Welcome Welcome to MercyCare’s Service Directory for 2013-2014. This directory provides a comprehensive overview of the distinct and diverse range of services that MercyCare provides.

MercyCare is a leading Catholic provider of hospital, health, aged care, family and community services. We provide services directly to over 25,000 people and impact on the lives of over 100,000 people in the community each year.

Volunteering MercyCare’s volunteers are the heart and soul of our organisation. Each year our volunteers kindly donate over 13,500 hours of their time and energy to help provide a wide range of services to the West Australian community. Volunteering at MercyCare gives people the opportunity to become part of a team, interact with a diverse range of people and make a real difference in other’s lives.

Our volunteers have the opportunity to be involved in a range of service areas across MercyCare, including Aged Care, Child Day Care, Community Support, Workforce Development and Mercy Hospital.

MercyCare volunteers are given the opportunity to work in a service area that they have a particular interest in. Tasks may include helping clients to learn English, delivering flowers to patients, arts and craft sessions, administration duties, gardening or assisting in our seniors day centre.

Services for the aged

Home and Community Care (HACC)Provides low-level care, focused on assisting with jobs that have become a burden, which includes everyday household duties, providing social support and personal care as well as shopping for groceries or any other essentials. Other HACC services available include nursing and home/garden maintenance services.

McAuley House Seniors Day CentreAttends to the needs of elderly people living in the Wembley area or surrounds. A bus is available to transport clients to and from the centre. The centre provides a home-like environment where people are able to simply enjoy a cup of tea amongst others or take part in activities and entertainment provided by the facility. Activities include picnics, afternoon teas, games and guest speakers.

Davis House Respite for Carers ProgramA flexible respite option for family carers caring for a loved one living with dementia. This service provides both day care from Monday through to Saturday and overnight respite care on Tuesday, Wednesday and Thursday. Carers are specifically trained to care for those with dementia and attend to clients emotional and physical needs to make life as stimulating, fulfilled and fun as possible.

Community Aged Care Packages (CACP) CACP provides a higher level of care than HACC and is aimed at maintaining independence by assisting people living in their own home. Assistance options include housekeeping, laundry assistance, preparation of meals, shopping, gardening and social support.

Community Nursing - Wound Care MercyCare has Registered Nurses available to provide wound and clinical care services in the comfort of your own home.

National Respite for Carers’ Program (NRCP) - In Home Respite Specifically aimed at providing flexible respite to carers wishing to keep their loved one at home whilst having the opportunity to take some time out and do the things they want or need to do. Trained support workers assist clients with personal support, meals, entertainment and stimulation. Services available include three- or six-hour, day and night, in-home or out-of-home respite, one off or regular respite, carer support and education, crisis respite and 24hr “on call” respite for emergencies.

Mercy at Home Available to people of all ages who need support at home, Mercy at home provides services tailored to your needs. Whether clients are new Mums, fly-in-fly-out workers or older people we can provide the services you require, such as running errands, paying bills, cleaning and more.

RESIDENTIAL CARE

Residential Aged Care MercyCare’s Residential Aged Care Facility supports residents throughout the ageing process in a dignified and respectful manner. Our staff are highly skilled and dedicated people who love working in aged care and our strict recruitment guidelines ensure our care is of the highest standard.

Our staff provide person centred care to every resident residing in our high or low care units. Resident’s specific and often complex care needs are assessed, documented and reviewed by Registered Nurses who supervise our care teams in providing the identified individual care needs of each resident. They work closely with our therapy teams and the resident’s GPs to ensure a consistently high standard of clinical and lifestyle care is provided 24 hours per day.

Anne Xavier Wing 40 bed Low Care Residential – Non Secure Unit Anne Xavier wing has 40 low care beds and is part of the overall vibrant community of MercyCare’s Residential Aged Care facility.

Each room has its own en-suite and all are spacious, allowing plenty of room for residents to add their personal touch. Resident’s are encouraged to participate in the many activities on offer or enjoying other options such as a ‘High Tea’ with friends or family in the beautiful surrounding gardens.

Ursula Wing 16 bed Low Care Residential - Secure Unit With only 16 residents, the Ursula Wing is a small, homely community that meets the highly specialised care needs of people living with dementia. There is plenty of room for them to wander in a vibrant purpose built unit and lovely gardens, enabling the residents to enjoy the many activities on offer in a pleasant, safe and secure community.

