FROM THE PRESIDENT

A FAREWELL MESSAGE FROM PETERby Dr. Peter Rechmann

Most times the final message from a president isself-congratulatory, recounting the progress oforganization during his time on duty. And it would befitting and proper if I did that since we have madesignificant advances in our relationships with the dentalcommunity—The American Dental Association andThe Academy of General Dentistry in particular, aswell as broad inroads into Dental Schools. Furthermore

at the American Dental Education Association during the first Special InterestGroup - Lasers in Dentistry meeting Academicians from eight differentUniversities discussed integration of Lasers in Dentistry in their teachingcurricula.

The credit for the success should be assigned to the Board, CommitteeChairs and ALD members, as well as the tireless work of Gail and herstaff. Functioning tirelessly as a team is what made all of these thingshappen. Having said all of that, I could go on and on, but now I want todevote my final thoughts on science, particularly the concept of thermalrelaxation time.

What’s this, you might be thinking, science belongs on the other pages,specifically in the Journal of Laser Dentistry (JLD). But I think there is alot more to learn from laser science than simply how to use our technologymore skillfully. We all know that when we use lasers, we have to payspecial attention to thermal relaxation time if we want to achieve the desiredand best treatment results without causing collateral damage.

I have observed, and at this point I invite you to imagine that theInternet and particularly the ability to communicate with email, is a laser—powerful for sure, but dangerous and easily abused. The abuse that I wouldlike you to think about, happens when we do not pay sufficient attention,frequently we do not pay any attention at all, to the consequences of ourmessages and responses. In my practice wehave two-hour staffmeetings every week. Atthose meetings, we eachbring up the things that wefeel have affected the

practice both positively and negatively during theweek. At last week’s meeting, my patientcoordinator, who is present during all my treatmentplan presentations, contributed that “a positive mustgo to Dr. Triliouris for the way she is doing alltreatment plan presentations. They convey her caringnature and she puts her whole self into them so muchso that there is just no way the patient will not followher recommended treatment.”

Wow! I was impressed with her remarks and Ithanked her, but most importantly, she got me thinking.There is so much written about treatment planpresentation. Each of the gurus has his or her answerto successful treatment plan presentation andacceptance. But I think you just have to be there, fullyand with your heart in your hand to show you care.

Of course, you have to be prepared. You have totalk in language that is clear to somebody that is not adentist. You should have pictures of the patient’s oralcondition to show them what you see. You should havepictures or models of what you are recommending sothe patient can see what you are talking about. Youshould be able to offer financing so that they can affordthe treatment. You need to listen to what they want, not

everybody wants a Hollywood smile. But unless youput yourself in their shoes, show them your human sideand make them believe you have their best interest inmind, it will not work.

What do I mean by being there? It is exactlythat “touchy feely” thing that most dentists havedifficulty with. You know that your patients don’tknow you really care until you tell them and showthem with your actions as they get to know you andyou get to know them. That is the way you developlong standing relationships and how you maketreatment plan presentations that are successful andthat are accepted by the patient. This is what wemust all do to keep us all working in these difficulteconomic times.

We don’t appreciate theone dimensional nature of this form of communication, nor the way thatour email will be received. When I say received I mean 1- read, 2-understoodand 3-experienced emotionally. As a scientist and researcher, I would liketo bring you back to the necessity to respect the thermal relaxation time ofhuman beings, especially in communication. We need time to relax, tothink, to consider and to properly compose both our thoughts and languageand then finally to fully imagine how what we say will be received. Thistakes time, relaxation time if you will …

I will close with some thoughts you perhaps might want to print and poston your computer screen.

