StandardforAttendanceStudents are required to attend a cumulative attendance of 67% or higher of their scheduled hours in order to beconsideredmaintainingSatisfactoryAcademicProgress.Evaluationsareconductedattheendofeachevaluationperiodto determine if the student hasmet theminimum requirements. If the student falls below the 67% at the time ofevaluationthestudentwillbeplacedonaSAPwarning.AttheendoftheSAPwarningifthestudenthasfailedtoreachthe 67% attendance rate the student may be terminated from the program. However, if the student has madesignificantadvancementtomeettheattendancestandardof67%andtheAcademyDirectorcanreasonablydeterminethatthe67%attendancestandardcanbeachieved,thestudentmaythenbeplacedonaprobationstatusafteraprevailupon appeal by the student has been approved allowing the student to continue in the program until the nextevaluationperiod.

DeterminationOfProgressStatusStudentsmeetingtheminimumrequirementsforacademicsandattendanceattheevaluationpointareconsideredtobemakingSatisfactoryAcademicProgressuntilthenextscheduledevaluation.

FinancialAidStudentswhodonotachievetheminimumstandardforSatisfactoryAcademicProgressarenolongereligibleforTitleIV,HEAprogramfunds,ifapplicable,unlessthestudentisonwarningorhasprevaileduponappealofthedeterminationthathasresultedinthestatusofprobation.

WarningStudents who fail tomeetminimum requirements for attendance or academic progress are placed onwarning andconsideredtobemakingSatisfactoryAcademicProgresswhileduringthewarningperiod.Thestudentwillbeadvisedinwriting on the actions required to attain SatisfactoryAcademic Progress by the next evaluation. If at the endof thewarning period, the student has still notmet both the attendance and academic requirements, the studentmay beterminatedfromtheprogramorplacedonprobation.

FinancialAidWarningTheschoolevaluatesSAPattheendofeachpaymentperiod.StudentswhodonotmeetSAPmeasuresdescribedabovewillautomaticallybeputonaFinancialAidWarningforonepaymentperiod.Thisstatusmayonlybegiventoastudentwhowasincompliancewiththeinstitution’sSAPstandardsatthepreviousevaluation.StudentswhoareputonaFinancialAidWarningwillcontinuetoreceiveTitleIVaidforthenextpaymentperiodsaftertheyreceivethewarningstatus.Thestudent’sacademicprogresswillbeevaluatedattheendofthewarningperiod.Itmaynotbeassignedforconsecutivepaymentperiods.TheFinancialAidWarningstatusisappliedonlytostudentswithareasonableexpectationofimprovementinonepaymentperiod.IfastudentismakingSAPattheendoftheFinancialAidWarningperiod,theyshallbereturnedtonormalSAPstatuswithnolossofTitleIVeligibility.Astudent,whohasnotimprovedhis/herstandingandstillfailstocomplywiththesatisfactoryacademicprogresspolicyattheendoftheFinancialAidWarningperiod,willbeineligibleforfutureaiddisbursements.ItisadvisableforanystudentonwarningstatustomeetwithboththeFinancialAidDirectorandAcademyDirectortoensurefuturesatisfactoryprogressandaideligibility.Thestudentmaycontinueonacashpaybasiswithanapprovedpaymentplanandaresubjecttospaceavailability.Arrangementsforpaymentmustbeapprovedwithinten(10)schooldaysafternotificationoflossofTitleIV.

AppealProcedureIfastudentisdeterminedtonotbemakingSatisfactoryAcademicProgress,thestudentmayappealthedetermination.Reasons for which studentsmay appeal a negative progress determination include death of a relative, an injury orillnessof the student,oranyotherallowable specialormitigating circumstance. The studentmust submitawrittenappeal to the school on the designated form describing why they failed to meet Satisfactory Academic Progressstandards, along with supporting documentation of the reasons why the determination should be reversed. Thisinformationshouldincludewhathaschangedaboutthestudent’ssituationthatwillallowthemtoachieveSatisfactoryAcademicProgressbythenextevaluationperiod.Appealdocumentswillbereviewedandadecisionwillbemadeandreportedtothestudentwithin30calendardays.Theappealanddecisiondocumentswillberetainedinthestudentfile.Ifthestudentprevailsuponappeal,theSatisfactoryAcademicProgressdeterminationwillbereversed.