Catherine and Claver Wings 57 bed High Care – Non Secure Unit High care residents are encouraged to participate in both internal and external activities and maintain their connection with the broader community where possible. With large spacious rooms (some with en-suites) residents and their families are able to individualise their rooms to ensure a homely atmosphere. Rooms are surrounded by lovely courtyard gardens and covered walkways which give easy access to our Chapel with its beautiful stained glass windows and peaceful garden surrounds. Mass is celebrated in the Chapel Sunday to Friday and all are welcome to attend.

RETIREMENT LIVING

Mercy Village ~ Wembley Mercy Village Wembley is idyllically situated close to Lake Monger in Wembley, offering a range of accommodation options as well as providing onsite services such as podiatry, beauticians, fitness classes and social activities. Mercy Village also provides both recreational and social activities which are available to all residents.

PASTORAL CARE

Pastoral Care Pastoral care provides emotional, social and spiritual support as well as helping people with grief and bereavement and adjusting to their changing lifestyle. This service provides assistance with problem solving and provides links with other community services and helps people to discover new interests.

Child Day Care Services MercyCare’s Child Day Care Services provide six centres offering long day care, before and after school care or a combination of both. Our staff are experienced in providing quality programs that are age-appropriate and engaging for children. By making the environment a happy place we give children every chance to explore and play their way through their child care experience.

Community Care and Support ServicesCommunity Care and Support Services (CCSS) consists of two programs – Community Detention for Unaccompanied Minors (UAM) and Community Detention for Adults (Vulnerable Adult Males and Family Groups). Both services provide arrangements that support a person’s detention placement in the community, whilst the Department of Immigration and Citizenship (DIAC) resolves their immigration outcome.

The aim of the both programs is to integrate and support clients into the Australian way of life, through assisting service users to access other services such as education and health.

Referrals come through the Department of Immigration and the Australian Red Cross.

Out of Home Care Intensive Family Support Service

Supporting families at risk of having their child/ren taken into care this service is a specialised intensive program that works with families to set and achieve goalsthat will improve the families’ resilience and coping skills, provide safety and reduce the risk of harm to children. The best outcome is for children to remain safely in the care of their family or extended family.

Fostering Services MercyCare’s Fostering Services offer care and support to children and young people who are unable to live with their families. Foster carers are recruited and trained, and provide medium- to long-term (6 months or more) care for children aged 0-17 years within the Perth metropolitan area.

MercyCare also recruits and trains specialised foster carers who care and support children and young people with high needs and difficult behaviours, who are unable to live with their families.

Family Group Homes Family Group Homes operate in the northern suburbs to accommodate children ranging from 0 - 18 years of age who cannot live at home. Working in partnership with the Department for Child Protection we provide the highest standard of care and ensure the best outcomes for the children we care for. Through a Therapeutic Model of Care we develop a plan for each child that can include either long-term fostering, reunification with their birth parents or kinship care.

Workforce Development Centre A free service designed to assist Culturally and Linguistically Diverse (CaLD) clients at any point throughout their life to make educational, training and work choices. This service assists clients with getting information on training and opportunities to use skills or learn new or additional skills.

Settlement Grants Program The Settlement Grants Program works with permanent residents who have arrived in Australia during the last five years as humanitarian entrants or family stream migrants with low levels of English proficiency, to settle in Australia as soon as possible.

Rental Ready Rental Ready is a free training course that provides advice and skills aimed at helping people acquire and live in rental properties in Australia. The course includes understanding tenancy rights and responsibilities and a practical module on cleaning and property maintenance.

Community Support Program MercyCare’s Community Support Service work with the four key principles of engagement, inclusiveness, collaboration and capacity building to build stronger community networks. Activities provided enable clients an opportunity to develop their knowledge, skills and confidence which they then use in their own lives.

Community Support and Workforce Development cont...Child and Parent Centre

Together with Warriapendi Primary School, in Balga, MercyCare is setting offers a welcoming place at the school for parents and carers to come with their young children. The service can be tailored to individual needs and provide assistance such as parenting workshops, child and maternal health services, playgroups, early learning programs, counselling or family support.

On The BEEaT (The Bridge to Education, Employment and Training) On the BEEaT is a free course for men and women of any age from CaLD backgrounds. This service aims to build confidence, self-esteem and assertiveness through training which focuses on developing skills such as, job searching, customer service and general cleaning.

