Be clear how you want to use the monitoring and who is it for – you or your funders
How?
• In order to measure impact, HALE uses pre and post questions this allows us to quantify the impact of the softer milestones such as increased confidence etc.
• Using appropriate monitoring forms for your service users – one form does not fit all!
Remember if you change your milestones change your monitoring forms!

• Use an Impact Map, and try to match your inputs, outputs, along with your forecasted
outcomes and then the estimated cost impacts...a must for this is the unit costs of health and social care to get a copy email