Meet your ALD Candidates for Board Service 2009The Academy of Laser Dentistry Nominations Committee is pleased to present the 2009 nominations for officers and members of theBoard of Directors. The following is a short biography of each of these leaders:

PRESIDENT ELECT: Dr. Steven Burman is in privatepractice of general dentistry in Manalapan, New Jersey. He holdsAdvanced Proficiency in Nd:YAG and Er:YAG wavelengths aswell as Educator status. He is presently the Vice President of theALD, and has served as Chair of the subcommittee on Information

Systems. He is a Fellow of the Academy of General Dentistry, a Master of theAcademy of Laser Dentistry and has presented lectures on laser use and trainingsthroughout the United States and Canada. Dr. Burman may be reached by email [email protected]

VICE PRESIDENT: Dr. Ana Maria Triliouris is a generaldentist in private practice in Merrick, New York, and is a chartermember of the ALD. She currently serves as the Academy’sTreasurer. She is among the very few dentists that started usinglasers in 1990 and has volunteered for the ALD in different

positions since then. She was the first Editor of the ALD first publication“Wavelengths”. She is an active member of the ADA and its components as wellas the Academy of General Dentistry and the American Association of WomenDentists. She is also the chair of the Dr. Eugene Seidner Student ScholarshipCommittee and the Editor of the newsletter, “Lightwaves” at the ALD. Dr. Triliourismay be reached by email at [email protected].

TREASURER: Dr. Arthur Levy maintains a privatepractice in Chester, New Jersey, and is a charter member of theALD. He holds Advanced Proficiency in Nd:YAG. He has beenactive in numerous committees and on the Board of Directors sincethe beginning of the Academy in 1993, and he currently serves as

SECRETARY: Dr. Glenda Payas practices generaldentistry in Tulsa Oklahoma. She is a charter member of the ALD,and holds Advanced Proficiency in CO2 and Er:YAG wavelengths.Dr. Payas currently serves on the Board of Directors and iscompleting her first 3 year term of service in 2009. She was one

of the first dentists in the United States to use a laser, starting in 1991. She holds aMastership in the Academy of General Dentistry and the Academy of LaserDentistry and is a Clinical Instructor for Dr. John Kois at the Kois Center, Seattle,Washington. NOMINATED BOARD MEMBERS for 2009-2012

Dr. Charles Hoopingarner is in prívate practice in HoustonTexas since 1973. Having achieved both Standard and AdvancedProficiency certification in Er:YAG and Diode from the Academyof Laser Dentistry, Dr. Hoopingarner has used dental lasers as anintegral part of his patient care delivery system for the last twelve

years. Dr. Hoopingarner was an Adjunct Associate Professor in the Departmentof Anatomical Sciences, University of Texas Health Science Center at HoustonDental Branch for 11 years and is currently serving as a clinical adjunct in thedepartment of restorative dentistry, lecturing in cosmetics and reconstructivedentistry. While serving as a co-director of the Temporomandibular Disorderssection of the General Practice Residency program, Dr. Hoopingarner helpedestablish the University wide craniofacial pain team. Dr. Hoopingarner has been amember of the ADA, TDA, and GHDS since 1973. He has been membership

committee chairman, Editorial reviewer and business manager of the GHDSJournal, a TDA delegate, as well as program director, social chairman,administrative chairman and general chairman of the “Star of the South” dentalmeeting. He currently serves ALD as a member of the General and ScientificSessions Committee and Membership Committee. Dr. Hoopingarner may bereached at [email protected].

Angie Mott, RDH, practices in Tulsa, Oklahoma, and is thecurrent chair of the Auxiliary committee. She also serves on theScientific Sessions, Membership, and Regulatory Affairscommittees. She holds Advanced Proficiency in Nd:YAG as wellas Educator Status, and is an ALD Recognized Course Provider.

She has presented at ALD Conferences and at the Oklahoma Dental HygieneAssociation meetings. Angie is an invited speaker at RDH Under One Roof 2009in Las Vegas. This is Angie’s second term of board service as the AuxiliaryRepresentative to the ALD Board. She may be reached by email [email protected].

Dr. Peter Pang has been in full time private practice inSonoma, CA for over 20 years. He has been awarded Fellowshipstatus with the Academy of General Dentistry, Academy of LaserDentistry and LVI Global. With ALD he has also achieved bothAdvanced & Educator levels. A frequently invited speaker on the

topic of Cosmetic Dentistry and Lasers in Dentistry, he has been published inseveral journals. Dr. Pang is ALD’s Chairman of Science and Research; he iscurrently completing his second year of board service and is nominated to servehis first full 3-year term 2009-2012. Dr. Pang may be reached by emal [email protected].