Geared Up To Go Geared Up To Go provides supported transition to education, training or employment for disengaged Year 11 and 12 students from CaLD backgrounds in the North Metropolitan Education Region.

Accommodation, Support and Youth Services

Carlow House Providing long-term accommodation (9 months) for homeless young people aged between 16 and 18 years, Carlow House offers a safe and supportive environment aimed at preparing young people experiencing homelessness to live independently. A Youth Worker is available in Carlow House 24 hours per day, 7 days per week, to support and assist the young people to develop their independent living skills so that they are able to live on their own in the community.

Coolock Units MercyCare’s Coolock Units provide medium-term accommodation (6 months) for single mothers between the ages of 16 and 25 years. The service assists single mothers needing support as they work towards independent living. Single mothers are accommodated in 2 bedroom units in the City of Stirling and Support workers are available to give assistance with the development of social, parenting and life skills and to provide emotional support.

Youth Support ServiceProviding assistance to young people, aged 18 to 25 years, to obtain their own accommodation in the private rental market, the Youth Support Service provides Support Worker visits to help young people maintain their independent life style.

Reconnect Reconnect is an outreach service providing assistance to young people, aged 12 to 18 years, who are having difficulties staying at home, or who have recently become homeless because of family issues. Reconnect works with young people as well as their families.

Mercy Hospital Mercy Hospital Mount Lawley (first opened in 1937 as St Anne’s Hospital) is a licensed 205 bed facility that provides acute, overnight hospital and day treatments across a range of clinical specialties. In retaining the history of St Anne’s care begins with babies and families continuing across the ages in the surgical and medical domains to care for the elderly and those with mental health issues. We assist transition back into the community with rehabilitation and restorative services. The hospital provides services to private, public, entitled veterans and self-insured patients.

Endoscopy Unit Providing a full range of diagnostic and therapeutic procedures for both day and inpatients, the Endoscopy Unit offers procedures such as gastroscopy, colonoscopy, cytoscopy and urodynamics.

Family Birthing Unit (FBU) A dedicated team of obstetricians, paediatricians and midwives work together in the FBU to provide high quality maternity service catering for women and their families through all stages of pregnancy, labour, birth and the post natal period. Our modern Birthing Suite offers six delivery rooms with the latest facilities and technology. The Postnatal Ward is a 30 bed facility with 21 large rooms boasting double beds and private ensuites allowing families to stay together during the first few days after birth. Most patient rooms also have stunning views of the beautiful Swan River. Our recently refurbished Special Care Nursery is equipped to care for babies from 34 weeks.

Services include ante-natal classes, childbirth and early parenting education, physiotherapy and a lactation consultant service. Guided tours of our FBU are held every Sunday at 2.00pm.

Mercy Conference Centre The Mercy Conference Centre can cater for a maximum group of 100 people. Smaller areas to accommodate 30 to 60 people are available by hiring Seminar Room 1 or 2. Pre-admission Clinic This clinic provides a multidisciplinary assessment and screening service for patients prior to their admission to hospital.

Designed to ensure a more relaxed, informed admission process, our experienced nursing staff assist with completion of admission forms, nursing assessments, discharge and aftercare arrangements and education relating to each patient’s procedure.

Mercy Restorative Unit (MRU)MRU is a public 24 bed inpatient facility delivered in conjunction with Royal Perth Hospital. The unit was established to service the specific needs of patients in the inner Perth metropolitan area and provides care to patients who have been medically stabilised at Royal Perth prior to transfer to our unit. For admission to MRU, please contact the Department of Geriatric Medicine at Royal Perth Hospital.

The Day Hospital Part of MRU, the Day Hospital supports outpatients requiring ongoing medical assessment and/or rehabilitation therapy.

Employment and TrainingMercy Training is committed to meeting the vocational needs of the Aged Care and Community Service industry through the delivery of traineeships and training and assessment services.

As an established Registered Training Organisation (RTO) Mercy Training can provide a range of qualifications that support and develop new and existing employees.

From 2014 a range of traineeships will be offered including:

• Residential Care

• Business

• Child Care

Traineeships combine practical work experience with structured on the job training. Working with employers and supervisors, Mercy Training strives to create a unique learning experience for all its participants.

Catherine McAuley Leadership ProgramThis award programme aims at developing women aged between 28 and 40 years, in leadership and service. The course incorporates mentoring opportunities, leadership development and a seminar series as well as an intergenerational day. Community awareness is important an important part of the program.
