Welcome to the South West Nova District Health AuthoritySouth West Health provides health and wellness services and programs to more than 60,800 residents in Shelburne, Yarmouth and Digby Counties. Three hospitals and three community based programs are administered by the district: Roseway Hospital, Digby General Hospital and Yarmouth Regional Hospital as well as Public Health, Mental Health and Addiction Services.Our staff, physicians and volunteers work hard to provide excellent care. We work to promote health in our communities and to improve quality of life. Our goal is to meet your physical, emotional and spiritual needs within a caring and supportive environment. Many of our staff can speak French to assist our Acadian population.Thanks to support from our communities, we are able to provide you with this handbook. The Health Care TeamWhile you are in hospital, many people work together to help you. The attending physician is the doctor in charge of your care. Sometimes they may ask other doctors to see you.The Nursing Staff:

a) A Nurse Manager is responsible for the overall management of the unit you are on.

b) Registered Nurses (RNs) are responsible for organizing and providing your care.

c) Licensed Practical Nurses (LPNs) are also responsible for your care and report to a Registered Nurse.

d) Continuing Care Assistant (CCA) are responsible for basic care and report to a Registered Nurse or an LPN.

e) Personal Care Workers (PCWs) are responsible for basic care and report to a Registered Nurse or an LPN.

f) Student Nurses may help you; they are supervised by nursing instructors from their school or their preceptor.

g) The Ward Clerk works mainly at the Nurses Desk and answers telephone calls, greets visitors and keeps your chart updated.

Other important health care staff may help you, such as:Physiotherapists and Occupational Therapists help you return to normal activities when you are injured, have a disease or special needs. These services can be asked for directly or through referrals from your family doctor or specialist.Respiratory therapists specialize in lung illnesses. Doctors will ask these staff to do breathing tests, asthma and COPD treatment, oxygen therapy, CPR and life-support.The EKG technician can monitor your heart to help the doctor diagnose your condition.Lab and X-ray staff will do the tests the doctor orders. These tests will help the doctor determine your health problems and monitor your progress.The Dietitian and Food & Nutrition Services staff carefully planned our menu according to Canada’s Food Guide to Healthy Eating and created recipes which are tasty and meet special diet needs. They can answer questions about your nutrition needs.The Speech /Language Pathologist helps people who have trouble with speech, voice, language or swallowing problems.

Pharmacists give out medications to hospital patients based on doctor’s orders. If you have questions about your medications, ask to speak to a pharmacist.Environmental Services Staff provide us with a clean facility. They will clean your room and change your bed.Social Workers work with patients and their families to understand their needs and to guide them to services both in the hospital and in the community.Diagnostic Imaging Staff will conduct tests such as xrays, CTs and MRIs.Mobility Team members are people such as Physiotherapists, Occupational Therapists and Continuing Care Assistants whose goal it is to help you maintain or improve your mobility while in hospital.Porters may take you from your room to other areas of the hospital for your tests. They will also deliver mail to your room or floor.Others – there are many other health professionals that may be required to help you recover from your illness or that can help you stay well.

Your Hospital StayActivities at Roseway Manor (Roseway Hospital)Alternate Level of Care patients are able to participate in activities offered by Roseway Manor during the week. For a schedule of events, contact the Recreation Director at Roseway Manor at 902-875-4707. You can discuss this with a nurse.

Allergies and SensitivitiesIt is important to tell us if you have any allergies to medications, foods, plants, dyes or latex. Due to latex allergies, rubber balloons are not permitted, foil balloons only. The facility is also scent free. Please check with staff before bringing food or flowers from home. Other patients or staff could be allergic.

Bonjour! ProgramSouth West Health staff who choose to display the "Bonjour!" symbol are proudly displaying the fact that they can communicate and provide some services in French and in English. It is intended to help you find a French-speaking person more quickly when you use health services.