FinancialAidAppealProcessTheFinancialAidAppealprocessallowsstudentswhoarenotmeetingSAPstandardstopetitiontheinstitutionforreinstatementofTitleIVaideligibility.StudentsmayhavetheopportunitytohavetheirfinancialaideligibilityreinstatedbyappealingthedecisionandbeplacedonFinancialAidProbation.Studentshavefive(5)businessdaysafterbeingnotifiedtoinstituteandappeal.TheappealmustbeinwritingandgiventotheFinancialAidDirector,whowillmeetwiththeFinancialAidAcademicAppealsCommitteetomakeadecisionontheappeal.Thebasesonwhichastudentmayfileanappealincludedeathofarelative,aninjuryorillnessofthethestudent,oranyotherallowablespecialormitigatingcircumstance.Studentsmusprovidesupportingdocumentsanddescribeinwritinganyunusualcircumstancesthatwarrantspecialconsideration.StudentsmustprovideinformationastowhytheydidnotmakeSAPandwhathaschangedthatwillallowthemtomakeSAPbythenextevaluationpoint.TheFinancialAidDirectorwillprovideadecisioninwritingwithinten(10)businessdays.ProbationStudentswhofailtomeetminimumrequirementsforattendanceoracademicprogressafterthewarningperiodmaybeplacedonprobationandconsideredtobemakingSatisfactoryAcademicProgresswhileduringtheprobationaryperiod,ifthestudentappealsthedecision,andprevailsuponappeal.Additionally,onlystudentswhohavetheabilitytomeettheSatisfactoryAcademicProgressPolicystandardsbytheendoftheevaluationperiodmaybeplacedonprobation.The student will be advised in writing of the actions required to attain Satisfactory Academic Progress by the nextevaluation period. If at the end of the probationary period, the student has still notmet both the attendance andacademic requirements required for Satisfactory Academic Progress, the studentwill be determined as NOTmakingSatisfactoryAcademicProgressandthestudentwillbeterminatedfromtheprogram.

FinancialAidProbationIftheinstitutionapprovestheappeal,thestudentmaybeplacedonprobationaftertheinstitutionevaluatesthestudent’sprogressanddeterminesthatthestudentdidnotmakeSatisfactoryAcademicProgressduringthewarningorpreviousevaluationperiod.Probationwillonlybeforoneadditionalpaymentperiodduringwhichthestudentremainseligibleforfederalaid.Thestudentwillbeplacedonanacademicplantoregainsatisfactoryacademicprogressstatustocontinuetobeeligibleforfederalaidbeyondtheinitialprobationaryperiod,thestudentmusteitherbemeetingthecumulativeattendanceandgradestandardsaspublishedbytheschool,orbeincompliancewithanycustomizedwrittenacademicplan.Theacademicplanmaybeascomplicatedasacourse-by-courseplantowardcompletion,orassimpleasamathematicalformulathatspecifiesthepercentageofcourseworkthatthestudentmustnowcomplete.Anacademicplanmaycontinueformultiplepaymentplans.Thestudentisnowonfinancialaidprobation.AtthecompletionoftheplanthestudentwilleitherbeincompliancewithSAPorhavecompletedtheprogram.Ifthestudentfailstomeetthestandardsestablishedintheplan,thestudentwillnolongerbeeligibleforTitleIVaid.Intheacademicplan,theschooldeterminestheminimumgradepointaveragethestudentmustachieveandthenumberofclockhoursthatthestudentmaytake.Anacademicplancanbechanged,ifthestudentfilesanotherappeal.Thereasonsforchangingtheacademicplanarerelatedtounusualcircumstancesthatpresentedthestudentfrommeetingtherequirementsoftheoriginalacademicplan.Thesereasonsmayinclude:tworequiredcoursesareonlyavailableinthesametimeslot;theschoolisunabletoofferarequiredcoursethetermoutlinedintheplan;orpersonalsituationoremergency.Alldocumentsconcerninga

writtenappealofaSAPevaluationmustbekeptinthestudentfile,includingtheschoolswrittendeterminationofsuchappealanddetailsofanyacademicplanthatmaybedeveloped.

Re-EstablishmentOfSatisfactoryAcademicProgressStudentsmayre-establishSatisfactoryAcademicProgressbymeetingminimumattendanceandacademicrequirementsbytheendofthewarningorprobationperiod.

Interruptions,CourseIncompletes,Withdrawals,orRepetitionsIfenrollmentistemporarilyinterruptedforaLeaveofAbsence,thestudentwillreturntoschoolinthesameprogressstatus as prior to the leave of absence. Hours elapsed during a leave of absencewill extend the student’s contractperiodandmaximumtimeframebythesamenumberofdaystakenintheleaveofabsenceandwillnotbeincludedinthestudent'scumulativeattendancepercentagecalculation.Studentswhowithdrawpriortocompletionofthecourseandwishtore-enrollwillreturninthesameSatisfactoryAcademicProgressstatusasatthetimeofwithdrawal.Courseincompletes, repetitions,andnon-credit remedial courseshavenoeffectupon the institution’s satisfactoryacademicprogressstandardsiftheinstitutionhasnosuchitemsorpolicies.