Dr. Steve Parrett is in the private practice of general dentistryin Chambersburg, Pennsylvania where he is currently chairmanof the Department of Dentistry at Chambersburg Hospital. He holdsstandard proficiency in Er:YAG, Diode and CO2 wavelengths. Hehas served on the membership committee of ALD, is a member of

the Dean’s Faculty at the University of Maryland dental school and has served onthe House of delegates of the ADA and PDA. He has achieved Fellowship statusin the Academy of General Dentistry and has served many years as a clinicalevaluator for Dr. Gordon Christenson’s CRA independent research organization.Dr. Parrett currently serves on ALD’s Membership committee and has served onthe ALD Communications committee in the past. Dr. Parrett may be reached [email protected].

The offices of President and Immediate Past President are automaticallyfilled, and are not part of the voting process. These two officers are:

PRESIDENT: Dr. Tony Hewlett is in private practice ofgeneral dentistry in Stanwood, Washington. He currently servesas the ALD President-Elect. He received the Academy’sDistinguished Service Award in 2004 for his outstandingcommitment and contributions. He has served on the Executive

IMMEDIATE PAST PRESIDENT: Peter Rechmann,Prof. Dr. med. dent., was originally a medical staff member inthe Department of Oral Surgery and Oral Medicine at Heinrich-Heine-University in Düsseldorf, Germany. Since 2001, he has beenat the University of California at San Francisco, Department of

Preventive and Restorative Dental Sciences, where he is currently a Professor inthe division of Prosthodontics as well as the Director of the Clinical SciencesResearch Group. Dr. Rechmann currently serves as ALD President; he holdsAdvanced Proficiency in CO

2 and Er:YAG. He was presented with the T.H. Maiman

Award for research by the ALD. Dr. Rechmann may be reached by email [email protected].

CONTINUING DIRECTORS

Dr. John Graeber has maintained a comprehensive fulltime general practice in East Hanover, New Jersey. He hasbeen awarded Mastership in the Academy of GeneralDentistry, has Advanced Proficiency in Nd:YAG as well asEducator status and is a Recognized Course Provider. He has

been teaching the Standard Proficiency course since 1996 and has taughtdentists all over North America how to use their new lasers. His term willcontinue until 2010.

Dr. Gabi Kesler is a lecturer in the department of OralRehabilitation at the school of Dental Medicine, Tel AvivUniversity, Israel and the coordinator of the graduate andpostgraduate Dental laser program. He has Advanced Proficiencyin CO

2 and Er:YAG, is a Recognized Course Provider and has

Educator status. He has also received the Leon Goldman Award for clinicalexcellence. He established the Israeli Chapter of the Academy of Laser Dentistry,served as its first president in 2005 and currently serves as ALD’s Chairman ofInternational Relations. His term will continue until 2011.

Dr. Mitchell Lomke is in private practice of General Dentistryin Olney, Maryland. He has Advanced Proficiency in Er:YAGand diode wavelengths, has Educator status, and is a RecognizedStandard Course provider. Dr. Constanza Martinez is in private practice in Bogota,Columbia. She has Advanced Proficiency in diode, has Educatorstatus since 2005 and has been an active member of our Academysince 1998. She has served as an examiner for several years andhas attended ALD’s annual meetings steadily since 1998. She

directs a group of specialty dentists that assist her in the complete management ofher patients’ routine and specialty care. She is also an active member of theAcademy of Osseointegration. Her term will continue until 2011.

Dr. Michael Swick is a general dentist and has offices inAllison Park and Conneaut Lake Pennsylvania. He holdsAdvanced Proficiency in the diode and Er:YAG wavelengths,has Educator status, and is a Recognized Course Provider. Hehas served on the Science and Research, Scientific Sessions, and

ALD Announces 2009 Student Scholarship Award RecipientsBy Dr. Ana Triliouris, Student Scholarship Chair

The Academy of Laser Dentistry (ALD) is proud to announce threerecipients for the Dr. Eugene M. Seidner Student Scholarship Recognition for2009. Each clinician will give an oral presentation as part of the Scientific &General Sessions of the Academy of Laser Dentistry’s 16th Annual Conference,April 22-25, 2009 at the Green Valley Ranch Resort, Las Vegas, NV.