Bringing Food From HomeFresh food brought to you must be stored in the fridge. Make sure it is labeled with your name, room number and date. Always check with a nurse before eating this food if you are on a special diet or fasting for tests.

Care Clown Program – See Palliative and Supportive Care

Child SafetyTubing/LinesInfants and children can be very active. Often your child will have intravenous (IV) or monitoring equipment attached to them. The staff may try to reduce risks by doing things like putting tubing under clothing or keeping IV pumps at a distance from your child.It is important to let your nurse know if you will not be in the room with your child. Tell the staff if you have seen your child become tangled in any lines or tubing.Crib/BedsCrib and bed rails must be locked in the full up position at all times unless someone is at the bedside. The height of the bed should remain in the lowest position to prevent falls.Consider the use of a dome over the crib if the child is active. All food should be removed from the crib/bed when your child is sleeping. Many of the

children like to have toys with them as they fall asleep, some may have to be removed after the child is sleeping.Any child three years of age or younger should be cared for in a crib. If you wish to have your child sleep in a bed, you must sign a release of responsibility form. For safety, it is not recommended that you sleep with your child in the same bed.Tubs/Baths/ShowersDo not leave small children alone in the tub or shower. Make sure the temperature of the water is not too hot before putting your child in the tub or shower.Please get a bath mat from the nursing staff. The tub and shower floor can be slippery. Be sure you can reach the call bell at all times.Empty the tub as soon as you are finished with it. A tub of water must never be left unattended.SecurityAll staff members have to wear hospital-issued id badges. Do not hand over your child to anyone who is not wearing their badge.

ClergySee Religious and spiritual care

ConductStaff patients and visitors must conduct themselves well. Abusive behavior to anyone is not tolerated. If you experience abusive behavior from others please tell the nurse manager. Abusive behavior includes controlling, harassing, intimidating, isolating or threatening anyone, and, being aggressive, forceful or destructive.

Consent If you need certain types of procedures, you will be asked to sign a consent form before the procedure. Please read this form carefully and ask your doctor if you do not understand the nature and risks of the procedure.You will also be asked to sign a consent form before release of any information about your general condition or your image (photo or video) for legal, promotional or educational purposes. Forms also have to be signed for release of responsibility for smoking passes and for leaving against medical care.

Electrical AppliancesFor safety reasons, only CSA-approved electrical appliances may be used while in hospital. Our maintenance staff will check and approve your personal electrical appliances. This includes electric razors, blow dryer, heating pads, fans, and medical devices including CPAP machines and portable ventilators.

Ethics SupportThe district ethics committee is available, on request, to help staff, patients and families explore options for their difficult ethical questions and dilemmas related to health care. In health care settings, ethical questions arise when “the right thing to do” is unclear, or when people disagree about what is best for a patient. The Ethics Committee is a consultative and advisory body and does not make decisions regarding a person’s care or the organization’s policies. For more information contact the Ethics Committee Chair at 902-742-3542 ext. 1798.

Falls PreventionWhile in hospital we want to ensure your safety. Part of that is preventing falls. On admission you will be orientated to your room and unit, and if able, you are encouraged to get up and do things for yourself.Patients, staff and family members all play a role in preventing falls. Always ask for help if and when needed and keep things within easy reach, especially the nurse call bell.For PatientsPlease ask/call for help if you:

• feel light headed • are unsteady on your feet• not sure how to move with an IV pole

Staff may ask you to call for help when you: • have to use the bathroom• want to move to or from the chair /bed or need any other help

Sit up and stand up slowly before trying to move around the room. Use the bathroom and hallway handrails.Keep things within easy reach such as:

• the call button • water• tissue • phone• commode • walker• cane • wheelchair • shoes • any other items that you use often

If necessary ask staff to place them within reach.• If something gets spilled or you see any liquid or debris on the floor, tell

a staff member.• Wear non-slip supportive footwear. Regular socks and slippers are

slippery on hospital floors. Do not wear clothing that has straps that hang to the floor or robes that are too long.

