St. Joseph Earns Highest Honors From HANYS St. Joseph Hospital was named by the Healthcare Association of New York State (HANYS) as one of the winners of the 2017 HANYS Pinnacle Award for Quality and Patient Safety. The award was presented at a ceremony held by HANYS in June. St. Joseph was honored for successfully addressing alarm fatigue, which resulted in a significant improvement in patient room quietness. St. Catherine’s also won for its initiative that enabled the hospital to exceed state and national performance in administering plasminogen activator (tPA) to stroke patients. “This achievement reflects the high level of professionalism and dedication to patient safety,” said CHS Executive Vice President and System Chief Medical Officer Patrick M. O’Shaughnessy, DO. “Through the implementation of systematic performance improvement strategies, the team at St. Joseph has significantly raised the bar.” St. Joseph’s “Reducing Rule-Based Error: Implementation of PDSA and Its Impact on Alarm Fatigue” resulted in a 39% reduction in non-critical clinical alarms per patient day (exceeding the original goal), a decrease of more than 9% in transfers to a higher level of care and a 26% improvement in HCAHPS Quietness of Room at Night. St. Joseph was chosen from among 86 statewide nominations. Winners were selected in four categories: three for acute care based on bed count and one for post-acute/outpatient providers. Fall is here, kids are back in school and the holiday season is fast approaching! As part of our mission to improve our community’s health, this newsletter helps keep you informed and offers valuable information. You will read about the services the hospital provides, recent quality achievements, new

As the Executive Vice President and Chief Administrative Officer of St. Joseph Hospital, I am confident in the quality of our services, and I am especially proud of our team of physicians, nurses and health care professionals who deliver that care. It is a responsibility and a privilege to treat more than 34,000 patients every year. Though bricks and mortar are essential, the heart and soul of St. Joseph rests in its people. A member of Catholic Health Services, St. Joseph is a 203-bed, community hospital with comprehensive inpatient and outpatient medical, critical care and surgical services. Other services include a stroke center, ambulatory surgery unit, The Center for Sleep Medicine, The CHS Center for Hyperbaric Medicine and Wound Healing, a diabetes education center, speech and swallow services, a full range of outpatient radiology services, with an MRI, cardiac CT scanner and CHS Cancer Center.

The hospital has expanded its outpatient capabilities with a state-of-the-art infusion center. The Catholic Health Services Cancer Center at St. Joseph Hospital, a six-chair infusion unit, provides intravenous therapy in a comfortable and controlled environment. Chemotherapy administration in a community hospital is safe and allows the patient the convenience of remaining close to home.

Designed for patient satisfaction and comfort, the treatment unit delivers specialized, high quality, individualized care. Under the supervision of Bhoomi Mehrotra, MD, chair of Catholic Health Services’s Oncology Service Line and director of oncology for St. Francis Hospital and Natalya Krichmar, MD, chief of hemotology/oncology at St. Joseph, the unit offers:

• A staff of certified infusion and chemotherapy nurses

• Nurse practitioners board certified in oncology

• Treatment available six days a week

• Convenient morning to early evening hours

St. Joseph has secured the Get With The Guidelines® (GWTG) AFIB Gold Quality Achievement Award from the American Heart Association and Heart Rhythm Society. St. Joseph earned the award by meeting specific quality achievement measures, including the proper use of medications and aggressive risk reduction therapies, such as safe anticoagulants to prevent stroke.

“This honor recognizes our commitment to atrial fibrillation care,” said St. Joseph’s Executive Vice President and Chief Administrative Officer Peter Scaminaci. “Evidence indicates patients benefit when treated at hospitals that have adopted the GWTG program through fewer readmissions and better quality outcomes.”

GWTG AFIB was developed to assist health care professionals provide the most up-to-date, research-based guidelines for patients with atrial fibrillation, an irregular heartbeat that can lead to stroke and other complications. Also evaluated is the usage of medications to stabilize the heart rate and rhythm and other medications required to treat additional heart disease. Before discharge, patients receive education on how to manage their condition, learn about potential stroke risk and receive counseling and plans for follow-up care.

CHS Marks 20 Years as a System With a Refocused Mission

In celebration of 20 years as an integrated health care delivery system, Catholic Health Services (CHS) is further articulating its mission, vision and value statements. This development supports CHS’s organizational identity, continuing the legacy of the founding religious sisters who were inspired by their love for Jesus to establish hospitals and other agencies across the region. Many decades of growth and the emerging needs of local communities have prompted strategic advances in CHS’s delivery of quality services to all Long Islanders. Through the combined efforts of a diverse array of professionals, CHS remains faithful to and focused on this rich heritage.

Although the technology used has become more sophisticated, the organization’s core purpose has remained constant: bringing Christ’s healing mission to those in need. This is true for all of CHS’s services, regardless of whether they are inpatient or outpatient care, residential or day programs, in a patient’s home or in a clinical setting. Outstanding contributions to the nursing profession and to the field of health care must be demonstrated. Going above and beyond and participating in activities that foster a positive public image of nursing as a profession are some of the criteria required to be considered a Nassau-Suffolk Hospital Council Nurse of Excellence. St. Joseph’s Pam Martin, RN, BSN, exemplifies these qualities and more.

