Aims and objectives What are your aims and objectives in providing the regulated activities and locations shown in part 3 of this statement of purpose
Anglian Community Enterprise (ACE) Community Interest Company provides services that are aimed at improving health, promoting well-being and preventing ill-health. We aim to prevent unavoidable admissions in to the acute sector by supporting people to remain in their own homes wherever practicable. Our community teams are wrapped around GP practices and work closely with social care and with the local acute provider to ensure seamless services for our patients. In addition to domiciliary care we also provide services within clinic environments. We also provide inpatient rehabilitation within our 2 community hospitals and primary care services within our 2 GP practices. ACE provide services predominantly to North East Essex, however we do reach into other parts of Essex and Suffolk. ACE is a staff owned social enterprise and delivers services under NHS contracting regulations. Our strategic objectives are:- 1. Within this site we run peripatetic services and house managerial functions – including training, finance, HR, IT and performance and assurance. The regulated activities that are managed from this site are Personal Care, Treatment of Disease, Disorder or Injury and Surgical Procedures. The CQC service definitions provided from this site are Community Healthcare Services (CHS) and Community Based Services for people with a Learning Disability (LDC) The operational teams deliver community based physiotherapy, community nursing services, intermediate care services, childrens nursing and therapy services, pain and muscular skeletal services (some of which require surgical intervention, ie, x-ray guided pain injections). The operational services that are managed from this site are headed by our appropriately qualified Directors and Associate Directors and are supported by their Integrated Care Managers and teams at different locations. The complete site, known as Clacton Hospital, is leased by ACE and is shared with other tenants including Colchester Hospital Foundation Trust, North Essex Foundation Trust, and Essex County Council. The regulated activities that are managed from this site are Treatment of Disease, Disorder or Injury. The CQC service definitions provided from this site are Community Healthcare Services (CHC) Acute Services (ACS) Doctors Consultation Services and Urgent Care Services (UCS). Within this site Anglian Community Enterprise Community Interest Company (ACE CIC) manages 37 inpatient beds for older people and rehabilitation across two inpatient wards. ACE CIC also provides the nursing component to a day case unit providing haematology and chemotherapy and also runs a minor injuries unit, outpatient services for physiotherapy and podiatry and occupational health. Within the minor injury unit X-ray facilities are accessible by our staff but the X-ray department is managed by Colchester Hospital Foundation Trust (CHUFT). Within each location and service there is a senior manager with delegated responsibility and authority to ensure the regulated activity is carried out in accordance with compliance. These service managers report to the responsible manager and escalation procedures are in place. Service delivery is monitored via a number of routes including activity data, compliance assessments, patient feedback and staff surveys. We have Integrated Care Managers for every team who are responsible for the day to day operational management of clinical service delivery with clear lines of accountability from all services up to the Board. The registered manger regularly visits all of the locations to gain assurance of compliance with the fundamental standards. The above applies in respect of all locations.