• If the side rails on your bed are up, do not climb over the rails or out of the end of the bed. Call for help.

For Family and FriendsIf you are unsure about getting the patient up by yourself please ask for assistance. If the patient is going to be alone, please let staff know when you are leaving. Leave the door and curtain open enough so that staff can easily see the patient from the doorway of the room.Fall Precautions - What to expect when a patient is on “Fall Precautions” 3 The patient will have a FALL RISK identifier. 3 More supervision is given for patient safety.3 Consultation to the falls prevention team (nurses, physiotherapist,

occupational therapist, dietitian, pharmacist, and other service providers).

Fire SafetyRoutine fire drills are announced over the hospital announcement system. When you hear the fire bell, please return to your room and ask your visitors to stay in your room and wait for further instructions. Your door will be closed by a nurse. Staff will check on you, keep you informed and help you evacuate in case of a real fire.

Flowers/BalloonsSometimes a roommate or staff may be allergic or sensitive to flowers. Some nursing staff have severe allergies to lilies. Always wash your hands after handling plants or flowers. Due to allergies latex balloons are not allowed in the hospital, foil balloons are allowed.

Gift ShopYarmouth Regional Hospital:Run by the Womens Auxiliary the gift shop sells many gifts, snacks, coffee, magazines, newspapers and more. Located in the lobby we are open:Weekdays ........ 9:00 a.m. to 8:30 p.m., Saturdays ........ 11:00 a.m. to 4:00 p.m. Sundays ........... 1:00 p.m. to 4:00 p.m.Gift shop volunteers deliver the daily newspaper six mornings a week. We have a cart that makes visits on Tuesday and Thursday afternoons to patients’ rooms.Roseway Hospital:The Roseway Hospital Auxiliary operates a gift shop near the main entrance to the hospital. They have cards, personal items, small gifts, newspapers (Chronicle Herald & Coast Guard), old paperback books, confectionery items and fresh coffee. Their usual hours are: Monday to Friday ......9:00 a.m. to 4:00 p.m.

Hairstylist ServiceYarmouth Regional Hospital: Your nurse can arrange an appointment with a hairstylist. You may also ask your own hairstylist or barber to come in and do your hair.Roseway Hospital: A Hairstylist is available at Roseway Manor (next door) from Mondays to Fridays. For an appointment, call 902-875-2200 or ask your nurse or the Ward Clerk to call for you.

Health RecordsYour hospital health record starts when you are registered. It holds all of the information about your care while you are in the hospital. You can access your health record by visiting your hospital’s Health Records department and filling out a “Release of Information Form” stating the

information you want to access. To view your chart there is no charge. If you want a copy of your record the charge is a $25.00 flat fee plus $1.00/page for the first 15 pages and then $10.00 for each half hour spent copying.

Hospital Services Not Covered By MSISome take home supplies and special services are not covered by MSI. You can buy a starter kit of medications to use until you can get your prescription filled.

Hospital Overstay PolicyOnce a patient is documented medically stable and is no longer in need of acute care hospital services or is awaiting placement in a long term care facility, they are no longer entitled to insured hospital services. They may be required to: leave hospital or be subject to $79.00 charge for each day after a three day notice period. There is an appeal process through the CEO of the district.

Identification BraceletWhen you are admitted an id bracelet with your name and birth date will be put on your arm. It is important that the care provider asks you your name to confirm your identity. They may ask other questions about your identity. They should also look at the bracelet before giving any medications or treatments. If the bracelet becomes damaged or comes off please tell your care provider.

Infection Prevention and ControlGerms spread easily in hospital from person to person. Everyone must practice good infection prevention and control at all times. Keeping your hands clean is the best way to prevent the spread of infection. Please remember to wash your hands often during your stay with either soap and water or hand sanitizer found in the entrances of the hospital and outside patient rooms. Ways you can help to stop the spread of infection:

• Remember to clean your hands often.• Remind visitors and staff to wash their hands or use the alcohol hand

wash outside your room before they visit you.• Use paper towel to shut off the taps after you’ve washed your hands. • Keep your hands away from your eyes, nose and mouth

• Keep your hands away from your IV tube, catheter tube, open wounds, etc.• Remind visitors not to come to the hospital if they are ill.• Cover your coughs and sneezes with a tissue or cough into your sleeve!