Each applicant’s eligibility is demonstrated academic achievement in thestudy of lasers in dentistry. The selection is based on an abstract of laserresearch submitted for presentation. The Scholarship funds cover one year ofALD membership, full conference registration to participate in ALD 2009,Standard Proficiency Certification participation, air transportation, and hotelaccommodations at the Green Valley Ranch Resort.

The following summarizes the work that will be presented on Saturday,April 25, 2009.

Bond Strengths of All Ceramics: Acid vs.Laser Etching

Bulent Gökce, DDS, PhD is a scholar from EgeUniversity School of Dentistry in Izmar, Turkey.

The purpose of this paper indicates that forsurface modification of the inner surfaces ofceramic restorations, the etching pattern obtainedby dental lasers could be an alternative toconventional acid etching.

Dr. Gökce is a full-time resident and an Assistant Professor at theDepartment of Prosthodontics of Ege University, School of Dentistry,Department of Prosthodontics and clinical instructor of third year dental schoolstudents. His PhD thesis is on comparison of bond strength to dental substrateswith different surface treatments. His current interests include research ofdental lasers, dental materials and mechanical testing, all-ceramic materialsand zirconia restorations, dental implants, dental adhesives and clinicalapplications. He is a member of International Team for Implantology, theTurkish Prosthodontic and Implantology Association, the InternationalAssociation for Dental Research and the Academy of Laser Dentistry. Forthis award, Dr. Gökce is mentored by Professor Birgul Ozpinar, PhD,Prosthodontics Department Chairman.

Glass Ionomer BondStrength to TeethPrepared with anErbium:YAG Laser

Ms. Huma Hamid andMr. Stephen M. Tapp arescholars from the Universityof Texas-Dental Branch atHouston, Houston –Texas.

The objectives of thisstudy are to provide original knowledge on the interfacial bond strength ofglass ionomer to laser-treated tooth structure by answering the question“How is the bond strength of a glass ionomer to the laser treated tooth as

compared to bur-treated surfaces, and which conditioner - the conventionaletch and rinse conditioner or a non-rinse, self-etch conditioner is moreeffective?”

Ms. Huma Hamid holds a Bachelor of Science degree in Biology fromthe University of Houston, Texas. She worked at Baylor college of Medicineas a Research Technician before coming to the University of Texas DentalBranch-Houston, where she is currently a third year student. Ms. Hamid isa member of the National Society of Collegiate Scholars (NSCS), AmericanStudent Dental Association (ASDA) and International Association for DentalResearch (IADR). While attending school, she also participates in variousresearch projects related to dental health.

Mr. Stephen M. Tapp holds a Bachelors of Science degree in Chemistryfrom the University of Chicago. He is a registered nurse in the State of Texasand has experience in clinical pharmaceutical research. He is currently athird-year DDS student at the University of Texas Dental Branch at Houston.Mr. Tapp is a member of American Student Dental Association (ASDA)and, American Association of Dental Research (AADR). His current interestsinclude clinical application of lasers in Restorative Dentistry, Surgery andTissue Regeneration.

A Quantitative Assessment of the Effect of Various AntimicrobialTherapies on Porphymonas Gingivalis Contamination on the ImplantSurface: A SEM Study

Mr. Brian Sang is a scholar from West Virginia University School ofDentistry, Morgantown, West Virginia.

Previous studies have discussed treatment methods for ailing implants,but none have compared methods for efficacy. The educational objectiveof this study is to determine if a laser is the pre-eminent treatment methodfor the elimination of P. gingivalis from the HA coated titanium surface.

Brian E. Sang is a fourth year dental studentat the West Virginia University School ofDentistry in Morgantown West Virginia. Heattended undergraduate college at MarshallUniversity in Huntington West Virginia majoringin Biological Sciences and performing researchin the field of paleontology. After graduationBrian plans to pursue a career in Oral andMaxillofacial Surgery. He is proud to say he is

Morale... What It Is and How Can We Improve Itby Dr. Alan Goldstein

The American Heritage dictionary definesmorale: As the state of mind of a person or group asexhibited by confidence, cheerfulness, and discipline.I will discuss how we can improve morale in a typicaldental office. I’m inclined to add the notion that thisis a more difficult task in a period when so many ofour patients are feeling the effects of the economic

downturn. The mood or emotional tone that we often encounter is related toanxiety, frustration and resignation. I’m not so sure that the current economicclimate is a major determinant of morale in our offices, but it would be silly todeclare that morale exists independent of circumstance.

