MUKH MANTRI PUNJAB HEPATITIS C RELIEF FUND (MMPHCRF)

1. Background: The epidemiological data on hepatitis C in Punjab is limited. A

recent study conducted in 10 districts suggest that prevalence of hepatitis C is

3.29% (anti-HCV positive) (Sood et al, 2015; Personal communication).

According to Census of India 2011, Population of Punjab was

approximately 28 million. Thus approximately 9.2 lakhs persons may be positive

for hepatitis C antibody, while approximately 6.1 lakh persons could be viremic

requiring treatment for hepatitis C considering two-third of them are HCV RNA

positive. Majority of these patients would develop progressive liver disease and

ultimately develop cirrhosis of liver and its complications including liver failure

requiring liver transplantation, variceal bleeding, severe infections, hepatocellular

carcinoma (HCC) and death. Presently hepatitis C is the leading cause of HCC in

Punjab. Hence treatment of chronic hepatitis is not only mandatory but should be

done early in the course. Successful treatment of hepatitis C was associated with

62-84% reduction in all-cause mortality (deaths), 68-79% reduction in risk of HCC

and 90% reduction in risk of liver transplant (Hill et al 2014). Hence treating

hepatitis at the appropriate time is very cost effective and in addition it saves lives.

With the advent of Direct Acting Antivirals (DAAs), the treatment has not only

become very effective (cure rates >95%) but also cheaper.

Treating all persons with hepatitis C along with preventive measures will

ultimately eliminate HCV infection from Punjab.

With this goal, Mukh Mantri Punjab Hepatitis C Relief Fund (MMFHCRF)

has been created to benefit these patients and save thousands of lives in Punjab.

2. Mukh Mantri Punjab Hepatitis C Relief Fund: MMFHCRF has been created to

provide free medical treatment to patients suffering from chronic hepatitis C in the

State of Punjab. The funds under this scheme will be provided by Finance

Department, Punjab and will be utilized for procurement of the drugs for the free

treatment of the hepatitis C cases in the State.

3. Beneficiary: The beneficiary of this scheme has to be a resident of the State of

Punjab. Any of the following can be used as a proof of residence:

a. Ration Card

b. Passport

c. Driving License

d. Aadhar Card

e. Voter Card

f. Bank or Post Office current Pass Book

g. Driving License

A copy of the proof of the residence of the State has to be kept by

the pharmacist. The copy of the proof will be signed by the pharmacist and

countersigned by the SMO of the District Hospital or nodal officer of the GMC. The

copy has to be affixed in the master register of MMPHCRF.

4. HCV Testing

HCV is primarily transmitted through percutaneous exposure to blood,

e.g., injections, tattooing, haircuts with infected razors, etc. Other modes of

transmission include blood transfusions, mother-to-infant, sexual transmission, etc

a. Testing

i. HCV antibody testing should be done with ELISA.

ii. Quantitative HCV RNA should be performed at baseline in all persons

who comes positive for anti-HCV by ELISA.

iii. HCV Genotyping is mandatory in hepatitis C patients who have liver

cirrhosis; otherwise it is optional.

iv. Anti-HCV is an antibody-based test, thus might remain positive

throughout life. Therefore it should not be repeated again for

determining the response to HCV treatment. Determination of response

to HCV treatment must be done only by quantitative HCV RNA testing

during and after the treatment with antivirals. This information may be

shared with patients undergoing treatment against hepatitis C to avoid

any confusion in the minds of patients.

b. Who should be tested?

i. One-time HCV testing is recommended for persons received blood

transfusion or its products before 2002

ii. Risk behaviours

1. Injection-drug use (current or ever, including those who injected once)

iii. Risk exposures

2. Long-term hemodialysis (ever)

3. Getting a tattoo in an unregulated setting

4. Healthcare, emergency medical, and public safety workers after

needlesticks, sharps, or mucosal exposures to HCV-infected blood

5. Children born to HCV-infected women

6. Prior recipients of transfusions or organ transplants

iv. Other

HIV infection

5. Baseline Testing

a. CBC for hemoglobin, WBC and platelets

b. Liver function tests (Bilirubin, AST, ALT, Albumin/globulin, alkaline

phosphatase)

c. Serum creatinine

d. Ultrasound of abdomen

e. Evaluation for diagnosis of cirrhosis [See below section 6.1]