A good handwash takes 10-15 seconds. You can time yourself by singing Happy Birthday or the ABCs. Remind your visitors if they are feeling unwell that they should not visit you or other patients in the hospital. This includes people with the flu, stomach viruses, chickenpox, etc. Sometimes staff and visitors wear gowns and masks to prevent germs from spreading from patient to patient. These precautions help protect everyone in the hospital environment.

Information about a PatientPlease choose one person to be your contact person. That person can give other family members and friends updates about your condition. Your contact person can call staff to see how you are doing.

Kitchen (Digby General Hospital) A kettle and small fridge are located in the galley on the ward. Ask your nurse to show you the location. Food and beverages are stored there for patients’ use for nourishment between meals. Perishable food brought in to patients must be stored appropriately and not left at the bedside. Label it with patient’s name, room number and date. Always check with the nurse before eating this food, if you are on a special diet or fasting for tests.

Leaving the Patient Care UnitNursing staff need to know where you are while you are under their care. You may be asked to sign a release form when you leave the unit you are in (ie: to go to the cafeteria, gift shop or to visit another patient outside the unit). When your family/friends leave, ask them to keep your room door open so the nurse can see you and make sure you are safe.

Living Room (Roseway Hospital)A kettle, microwave and fridge are located in the patient living room. Food and beverages are stored there for patients’ use for nourishment between meals. Perishable food brought in to patients must be stored appropriately and not left at the bedside. Label it with patient’s name, room number and date. Always check with a nurse before eating this food if you are on a special diet or fasting for tests.

MedicationsTell the health care provider about any medications you are on and what you have brought with you. Know what you are taking and why you are taking it. Make sure you know the name, the amount and how many times a day you take it. Remember to include any over-the-counter medications, herbal products, and supplements. Make sure you tell your health care provider of any allergies or reactions you may have had. Make sure your health care provider identifies you correctly before giving any medications. Ask what the medication is and what it is used for each time. Ask questions if it looks different than what you have had before. You may be asked to go to the bathroom before taking something for sleep.When you are admitted you will also be given a brochure on medication safety. Visit the SWH website under Patient Info - Patient Safety/My Medication Record for helpful information such as

• My Medication Record – a place to write down your medications • Patient Safety Tips – poster to help you become an active, involved, and

informed patient

Nurse Call BellIf you need a nurse, please use the call bell. It is attached to your bed. A call bell is also found in all washrooms.

Organ DonationWe support the work of the Nova Scotia Organ and Tissue Donation Program. You will be asked about your wishes related to donation. Even through you might have signed your MSI card for donation, it is important to talk to your family about your wishes.

Paid ParkingYou must pay $2.00 to exit the parking lot. If you visit often, you can buy a monthly parking card from the Business Office. The parking card allows unlimited parking.

Palliative and Supportive CarePalliative Care Program This is a district wide program available to patients and their families in Digby, Shelburne and Yarmouth. There is a palliative care nurse and social worker available at all sites as well as a physician located in Yarmouth who is accessible to all patients/families within the district through a physician consult. The Palliative Care Team works with other health care providers to alleviate pain and suffering and to provide the best quality of life for patients and families.Supportive CareSocial Work: We provide both palliative and medical social work to patients and families. There are a wide range of services provided, both in hospital and in the community.Spiritual/Religious Care: During your stay in hospital, spiritual care is available to you. On admission to hospital, you can identify your wish for spiritual care visits. Please tell a member of your health care team if you would like a visit from a clergy member at any time during your stay. There is a chapel available for patient/family use at all 3 sites.VolunteersThere are many dedicated volunteers within Digby, Shelburne and Yarmouth hospitals.. This service provides support to many areas such as inpatient units, the Cancer Centre, the Veteran’s Unit and the Emergency Department. These volunteers provide a wide range of services. There are several programs provided through Volunteer Services.

