SU Wellness Initiative

The University has expanded its commitment to the health and well-being of employees by launching the University Wellness Initiative. Under the banner of embody: your well-being, your way, the Wellness Initiative leverages a variety of resources on campus and beyond to help you pursue your own individual path to wellness. Whether it’s an evening Healthy Cooking class taught by a chef and nutrition expert from the Falk College, a lunchtime tai chi session or an eight-week stress-management course, the goal is to help you live healthier and feel better. The website for embody and the University Wellness Initiative is easy to remember: Wellness.syr.edu. It is a one-stop shop for events, programs, and wellness resources.

Condition Management

If you participate in a University health plan, two other programs also are available to you through Optum. The Condition Management Program partners you with a nurse dedicated to helping you or your dependents understand and manage chronic conditions. These conditions are: Heart Failure, Diabetes, Chronic Obstructive Pulmonary Disease, Coronary Artery Disease, and Asthma. They represent conditions that if not properly managed can lead to devastating results. And we all know that it is often difficult to manage chronic conditions whether it is a lack of education about the condition, a misunderstanding about your doctor’s instructions, or just a lack of moral support for lifestyle changes that you may need to make. The dedicated nurses in the Condition Management programs are trained to help you through these obstacles.

Condition Management

You may be contacted by an Optum nurse if your doctor makes a referral to them or if something in your medical or prescription drug claims indicates that you could benefit from the program, such as a first-time prescription for diabetic test strips. These are not sales calls – Optum’s services are provided free of charge to you as part of the University’s benefits package and your individual health information remains confidential: It is not shared with HR or anyone else at SU. So please take advantage of this additional support. Also, you don’t need to wait for an Optum nurse to call you. Simply reach out to the NurseLine. They can walk you through the programs, determine if one of them would benefit you, and help you get started.

The second new program available to health plan participants is the Treatment Decision Support Program. This program teams you with a nurse who will help you understand and evaluate treatment options and providers for specific conditions. The goal is to help you to make informed choices that lead to better health outcomes. The nurses can: • Offer evidence-based information on Medical Conditions • Provide information on Alternative Treatment Options • Explain relevant Decision Points unique to each individual • Help prepare you for your Appointments • Identify a need for Specialty Care • Refer you to In-Network Providers that meet quality and efficiency

standards • Schedule Appointments You are likely to hear more about Optum over time. What we want you to know now is that Optum is there to provide SU employees, retirees

and others with valuable programs, advice and support to help meet your health needs.
