Software Request Directions
ADD
CHANGE
Change requests need to be initiated for the following scenerios. Change in:
DELETE
CSC GTS Manager verifies and determines the initial need (ADD), change to (CHANGE) and deletion (DELETE)of software. This is for all software, including mainframe and server software (this includes mainframe softwarelike Gentran).
To initiate the ADD, CHANGE or DELETE request the CSC GTS Manager will fill out a Software RequestTemplate as described below and submit to the CSC Software Coordinator.
For add requests the columns that are highlighted in Yellow are to be completed (Columns A through L, O, T, Wand X) as appropriate. An “A” needs to be placed in the Action Column (Column A). See comments (red flag attop of each column) for each column for further directions on information that is required. The more accurate theentries, the quicker the request will get processed. For example, there are frequently several editions (professional,standard, developer, enterprise) for one product – the specific edition needs to be filled in. It also needs to beclearly stated in CSC Comments (Column Z) if the request is for (1) a new project project (in which case you mustfill in Project Work Order Number (column X) and SAP Budget Code/WBS Number (column Y)); (2) increasedwork load (new workers must be added due to increased work load from Zurich); (3) steady state support for Zurichapps; (4) was no longer in use; (5) filling a vacancy where replacing someone who is leaving; or (6) for knowledgetransfer. . It would also be important to comment if it is known whether the installation package is available or notavailable.
· Name of the person using the software (Receiver or Sender Name to be updated)
· Location of the software (Receiver or Sender Location to be updated)
· Organization change – Business Unit, application change (BU, Application to be updated)
· Software change – Software Product, Version change (Software Product Names, Version to be updated)
· Movement of the software to another computer (Workstation Id needs to be updated)
· Request status if the request is being put on Hold (CSC Comments needs to be updated)
The information that currently is in place for this software is to be entered as a row in the Software RequestTemplate with a “C” in the Action Column (Column A). Then based on the type of change, the appropriate cell(s)need to be annotated with the original value and the new value. The changed cells are then to be highlighted in red.For example if software is moving to a new computer the original Workstation Id and the new Workstation Id are
to be entered in the Workstation ID cell and highlighted in red.
For software deletes a “D” needs to be placed in the Action Column (Column A). The information that currently isin place for this software is to be entered as a row in the Software Request Template. Enter in Software ActionDate (column T) the desired uninstall date. Enter in CSC Comments column any special directions/informationrelated to the removal.