Care Clown Program: This program provides trained volunteers that work within the health care setting. Patient Satisfaction SurveysWe use many different methods to survey our patients about their experience with us. Patients are surveyed in hospital and sometimes we survey patients once they have returned home by mailing them a questionnaire.Patients have the right to refuse to participate in a survey. Patient SafetyThe South West Health Care Team helps to identify possible patient safety risks. We work to prevent these risks and to improve the quality of care we provide. We encourage you to become part of the team. Take an active role in keeping yourself and your family safe while in hospital. Many of our patients are elderly and have poor hearing and sight. In the “unfamiliar” hospital environment, there is increased risk of falls. Family and friends can help by ensuring the patient is safe when they leave, eg. if bedrails where lowered during the visit, please put them up before you leave. Check to be sure the patient’s nurse call bell is within reach.

Personal DirectivesThe Personal Directives Act (PDA) is a valuable tool for Nova Scotians to plan for their future. The PDA allows you to put in writing your wishes in regards to health care decisions, in the event you become incapacitated and cannot consent to treatment. Health care decisions may be medical treatment, personal care, or placement in a long term care facility. Under the Act, these documents are referred to as Personal Directives.The Act also specifies an ordered list of who can act as a substitute decision maker (also sometimes called a delegate) for you if you do not have a written Personal Directive.For more information go to http://gov.ns.ca/just/pda/ or talk to your health care provider.

Physician VisitsOur physicians usually make inpatient rounds daily in the morning, but may visit later in the day as well. If the need should arise to see a physician “after-hours”, the nurse on the unit would contact your family physician. If unavailable, the nurse would contact the doctor “on call”.If a family member wishes to see a physician regarding your condition, a meeting with the physician can be arranged through the nurse in charge of your care. This meeting will be set up for a time that is good for you and the doctor. You should pick one person to be the spokesperson.

Preparing To Go Home (Discharge Planning)Staff can help you arrange support and care after you leave the hospital, such as:

• Information about services and equipment available in your area. • Referrals to care facilities such as nursing home or residential care. • Arrangements for other services you need such as Home Care, respite

care, Meals on Wheels and Veteran Affairs. • Contacts for financial assistance.

Please send extra items home with your family and have them bring in other items as the time they are needed (such as clothing). The more items each patient brings to the hospital, slows downs our cleaning staff, and makes it harder to implement our infection control cleaning practices. If there is something you would like brought in from home, please check with a nurse first. For safety reasons, your wallet and valuables should be sent home with your family.

ValuablesYou are responsible for your own clothing and items you keep in your room. Please bring only necessary personal items to the hospital, including just a small amount of cash.You can deposit valuables in the hospital’s safe. Deposits can be made at any time, but the best time to do so is when you are admitted. You will be given a receipt. The original receipt must be presented to withdraw money or valuables from safekeeping.CAUTION: When you remove your eyeglasses, false teeth, hearing aides, etc., make sure you place them in an appropriate container and put the container in your bedside table. Do not wrap items in Kleenex, papertowels or place them on your food tray or on your bed. If you have any questions, please do not hesitate to ask your caregiver.

Vending MachineRoseway Hospital:A snack machine with fruit, muffins, milk, snack and other items is located in the hallway on the 2nd floor, just before the doors to the Roseway Manor. There is a pop machine there, as well as two on the main floor in the outpatient waiting area. Digby General Hospital:Snack/Pop machines are located in the cafeteria and in the outpatient waiting area (ground floor).

Visiting HoursYarmouth Regional Hospital: Visiting hours are from 11:00 a.m. to 8:00 p.m. daily, with permission. Please, only one to two people to visit at one time so that it does not tire the patient. Exceptions can be made in special circumstances. Peds (Pediatric Unit)A family member is encouraged to stay overnight with their child. They can come and go anytime within reason.