6. Treatment of Patients with Hepatitis C:

The technical committee of the experts from Department of Hepatology,

PGIMER, Chandigarh has formulated 4 regimens for the treatment of Chronic

Hepatitis C cases. The regimens have been recommended for ease of the treating

physicians in order to classify the cases as per viral load and genotype of Hepatitis

C (Table 1 and 2 and figure 1).

Regimen-1: Tablet Sofosbuvir (400mg/day)+Ledipasvir (90mg/day)

Regimen-2: Tablet Sofosbuvir (400mg/day)+Ledipasvir (90mg/day) and Capsule

Ribavirin (1000 mg/day if body weight <75 kg and 1200 mg/day if body

weight is ≥75 Kg)

Regimen-3: Tablet Sofosbuvir (400mg/day) + Tablet Daclatasvir (60mg/day)

Regimen-4: Tablet Sofosbuvir (400mg/ day) + Tablet Daclatasvir (60mg/day) and

Capsule Ribavirin (1000 mg/day if body weight <75 kg and 1200

mg/day if body weight is ≥75 Kg)

All patients must be treated as per algorithm shown in Figure 1.

¶Systematic Review and Network metaanalysis have demonstrated that both regimen (Ledipasvir and daclatasvir based) are equally effective achieving sustained viral response (SVR, cure) in non-cirrhotic patients, both naïve and treatment experienced, while adverse effects are also similar (WHO Guidelines 2016). Hence HCV Genotyping is not mandatory in HCV patients without cirrhosis; it remains optional because it does not affect the ultimate outcome, i.e., SVR (cure). SVR in these patients ranges between 95% to 100%. However, HCV genotyping and Resistant Associated Variants (RAVs) analysis are recommented in those very small number of patients who do not achieve SVR and should be referred to Department of Hepatology, PGIMER, Chandigarh.

§Patients with decompensated cirrhosis should be referred to Department of Hepatology, PGIMER, Chandigarh. For details see below “Decompensated Cirrhosis”; Section 4.3.

7 Special Conditions:

7.1 Evaluation of Cirrhosis: The diagnosis of cirrhosis is considered and ribavirin use

is recommended if any of the following situation is present.

o Clinical Diagnosis of Cirrhosis: The clinical diagnosis of cirrhosis is

based on clinical (stigmata of chronic liver disease, splenomegaly,

ascites, history of hepatic encephalopathy or variveal bleed, etc.),

imaging (shrunken liver, irregular surface, altered echotexture,

evidence of portal hypertension (portal vein >13mm and presence of

collaterals) and endoscopic (presence of varices) parameters. All

patients must undergo abdomen ultrasound to look for evidence of

cirrhosis and for the presence of HCC. Please note that patients with

decompensation of cirrhosis or with HCC, must be referred to tertiary

care center (PGIMER, Chandigarh).

o Fibroscan (Transient Elastography):

Cirrhosis (METAVIR F4): LSM ≥12.5 kPa

Safety of Fibroscan in pregnancy has NOT been established. Hence,

it must not be used in a pregnant lady.

If Fibroscan (Transient Elastography) is not available, then the

absolute platelet counts, the AST-to-platelet ratio index (APRI) or FIB-4

index score can help in making diagnosis of cirrhosis.

o Platelets <75,000/cmm

o APRI = [(AST (IU/L)/ AST_ULN (IU/L)) x 100]/ platelet count (109/L)

An FIB-4 index higher than 3.25 had a positive predictive value to

confirm the existence of a significant fibrosis (F3-F4) of 82.1% with a

specificity of 98.2%.

