The anal sphincter complex is seen intact on the postoperative

endoanal ultrasound (arrow and arrowhead point to the internal and

external anal sphincters, respectively).

Submucosal fistula at peroxide-enhanced endoanal ultrasound. 28-year-old male with left

anterolateral submucosal fistula. Transverse endoanal ultrasound image obtained in the low

anal canal (A) and 3D reformation (B) after cannulation of external opening and peroxide

injection. A shallow submucosal fistula is seen as an echogenic focus after peroxide injection

(arrow in (A)). 3D reformation shows fistula as a linear echogenic track (arrowheads in (B)).

The fistula extends just inferior to the margin of the internal sphincter. A cutting therapy

applied to this fistula would not result in sphincter division. IAS, internal anal sphincter.
