Is this an OSHA Recordable injury? Was the injured worker being paid for work or performing work on
behalf of the employer at the time of the event or exposure? Was the injured worker’s presence at the place of the injury exposure
related to a condition of employment? Was cause of incident within control of employee and/or employer?

1. Fatalities
2. Amputations (involving bone)
3. Concussions and/or cerebral hemorrhages
4. Injury to internal organs
5. Bone fractures excluding fingers and toes, compound bone fractures for fingers and toes
6. Crushing injuries involving bone
7. Tendon and ligament tears
Serious Injury Incident Rate (SIIR)
Severity Criteria 8. Herniated discs (neck or back)
9. Lacerations – 3 to 4 inches in length resulting in severed tendons, a deep wound requiring internal stitches, etc.
10. 2nd (10% body surface) or 3rd degree burns
11. Eye injuries resulting in eye damage or loss of vision
12. Injections of foreign materials e.g. hydraulic fluid
13. Severe Heat Exhaustion and all heat stroke cases (Severe Heat Exhaustion cases are those where all the following symptoms are present: profuse sweating, nausea and confusion. If fainting occurs due to the heat exposure then this is an automatic severe case.)
14. Dislocation of a major joint (hip, shoulder, elbow, etc.)

Question Employee sustained a "slight" fracture to his tibia, didn't lose any time from work and wasn't on restricted duty. Just over the counter meds. Is this considered as a serious injury?

Employee sustains a crush injury to two fingers. During surgery it is determined that the fingers should be amputated because the severity of the injury. Thus, is this injury classified as a crush or amputation. Are the severity criteria determined from the result of the traumatic injury only, or the results of medical treatment for the injury?
