• ผ ปวย asthma ทไดรบการรกษาดวย fixed dose controller และ SABA as needed ในกลมทไมเคยได ICS มากอน สามารถไดรบ well control ถง 80% และพบวาผ ปวยสวนมาก (70%) สามารถไดรบ well control โดยการใช ICS อยางเดยว
• ถาแพทยมความเขาใจในการปรบยาตามความเหมาะสมการใช SMART approach จงไมมความจาเปนในผปวยสวนใหญและอาจทาใหมการใช LABA โดยไมจาเปนในผปวยจานวนมาก
LABA in asthma: FDA warningLABAs should only be used long-term in patients whose asthma cannot be adequately controlled on asthma controller medications (ICS). LABAs should be used for the shortest duration of time required to achieve control of asthma symptoms and discontinued, if possible, once asthma control is achieved. Patients should then be maintained on an asthma controller medication. 28/2/2010

SMART is not so simple
• ไมมขอมลวา compliance (controller) ของผ ปวยดขนใน SMART
• ผ ปวยสวนมากจะตองม inhaler 2 อนเปนอยางนอย
• ผ ปวยบางสวน อาจกงวลเรอง ICS, ราคายา อาจทาใหไมใช reliever อยางทควรใช
• ผ ปวยมกจะถามวาตองลางปากหลงใชหรอไม
• ไมมแนวทางชดเจนในการปรบ controller วาควรจะปรบ controller เมอไร ใน SMART เชน ถาผ ปวยใช reliever 1 ครงตอวน ควรจะเพม controller อยางไร

Secondary outcomes: the number of severe exacerbations, mean use of as-needed medication ,prescribed asthma medications, peak expiratory flow (PEF), Asthma Control Questionnaire (five-item version; ACQ-5) score health economics and safety.

No signigicant difference in time to severe exacerbation and severe exacerbation rate, no difference in sputum eosinophils count after 6
months
LESS: ICS and cost in SMART group• 18% of control group did not use LABA• 11% of control group used montelukast(increased unnecessary cost)
ICS and oral steroidsThe mean (range) ICS dose (expressed as
beclomethasone dipropionate equivalents) was 1,015(30–4,000) mg/day in the CBP group and 748 mg/day
(500 mg/day maintenance + 248 (range 0–2,710) mg/day as needed) in the budesonide/ formoterol

In real life, SMART approach does not reduce oral corticosteroids nor significantly reduced

• At present, there is no evidence that better asthma treatment outcomes can be obtained by moment-to-moment symptom-driven use of ICS/LABA therapy compared with conventional physician-monitored and adjusted controller therapy

SMART efficacy: summary
Conclusion• Single inhaler therapy can reduce the risk of asthma
exacerbations needing OCS in comparison with fixed dose maintenance ICS
• Guidelines and common best practice suggest the addition of regular long-acting β2-agonist to ICS for uncontrolled asthma, and single inhaler therapy has not been demonstrated to significantly reduce exacerbations in comparison with current best practice, although results of five large trials are awaiting full publication
