EnrIquez1 , J. J. Gil-Gibernau2, V. Garriga1 , I. Ribes3, J. R eal-tim e sonognaphy is a well-accepted technique for evalu- ating abnormalities of the eye. Most previous reports on sonog- naphy of the eye have been in adults [1]. In this pictorial essay, we illustrate norm al and abnormal findings of ocular sonogra- phy in children. Technique Ocular sonognaphy was performed in 152 children ranging from 1 month to 15 years old. Abnormal sonogmaphic find- ings were found in 122. Examinations were performed with a 7.5-MHz linear-array transducer on a model 1 28 Acuson unit (Acuson, Mountain View, CA). Longitudinal and transverse views of each globe were obtained through the closed lid with conventional coupling gel. A stand-off is a preferable method to improve resolution in the near field. Gain settings were adjusted to assess the anterior and posterior parts of the globe (Fig. 1). For cases in which sedation was required, we performed ocular sonography with the patient under the general anesthesia that is normally used in young children and infants for ophthalmologic examination. In some cases, we sedated our patients with 50-100 mg/kg of chlonal hydrate administered orally30 mm before examination. Congenital Anomalies Congenital cataracts may vary from a few dustlike spots to a completely dense white opacity of the lens. On sono- grams, this latter form appears as a thickened wall of the lens. In many cases of congenital cataracts, the lens is spherically shaped, a condition called sphenophakia (Fig. 2). Primary vitreous is the embryonic vasculature of the eye and supplies nutrients to the developing lens and retina during early gestation. The hyaboid artery is the main arterial supply in this network. Primary persistent hyperplastic vitreous (PPHV) is characterized by incomplete absorption of the embryonic hya- bid artery and hypemplasia of embryonic connective tissue [2]. Leukoconia (an abnormal pupillary light reflection) and micmophthalmia are the most common clinical findings. The diagnosis of this condition usually is made fundoscopically, but when the patient has a lens opacity that precludes ophthalmo- scopic visualization of the retina, sonogmaphic examination is needed. Depending on the extent of the abnormality, a wide variety of sonognaphic manifestations can be seen (Fig. 3). Most cases are unilateral. Bilateral cases can be associated with tnisomy 13, 15, 18, and 21 syndromes and Nommie’s dis- ease,an X-Iinked recessive syndrom e associated with bilateral Ieukoconia and microphthalmia (from PPHV), deafness, mental retardation, and absence of the retinal ganglion cells. Coats’ disease (retinal telangiectasia) is a congenital retinal vascular anomaly characterized by telangiectatic retinal yes- sels and associated accumulation of lipoproteinaceous exudate in the submetinal space. There is a 4 to 1 male predominance. Sonognaphy and fundoscopy show typical findings (Fig. 4). The lesion is usually unilateral, and the differential diagnosis includes netinoblastoma, P PH V, and netmolental fibmoplasia. Coloboma is a congenital abnormality caused by disruption of normal closure of the fetal optic cleft [3]. Cobobomas may

Techn ique

O cula r sonognaphy was pe rfo rm ed in 152 ch ild ren rang ing

from 1 m on th to 1 5 yea rs o ld . A bnorm a l sonogm aphic find -

ings w e re found in 122 . E xam ina tions we re pe rfo rm ed w ith a

7 .5 -M H z linea r-a rra y transduce r on a m ode l 1 28 A cuson un it

Congen ita l c a tarac ts m ay va ry from a few dus tlik e spo ts to

a com p le te ly dense wh ite opac ity o f the lens . O n sono -

g ram s, th is la tte r fo rm appea rs as a th ickened w a ll o f the

lens . In m any cases o f congen ita l c a tarac ts , the lens is

sphe rica lly shaped , a cond ition ca lled sphenophak ia (F ig . 2 ).

find ing in seve re cases o f ocu la r traum a (F ig . 1 2). In c reas ing

wh ich a re o ften on ly m in im a lly echogen ic .

F ore ign bod ies m ay be found in pene tra ting in ju rie s . O n

sonognaphy , they a re seen as echogen ic le s ions (F ig . 13 ).

E x tens ive reve rbe ra tion a rtifa c t is v isua lized in cases of

m e ta llic ob je c ts . T he p resence o f a im in the eye m ay p reven t

v isua liza tion o f a poss ib le ocu la r le s ion .
