus on individuals more imminently requiring Personal Directive. Need to consider differing cultural components around death. Centre for Public Legal Education Alberta (www.cplea.ca) provides information on personal directives, power of attorney and wills – Green Sleeve. Next steps include scheduling of next meeting; activities on-hold pending release of budget.
4.2. Government of Alberta Financial Committee recommendations and potential
The McKinnon Panel findings have been released; Ernst & Young hired to complete review of AHS and AH to identify opportunities to streamline and reduce costs. Budget will be released in October; 2020 Budget will come in the spring.
