PARENTS: to be signed I understand ___________________ is participating in competitive high school athletics on the Boys Basketball Team. The playing and practice schedules have been outlined for me and I have clarified and resolved any questions. In team participation I realize my child has accepted commitments and responsibilities. I understand fulfillment of these responsibilities is not only part of team play but also enhances the experience for my child and his teammates. I agree to support these efforts to the best of my ability. I realize one way to do so is to attend games and volunteer with the basketball boosters. In attending contests I will make every effort to empower and uplift the team through positive cheering. I understand Skyline Basketball is a merit-based program and that playing time is earned through practice, performance, attitude, sportsmanship, and synergy with teammates. PLAYERS: to be signed I understand I have been selected as a participant for the 2018-19 Skyline High School Boys Basketball Team. I further understand this is an extra-curricular activity and that participation is a privilege not a right, and therefore revocable. A copy of the game and composite schedule has been outlined for me and I have reviewed both. I understand I am making a commitment to attend all team events and do so in a timely manner. I understand playing time is solely up to the coaching staff and is based on merit – including but not limited to my performance, attitude, attendance, sportsmanship, and communication which includes body language. I understand the one prime rule for the team this season – avoid doing anything that may embarrass my team, my school, or myself. This includes breaking the law, using drugs, vaping, and verbally or physically abusing classmates, teammates or opponents. I agree to abide by this rule. I understand the value good sportsmanship has in this program and I agree to compete to the fullest extent of my abilities WHILE exemplifying superior sportsmanship. I realize I will be expected to have exemplary performance in the classroom, and the community. I realize there may be schedule conflicts that occur during the season due to academic and health reasons. I will do everything in my power to communicate conflicts to coaching staff in advance. I also understand that family trips/travel will not be an “excused” absence and I will need to re-establish my role on the team upon my return. Player Name: Signature: Date:
