Restoring function and aesthetics with implant therapy is a lifetime investment for the patient; however, from day one, the maintenance of optimal peri-implant health is important to ensure long-term success and a high level of patient satisfaction.

Successful treatment outcomes are dependent upon good communication between all members of the dental implant team:

• Surgical

• Restorative

• Dental Laboratory

• Dental Hygienist and Auxiliary Members

• Patient

Prevention: Product Selection and Peri-Implant Health Maintenance

Selection of an implant is an important first step toward long-term peri-implant health.■ The 3i T3® Implant incorporates the proven OSSEOTITE®

Surface technology at the coronal aspect of the implant. In a five-year study, the dual acid-etched surface of the OSSEOTITE Implant presented no increased risk of peri-implantitis or soft-tissue complications as compared to a machined-surface implant.1*

Implant maintenance and follow-up care.■ Daily patient oral self-care and regular

professional maintenance are critically important to ensure optimal peri-implant health and long-term success of implant-supported restorations.

The Clinical Challenge:

Recognizing and Diagnosing Peri-Implant Disease

Accurately diagnosing changes in peri-implant soft- and hard-tissue health are essential for development of an appropriate course of treatment.

Possible diagnoses of peri-implant disease may include the following:

■ Peri-Implant Mucositis

■ Peri-Implantitis

■ Cement-Induced Peri-Implantitis

Diagnosing Peri-Implant DiseaseProper evaluation of peri-implant disease is a multiple-step process to arrive at a proper diagnosis. Assessment parameters include:› Evaluation of peri-implant health› Bleeding on probing› Presence of exudate› Radiographically detectable bone loss beyond the baseline› Radiographically detectable residual cement

■ Peri-Implant Mucositis*

• Characterized by inflammation of the soft-tissue mucosa around a dental implant.

• The prevalence of peri-implant mucositis has been reported to be as high as 80%.5

■ Peri-Implantitis*

• Characterized by inflammation, suppuration, increased probing depth and bone loss.

• The prevalence of peri-implantitis has been reported to be in excess of 12%.5

Treatment

The Clinical Challenge:

Treating the Ailing Implant

Numerous implant designs and surface treatments have emerged in the last few decades. Some of these have been shown to be more prone to the development of peri-implant disease once exposed to the oral environment. Once an ailing implant is identified, a proactive approach to treatment should be initiated that may include debridement and grafting.

Treating the Failing Implant

When peri-implant disease progresses to the point that the implant is not salvageable, the implant may need to be surgically removed. A common procedure for removal of a failing implant is the use of a trephine bur. This procedure may result in the removal of excess bone surrounding the implant, which may necessitate grafting of the site and delayed replacement of the failed implant.

Treatment of an Ailing Implant

■ Manual and/or chemical debridement is an essential first step in the treatment of an ailing implant in preparation for regenerative procedures.