7.1.1 Ribavirin Use: The dose of ribavirin should be monitored with

haemoglobin level, because Ribavirin is known to cause Haemolysis. If

significant anaemia occurs (haemoglobin <9 g/dL), the dose of ribavirin

should be decresed by 200 mg at every decrement. If the baseline

haemoglobin is low then a more rapid reduction of dose may be

required. Ribavirin administration should be stopped if the haemoglobin

level falls below 8.0 g/dL and the patient be referred to a higher centre.

Contraindications to therapy with ribavirin

1. Pregnancy or unwillingness to use contraception

2. Breastfeeding women

3. Severe concurrent medical disease, including severe infections

4. Poorly controlled cardiac failure

5. Chronic obstructive pulmonary disease

6. Previous ribavirin hypersensitivity

7. Co-administration of didanosine

8. Renal failure

7.2 Decompensated Cirrosis: The above-mentioned regimens should be

prescribed only to persons with compensated cirrhosis because they can

cause severe complications when prescribed to persons with

decompensated cirrhosis (presence of ascites, jaundice, history of

hepatic encephalopathy and variceal bleed or Child-Pugh score ≥7*Class

Table 3). Therefore, they should be used only in settings where

specialized care including liver transplantation is available and where the

degree of cirrhosis (compensated vs decompensated) can accurately be

assessed.

7.3 Therapy with direct-acting antivirals: contraindications/warnings

2. Safe in all grades of renal failure

Sofosbuvir

1. Amiodarone co-administration (caution also with beta-blockers)

dabigatran etexilate, digoxin, erythromycin, loperamide, protease

inhibitors, tacrolimus, etc.)

3. Renal failure (eGFR <30 mL/min/1.73 m2); sofosbuvir in a dose of 200

mg/d is relatively safe

7.4 Pregnancy and lactation

Safety data of new DAAs in pregnancy and lactation has not been

established and ribavirin is also teratogenic. Treatment currently cannot

be recommended during pregnancy, especially with ribavirin-based

regimens. Hence pregnancy should be excluded by appropriate test

before starting treatment for hepatitis C. Use of contraception be

ensured in a woman in a childbearing age for a period of 6 months after

the treatment has concluded.

7.5 Vaccination against hepatitis B is recommended for all susceptible persons

with HCV infection.

7.6 Pb-PGIMER-INASL-ECHO Clinic:

This clinic has been started on 7 June 2016; it will be held every 2 weeks

on Tuesdays between 11.00 am and 1.00 pm. The ECHO clinic is basically

a case-based learning exercise that gives an immense opportunity to all

of us to interact in the benefit of patients. Hence, difficult to treat

patients or any query may be discussed in this Clinic. In addition there is

a didactic lecture covering recent advances.

8. Drugs for HCV

8.1 Procurement of Drugs: The drugs for four recommended regimens shall be

procured by PHSC (Punjab Health Systems Corporation) after calling tenders

and doing rate contract as per rules.

8.2 Storage of drugs: The drugs for four recommended regimens after

procurement will be stored at 3 drug ware houses of the Department of Health

and Family Welfare (Verka, Bathinda and Kharar). The drugs from these ware

houses will be shifted to the linked districts.

8.3 Availability of the drugs for Hepatitis C:

The drugs for four recommended regimens will be available at the pharmacy

of three Govt Medical Colleges (GMC Amritsar, Faridkot & Patiala) and 22

District Hospitals in the first phase. These 25 health institutions will procure

drugs from the three drug ware houses and sub stock of these drugs will be

maintained by the pharmacist of these health institutions.

9. Reporting and Recording of the Hepatitis C cases:

The flow of the patients (suspected/ confirmed) and recording of chronic

Hepatitis C in the Government Hospitals will be as following:

9.1 A patient suspected to be suffering from Chronic Hepatitis C will be

examined by Medical Specialist (03 GMCs and 22 District Hospitals)

9.2 The patient will be sent for anti-HCV antibodies testing in the hospital

laboratory (ELISA test to be used for anti-HCV)

The patient will have to pay for the anti-HCV test.

9.3 All patients tested positive for Hepatitis C in the hospital (03 GMCs and 22

DHs) will be entered in Master Register of Hepatitis C where a Unique

Identification Digit (UID) will be issued and complete record of the patient will

be entered.

9.4 All patients tested positive for anti-HCV in the hospital (03 GMCs and

22 DHs) will be referred to collection centre of Lal Path Lab for blood collection

for Hepatitis-C viral load.

The samples will be collected daily at the collection centre of Lal Path Lab

(Annexure I)

The prescription slip will be stamped by the pharmacist for the type of the

test to be conducted and cost of the test in the Lal Path Lab.

Collection centre will retain a copy of the treatment card (referral for

testing) for record purposes.

The results of the samples will be conveyed every Monday and Thursday.

(The results of the samples collected by Monday will be conveyed on

Thursday and results of samples collected by Thursday will be conveyed

by next Monday)

The hard copy of the results will be available in the collection centre.

Patient can also obtain the result from the website of the Lal Path Lab.

The rate of viral load is Rs 2200/test

Patient will have to pay for these tests.

9.5 The patient having detectable viral in the viral load test will be examined

by the medical specialist for cirrhosis as per 7.1, 7.2 & 7.3 of this SOPs.

9.6 If the patient is not having cirrhosis, the test for genotype is optional.

Further course of management will be as per algorithm shown in Figure 1

on page 7 of these SOPs. All non-cirrhotic patients, both naïve and

treatment experienced, will receive Sofosbuvir (400 mg) + Daclatasvir (60

mg) daily for 12 weeks with the exception of naïve patients with genotype

1 or 4 with low viral load (<6 millions). These patients may be offered

treatment with Sofosbuvir (400 mg) and Ledipasvir (90 mg) daily for 8-

weeks.

9.7 If patient is having cirrhosis, the genotype test is mandatory. The patient

will be referred to collection centre of the Lal Path Laboratories for

genotype test.

The rate of genotype test is Rs 3000/test.

Patient will have to pay for these tests.

9.8 The test results of the Hepatitis-C viral load and genotype will be entered in

the Master Register by the pharmacist of the 03 GMCs and 22 DHs. The

patient will then be referred to the Medical Specialist for starting the

treatment as per reports and as per algorithm in Figure 1 on page 7 of

these SOPs.

In brief, cirrhotic patients with genotype 1 or 4 will receive Sofosbuvir (400

mg)+Ledipasvir (90 mg)+weight based Ribavirin combination for 12 weeks

and those with genotype 3 will receive Sofosbuvir (400 mg)+Daclatasvir (60

mg)+weight based Ribavirin for 24 weeks.

9.9 Treatment Card for Hepatitis C will be issued to the patient after entering

the personal details and treatment regimen.

Treatment card will be filled by the pharmacist as per the record in the

Master Register of the hospital.

9.10 The medical specialist will decide the regimen of the treatment as per the

genotype of the Hepatitis C as shown Table 1 & 2 on page 6 and algorithm

on page 7 of these SOPs. The treatment regimen will be entered by the

Medical Specialist in the treatment card with signatures.

9.11 It is desirable that the Medical Specialists should get the following tests

done before the start of the treatment

9.11.1 CBC: especially for Haemoglobin, Platelets and Total WBC counts

9.11.2 USG to look for presence of cirrhosis and HCC (Hepatocellular

Carcinoma). Refer to higher centre if HCC is present.

Patient will have to pay for these tests.

9.12 The drugs as per regimen will be issued by the pharmacist to the patient

after checking the entry of Medical Specialist on the treatment card. The

detail of the drugs issued will be entered in the Master Register by the

pharmacist.

10 Follow up: The patient will be followed up every 4 weeks (28 days) for the

entire duration of the treatment

10.1 First follow up: The first follow up will be around 28 (24-28) days after

starting the treatment. Discontinuity in the treatment is not desirable, hence

patients be advised to come 2-3 days prior to total consumption of medicines. The

medical specialist should examine the following:-

CBC report for haemoglobin, WBC and platelet levels. If initial

haemoglobin level is borderline (i.e., 9 or 10 g/dL), then follow up

after 7-10 days of starting treatment with CBC is desirable.