Cancer - The Oncology Interactive Education Series (OIES)This computer software benefits patients, families and health care professionals by providing up-to-date information on any of 24 specific types of cancer (such as breast, lung, colorectal and prostate cancers). Patients and families benefit from viewing 2-dimensional images of the anatomy and learning about treatment options. They can also review interviews with cancer survivors enabling them to learn from the experience of others who have been diagnosed with the same type of cancer. This program is available through the cancer patient and family resource room. Volunteers are available to support people who want to use this resource. Previous computer experience is not necessary to use this program.

Cardiovascular ClinicsCardiovascular health is about keeping blood vessels in leading to your heart and your head open and healthy. There are many risk factors slowing down the flow of blood through blood vessels. Look Good Feel BetterThis program is designed to support women challenged by the appearance related side-effects of their cancer treatment. Workshops are held monthly at Yarmouth Regional Hospital. Lymphedema TreatmentSouth West Health offers treatment for people suffering from the effects of lymphedema. Lymphedema is an abnormal collection of protein-rich fluid in the body. If left untreated, it can lead to infections, enlargement of the affected limb and hardening of the skin. This treatment is available primarily to women who have developed lymphedema due to mastectomy, removal of lymph nodes, radiation or any combination of these interventions. A physician referral is required.

Pre-Assessment ClinicAt the clinic you will be seen by a nurse who will help you get ready for your upcoming surgery. You will be asked a lot of health questions and given time to ask your questions. You will be given information regarding your surgery as well as information to help you prepare before, during and after the surgery. Testing can also be done such as blood tests, heart test, urine test, etc. Referrals to other health care staff may be done at this time (ie: physiotherapist, dietitian etc.).

Prenatal ClinicLocated on the 2nd floor of Yarmouth Regional Hospital, the clinic serves women from the tri-counties from the time pregnancy is confirmed until delivery. The clinic operates from 8:00 a.m. to 4:00 p.m. Monday to Thursday. The clinic provides care to you and your baby for a safe delivery. It will provide a good setting for all mothers to receive quality care. It will help the mother and family make informed choices. This will lead to a healthier Mom and baby.The clinic is made up of five doctors and a nurse educator. The doctors will

care for you and your growing fetus during your pregnancy, labor and delivery. The nurse will answer any questions you have about your pregnancy.After your pregnancy is confirmed your family doctor will refer you to the clinic. Public Health ServicesPublic Health works with others to understand the health of our communities and acts together to improve health. Public Health is focused on the health of the population. To do this, public health practitioners work with communities, families and individuals in a variety of ways including advocacy and policy development, planning and evaluation, community development, health promotion, population health, disease prevention and control. Rehabilitation ServicesThe rehabilitation services team works to help people who have an injury or illness that has changed their daily life. They also work with persons with special needs. Inpatient and outpatient physiotherapy and occupational therapy services are available from each hospital site. Ask your doctor if you would benefit from a referral for these services. People without a family doctor can call the nearest rehabilitation services department to ask about a self referral:

Renal Dialysis UnitOur renal Dialysis unit serves patients from Shelburne, Yarmouth and Digby counties. Most patients receiving renal dialysis are treated three times a week. South West Health also works closely with the Nova Scotia Renal Program www.nsrp.gov.ns.ca. The Renal Dialysis Unit is open 6 days a week from 7:00 a.m. – 11:00 p.m.

Wound and Ostomy ClinicProvides inpatient and outpatient care for acute and chronic wounds and ostomy care. Wounds such as leg and foot ulcers, skin tears and surgical wounds are treated. Call the Wellness Centre at 902-742-3542 ext. 1460 or the resource nurse 902-742-3542 ext. 1695. Clinics are held at Yarmouth Regional, Digby General and Roseway Hospitals.