Can we control something as subjective as morale? Sure we can say toour staff, “Leave your personal issues at the door, it is Showtime!” but in thereal world can any of us truly do that? A sick child, babysitting issues, an agingparent in whose care we are entrusted and a myriad of personal issues all canand do weigh us down. Who knows, I mean really knows, what goes on in thehearts and minds of our staff. We are not mind readers. Sometimes it is hardenough to get in touch with our own state of mind.

Creating community is one important way we can stay positive and upbeat,even in the trying world we live in. To create this sense of community we startevery single day with a morning meeting, commonly called a huddle in dentalpractice parlance. In addition to the most common agenda items for the huddle;the procedures that are planned and the finances associated with them, ourmorning meeting becomes the place where we can briefly unpack, as isappropriate, what is weighing us down. Brevity is essential; this is not a therapysession. Team members should be listeners, not amateur psychologists. Thisactivity helps create and maintain a loving and empathic office. It is not easybut it is essential. It does require a high level of emotional intelligence on thepart of the office leader. Sorry, I knew you were looking for some trick ormagic formula to get everyone radiating joy. There is no silver bullet, only thispiece of team building work each and every morning.

Our profession includes many Dental Consultants who will tell us whatmarketing plan to use, or how to script conversations for improved caseacceptance, or even teach us how to reduce our overhead. These are all importantthings for our practices. The assumption is that if we improve our productioncollection and net profit and then share the wealth via bonuses, the morale ofthe office will improve. I maintain that prosperity does not ensure a high espritde corps. Relationships among our staff and with our patients have a greaterimpact on morale than the finances.

Let me introduce the notion of character as it was recently used by JamesFlaherty in his February 2009 Coaches Reunion Workshop in WashingtonDC. He is a leader in the Coaching profession and the head of New VenturesWest, a coach training institute. According to him, character can be defined…asthe observable and automatic ways we act, react, express, think and feel thatbrings forth reactions from others and the world. Here is the point: our characteris the major determinant of the morale of those with whom we work. If youwant to look for the key to the morale of your office, look to your own character.Look to the way that your staff reacts to you, look to the gifts you offer; thepraise, gratitude, and appreciation and see if they have been received. Thenlook too, to the criticism and see how that is received and the effect that this hason morale. If we consistently make positive offerings to our staff and make thenecessary adjustments to insure that they are received, then the morale of yourstaff, the morale of your patients and pretty much the morale of everyone youtouch will be positive. If this sounds a bit too pie-in-the-sky for you, imaginethe inverse – how a surly, short and hypercritical character torpedoes moralefaster than Nurse Ratchet in One Flew Over the Cuckoo’s Nest.

Morale is a function of small things—a look, a tone, an empathic nod orjust an openness that says I am listening. We can never know with certaintywhat life has in store for us. If you, as the leader of your office, stay awake tothe incredible impact that your character has on the morale of everyone aroundyou, this positive aliveness that you engender can’t help but be infectious.

To recap:• Create a sense of community every morning by extending the huddle in a

way that gives everyone an opportunity to respond to the implied question,“How are you doing this morning?”

• Pay attention to finances but don’t be seduced into thinking that the dollar isthe answer to everything. Maybe the secret is staff morale.

• Be seen as a rainmaker by your staff. Create excitement by always havinginitiatives on the burner that will expand the practice. For example: a smilesale for Valentine’s Day, a well advertised discount on whitening andporcelain veneers, an appreciation initiative for Thanksgiving, perhapsenhanced hygiene services that include a discount on electric toothbrushesto clean off all the turkey and stuffing that is left on your teeth after the feastand all the other possibilities that you and your creative staff can imagine.

• Stay awake! Realize that your character is the biggest single determinant ofthe morale of your staff. Be a nurturer for every one of them.
