About this Directory

This directory is for use by primary care and community staff, in signposting patients to groups and services that may help them to improve their health and well-being.

It has been designed to be used in conjunction with the patient ‘prompt’, copies of which for pharmacies and general practice can be found over the page. In general practice, the ‘prompt’ information should be given to the patient in advance of their consultation if possible, to allow them time to consider if they have any well-being related issues they wish to raise. In pharmacies, the ‘prompt’ poster should be displayed where it can easily be seen. You can order copies of the ‘prompt’ materials from Health Improvement (01782) 298055.

Contact details slips are provided at the front of the directory. Two versions are available. The second has been designed with people with learning disabilities in mind. Please use these to provide patients with details of relevant organisations/services to take away.

Please note, the directory does not often include opening hours as these may be subject to change. Patients should therefore be advised to check opening times for the services, groups and organisations listed by phone, before calling in person.

If you wish to provide the patient with these or any other additional details about the organisations listed you can visit the Stoke Mental Health Directory website www.stokementalhealth.info This is a source of more comprehensive information about local groups and services across the area. It can also be used to source information about specific services and support groups for people with mental health issues.

If the directory does not meet their needs for information, you may wish to suggest the patient contacts their local Council for Voluntary Service or library for help in finding the information they require.

Do you feel unwell?If so there are a range of NHS Services to help you.

The following information will help you make the most of the NHS services available:

Self care➜ Can you treat yourself at home?

Self care: A well stocked medicine chest will help you treat many everyday illnesses and minor ailments at home. For example, a small supply of paracetamol or ibuprofen (available as syrup for children) and other remedies will help you treat common ailments such as coughs, colds, sore throats, indigestion, toothache, headaches and constipation. If you have children, don’t forget to include appropriate medicines for them.

Pharmacist➜Have you been to the pharmacist?

Ask your pharmacist: Pharmacists (sometimes called Chemists) are experts on medicines and how they work. They can also offer advice on common complaints such as coughs, colds, aches and pains and other health issues, such as healthy eating and giving up smoking. They can help you decide whether you need to see a doctor. You can talk to your pharmacist in confidence - even about the most personal symptoms and you don’t need to make an appointment. See list on pages 10-12.

NHS Direct➜Have you called NHS Direct?

Call NHS Direct: You can call NHS Direct on 0845 4647* for confidential health advice and information 24-hours a day, 365 days a year. The lines are staffed by nurses and professional advisors or visit the website on www.nhsdirect.nhs.uk. NHS Direct has become a first point of contact for patients seeking medical help outside normal surgery hours. Provides the first point of call for a wide range of conditions such as wounds needing stitches, rashes, ear infections, injuries from farming accidents and sore throats.

Doctors surgery➜Do you need to visit your Doctors Surgery?

Doctors surgeries: Your local Doctors Surgery provides a range of services, including general medical advice and treatment; prescriptions; referral to a specialist or hospital (where appropriate); jabs and tests (such as immunisations, blood tests or cervical smears). See list on pages 6-9.

Remember to tell your doctor if you have tried or are still taking self-care treatment.

Surgeries are always busy, so be sure to keep to your appointment time and cancel it if you need to; missed appointments waste precious time and resources.

Out of normal surgery hours (usually 6.30 pm to 8.00 am), all Doctors have an emergency service, North Staffs Urgent Care. This service is only for urgent medical problemsthatcannotwaituntilthenextdaytobetreated.PleasecontactyourGPSurgery as normal and follow the instructions given on the telephone message. Alternatively some practices can automatically divert your call to the Centre. Please do not attend the Centre without an appointment.

A&E / 999➜Do you or a family member need emergency hospital treatment?

Accident & Emergency (A&E) or 999: It is often very obvious when emergency care is needed for serious injury or illness. You should get medical attention by either taking the patient to the Accident & Emergency (A & E) Department in Hartshill or by telephoning 999 for an emergency ambulance.

An emergency is a critical or life threatening situation such as: •Lossof consciousness •Heavybloodloss •Suspectedbrokenbones •Persistentchestpainfor15minutesormore •Difficultybreathing •Overdose,ingestionorpoisoning

Remembertokeepcalm,doeverythingyoucantohelptheperson,butdon’tputyourself in danger and don’t give the person anything to eat, drink or smoke.Unless you need emergency medical attention avoid local A&E Departments. Doctors and nurses there are equipped to deal with serious cases of injury and illness, not routine and minor ailments. Calling an ambulance won’t necessarily mean you are seen any quicker at A&E as the most serious cases are prioritised.

Children and adults who are unable to find an NHS dentist for regular dental care are eligible to seek treatment at Dental Access Centres provided by the Salaried Primary Dental Care Service at various Health Centres across North Staffordshire.The service provided at Dental Access Centres complements existing NHS dental provisionbyGeneralDentalPractitionersinNorthStaffordshire.DentalAccessCentresprovide a complete range of NHS dental services including urgent care and courses of treatment. B-Clear Chlamydia ScreeningB-ClearisthenameforthechlamydiascreeningprogrammerunwhichcoversStokeonTrent and North Staffordshire and is part of the national chlamydia screening programme.The service operates from the Piccadilly Centre as well as a range of venues throughout North Staffordshire offering free and confidential chlamydia screening.

What is Chlamydia? Chlamydia is a sexually transmitted infection that rarely has symptoms and is contracted through having unprotected sexual contact. One in ten young people have the infection and are not aware of it. If left untreated it can cause problem with fertility for both male and females.

It is very easy to test and treat Chlamydia, a urine test for males and a ‘self taken swab’ for females. Chlamydia is treated using a single dose of antibiotics. Young people are encouraged to use condoms in order to ‘b-clear’ of the infection.

The screening programme uses several innovative ways to reach young people aged 15-24.

A‘postaltestingkit’isavailableonrequestthroughtheB-clearwebsiteorpeoplecandrop into any of the contraceptive and sexual health walk in clinics and have their test results sent via ‘text’ messaging - 95% of people choose this method to receive their results.

There is also a mobile unit which is out and about most day’s a week in a variety of area’s and venues across North Staffordshire, including youth services, schools, colleges and universities, outreach football matches, theme parks, and city centres.

Self-help for Common Mental Health ProblemsThe following is a list of resources for unsupported self-help to which you can signpost patients.Beforesuggestingunsupportedself-help,pleaserefertotheguidancebelow:

1. Is the person experiencing mild/moderate emotional/mental health difficulties?

2. Have you excluded physical causes for their presenting symptoms?

3. Can the person read well enough?

4. Is the person suicidal?

5. Is the person at risk of self-neglect as a result of their mental health difficulties?

6. Could the person be experiencing the symptoms of a severe mental healthproblemie.psychosis,seriousdepression,Bi-polarDisorder?

7. Has the person experienced a severe mental health problem in the past and could this be a relapse?

If answered yes to questions 1 – 3 and no to 4 – 7, unsupported self-help may be helpful.