Born in Manchester, Jamaica, Pam’s family inspired in her compassion and caring, especially her grandmother who was a midwife and caregiver. Her parents stressed the importance of a formal education, so when she came to the United States at age 13, school was an important priority. Beginning her career as an licensed practical nurse, she proceeded to a bachelor’s in nursing and master’s in nursing education—while balancing the demands of being a wife, mother and student.

Professionally, Pam is an advocate for patients and staff who embraces today’s health care challenges with an open mind and offers innovative solutions to any issues. Her calm demeanor ensures her patients receive quality care, and she holds herself and colleagues to equally high standards.

Pam is also the CNA coordinator for the Long Island Employment Opportunity Center at Farmingdale State College. She is actively involved in her

church—The Amityville Full Gospel Tabernacle Church and is an ordained clergywoman. Her missionary work has taken her to the West Indies, Jamaica and Panama visiting orphanages and bringing school supplies, clothes and food. After Hurricane Ivan in 2005, Pam co-founded United Neighborhood Network Association, a not-for-profit, community-based organization encouraging concerned Jamaicans and friends to “Do Your God’s Work Now” by sending essential supplies to those in need.

St. Francis Imaging Services at St. Joseph The hospital’s Cardiac CT Center operates under the umbrella of St. Francis Imaging Services and is open Monday through Friday, 8 am to 3 pm for elective scans. The tests are interpreted by St. Joseph Hospital’s Associate Chief of Cardiology Salvatore Trazzera, MD, FACC, FACP, FSCCT and Director of Cardiac Imaging at St. Catherine, St. Charles and St. Joseph Lev Lubarsky, DO, FACC, FASE, FSCCT, with results available within 24 hours. Marie Iannone of Farmingdale has been volunteering at St. Joseph for more than 30 years. A former contract administrator for the Department of Defense, Marie resigned due to health issues. Not wanting to sit home, Marie decided to volunteer. When she first started, her husband worked during the day, so she took a cab to and from the hospital. Once, Mike, her husband of 64 years, retired, he began

escorting Marie back and forth. Whenever Marie thinks of giving up the volunteer position, Mike reminds her that she is the “Information Lady”, and that people need her to be there for them. Marie works the front desk every Wednesday and not only directs visitors but also offers an ear to someone who wants to talk, or comfort to someone in need. She often is stopped in a store or restaurant by people whose lives she has touched. “I love what I do. I help people who are often having a tough time. I smile and try to make their day a little better.”

Marie the proud mother of three—two of whom work for Catholic Health Services—and has eight grandchildren and two great grandchildren.

Support Groups Volunteer Spotlight: A Woman of Distinction

The hospital’s Diabetes Prevention Program was awarded full recognition by the Centers for Disease Control and Prevention (CDC) as part of the National Diabetes Prevention Program. St. Joseph uses the CDC-approved curriculum, and its program has fulfilled the performance criteria of the Diabetes Prevention Recognition Program standards.

Program participants who are identified as being at risk for Type 2 diabetes learn lifestyle changes. Risk factors include being overweight, having a family history of Type 2 diabetes, being physically inactive and/or having had gestational diabetes. Members master the skills needed to lose weight, become more physically active and how to manage stress. A trained lifestyle coach guides and encourages participants, with support provided by other members of the group. There are 16 weekly sessions and 6 monthly follow-up sessions to help maintain the healthy lifestyle changes acquired through the program.

The newly renovated Observation Unit opened at the end of July. The six-bed unit offers a spacious, comfortable environment for patients requiring additional time for evaluation and treatment, but who may not need admission. The unit cares for approximately 8 patients per day who remain an average of 10 hours. Only about 10% of the patients are converted to inpatient status.

Since the beginning of the year, St. Joseph has been blessed by having a relationship with the Adults & Children with Learning and Developmental Disabilities, Inc. (ACLD) in Bethpage. The ACLD is a not-for-profit agency that serves the needs of individuals with developmental disabilities and their families. Part of the program ACLD offers the people they serve includes recreation and art therapies. St. Joseph has been the recipient of handmade coloring books for both adult patients and young children who may be visiting or waiting in the emergency room. The ACLD has also given the hospital colorful bookmarks, beautifully decorated pens, and greeting cards—all handmade by the creative people attending ACLD.

The items they donate are not only a huge benefit to St. Joseph but also give the people attending ACLD a great feeling

knowing that their items are being enjoyed by people in the hospital. The smiles on people’s faces is a great indication of the joy these handmade pieces bring.

Gala ShinesSt. Joseph’s 2017 Gala, “A Night to Shine” was a success with 375 guests enjoying a fun-filled evening at the Crest Hollow Country Club. This year’s honorees, Vijay Shah, MD, Giuseppe Caruso, MD, and Ron Steimel, helped raise more than $170,000. The proceeds will be used for Emergency Department improvements. Thanks to New Island Diagnostics, Classic Casino Productions and Maximum Entertainment for their support.

Thunderbirds Visit St. JosephAt the end of May, the hospital was honored to receive a visit from the world-famous U.S. Air Force Thunderbirds flight crew and engineers. The crew met with patients, staff and visitors prior to the Jones Beach Memorial Day Air Show. They also spent extra time with some patients who were veterans, and posed with former Air Force member and hospital volunteer Joe Giuliano. “It was so exciting,” said Joe. “I wouldn’t have missed this opportunity for anything!”