Number of tablets consumed to ensure compliance of treatment

The medical specialist will issue the next course of drugs for 4 weeks (28 days) on the treatment card and the same will be issued by the pharmacist after entry into Master Register and empty bottle of first month will be taken back by the pharmacist.

10.2 Second follow up: The second follow up will be done approximately 28 days

after the first follow-up, i.e. 8 weeks after starting the treatment. The Medical

Specialist will examine the patient for any complication and drug compliance.

The medical specialist will issue the next course of drugs 4 weeks (28 days) on the treatment card and the same will be issued by the pharmacist after entry into Master Register and empty bottle of first month will be taken back by the pharmacist.

10.3 Third follow up: The third follow up will be on completion of drugs for

another 4 weeks or 28 days (one bottle) of therapy. The Medical Specialist will

examine for any complication and drug compliance.

Viral load testing on completion of treatment for end of

treatment response (ETR) is optional (Figure 1). The viral load test after

treatment completion for ETR might be done if patient is willing and can afford.

Patient will have to pay again for the viral load test (2200/-) at

one of the collection centre of Lal Path Lab.

If the Hepatitis-C virus becomes undetectable then ETR is

achieved. This report result will be entered by the medical specialist in the

treatment card of the patient as “ETR (+)” and in the Master Register by the

pharmacist.

10.4 Fourth follow up: It will be done 12 weeks after completion of treatment or

after achieving ETR to check (Sustained Viral Response) SVR or Cure. If the

Hepatitis-C virus remains undetectable, then SVR or Cure for the HCV infection is

achieved. It should be documented and entered by the medical specialist in the

treatment card of the patient as “SVR (+)” and by the pharmacist in the Master

Register.

Patient will have to pay again for the viral load test (2200/-) at one of

the collection centre of Lal Path Lab.

No further treatment is required if the patient has achieved SVR

i.e. SVR (+).

Cirrhotic patients with genotype 3 (Table 1 and 2 and Figure 1) will receive

treatment for 24 weeks. Hence, 3rd HCV viral load should be done after 24

weeks of treatment instead of 12 weeks.

All cirrhotic patients who have been cured, i.e., SVR(+) must be followed up at

higher centre for the surveillance of Hepatocellular Carcinoma (HCC) and for

monitoring progression of the disease and development of other

complications.

Queries related to complications, side effects during the treatment of chronic

Hepatitis C cases should be discussed in “Pb-INASL-ECHO-Clinic” which is done

every 2 weeks except holidays between 11:00 am to 1:00 pm.

11 District Epidemiologist under IDSP will be overall nodal officer for

implementation of “Mukh Mantri Punjab Hepatitis C Relief Fund” (MMPHCRF)

scheme in the district. The master data of the cases of Hepatitis C will be retrieved

by the district IDSP team from the hospitals and will be analyzed as and when

required.

12 The data entered in the Master Register in each DH and GMC and drug stock

will be cross verified by Civil Surgeon, Medical Superintendent of GMC, Senior

Medical Officer of the DH and District Epidemiologist from time to time for smooth

implementation of the “Mukh Mantri Punjab Hepatitis C Relief Fund”(MMPHCRF)

scheme.

13 Bank Account: To provide financial assistance for the Hepatitis-C patients, a

society “Mukh Mantri Punjab Hepatitis C Relief Fund” (MMPHCRF) has been

constituted. The bank account of society at State headquarter will be operated

jointly by State Programme Officer Hepatitis-C and Director Health & Family

Welfare Punjab. The funds received from FD for MMPHCRF will be deposited in

this account and payment of drugs procured for treatment of Hepatitis C and other

approved activities will be booked in this account. Regular monitoring of this

account will be done by IDSP wing.