TransferHoursWith regard toSatisfactoryAcademicProgress, a student’s transferhours fromanother institution thatareacceptedtoward the student’s educational program are counted as both attempted and completed hours for the purpose ofdeterminingwhentheallowablemaximumtime-framehasbeenexhausted.SAPevaluationperiodsarebasedonactualcontractedhoursattheinstitution.

RequirementsforEligibilityforLicensureTheCaliforniaBoardofBarberingandCosmetologyhasdefinedtherequirementsforeligibilityforlicensureasaCosmetologistasfollows:7321.Theboardshalladmittoexaminationforalicenseasacosmetologisttopracticecosmetologyanypersonwhohasmadeapplicationtotheboardinproperform,paidthefeerequiredbythischapter,andisqualifiedasfollows:

LeaveofAbsencePolicyALeaveofAbsence(LOA)isatemporaryinterruptioninastudent’sprogramofstudy.LOAreferstothespecifictimeperiodduringaprogramwhenastudentisnotinattendance.InordertobeplacedonaLeaveofAbsence,thestudentmust:

1. Completeandsigntheschool’sLeaveofAbsenceRequestForminadvanceandsubmitanexactdateofreturntotheschool.

2. MuststatethereasonfortheLeaveofAbsence(LOA)request.ReasonsforwhichaLOAmaybegrantedaremedical,personal,orvacation.

3. MustbeapprovedbytheAcademyDirector.ApprovalofaLOAisnotguaranteed.4. MustbeinSatisfactoryProgressforattendanceat67%orhigherandgradepointaverageat80%orhigher.5. Leavesmustbeaminimumoffive(5)calendardaysandamaximumofsixty(60)calendardaysandmustnot

ThepolicyrequiresastudenttoapplyinadvanceforanLOAunlessunforeseencircumstancespreventthestudentfromdoingso.Forexample,ifastudentwereinjuredinacaraccidentandneededafewweekstorecoverbeforereturningtotheschool,thestudentwouldnothavebeenabletorequesttheLOAinadvance.TheschoolmaygrantaLOAtoastudentwhodidnotprovidetherequestpriortotheLOAduetounforeseencircumstances,iftheschooldocumentsthereasonforitsdecisionandcollectstherequestfromthestudentatalaterdate.Inthisexample,thebeginningdateoftheapprovedLOAwouldbedeterminedbytheschooltobethefirstdatethestudentwasunabletoattendtheschoolbecauseoftheaccident.Theschoolwillextendthestudent’scontractperiodandmaximumtimeframebythesamenumberofdaystakenintheLOA.Changestothecontractperiodontheenrollmentagreementmustbeinitialedbyallpartiesoranaddendummustbesignedanddatedbyallparties.TheschoolmaynotassessthestudentanyadditionalschoolchargesasaresultoftheLOA.ThestudentwillbewithdrawnifthestudenttakesanunapprovedLOAordoesnotreturnbytheexpirationofanapprovedLOAandthatthestudent’swithdrawaldateforthepurposeofcalculatingarefundwillbethestudent’slastdateofattendance.Inordertograntaleaveofabsencetheremustbetheexpectationthatthestudentwillbereturningtoschool.AstudentwhoisgrantedaLOAthatmeetsthesecriteriaisnotconsideredtohavewithdrawnandnorefundcalculationisrequiredatthattime.

Failuretomaintainacumulative67%ofattendancerateorhigher.Studentswhohavenotcompletedthecourse

HousingTheschooldoesnotprovideon-campushousingnorhaveanydormitoryfacilitiesunderitscontrol.Theschoolisnotobligatedtoprovidehousingandhasnoresponsibilitytofindorassistastudentinfindinghousing.However,therearerentalcommunitiesnearbytheinstitution'sfacilityandtheapproximatecostofhousingisbetween$950–$1,650/month.

TitleIVWithdrawalPolicy

“Official”VoluntaryWithdrawalAstudentisconsideredtobe“officially”withdrawnonthedatethestudentnotifiestheFinancialAidDirectororAcademyDirectorinwritingoftheirintenttowithdraw.Thedateofthedeterminationforreturnandrefundpurposeswillbetheearliestofthefollowingforofficialwithdrawals:Thedatestudentprovidedofficialnotificationofintenttowithdraw,inwritingorthedatethestudentbeganthewithdrawalfromtheschool’srecords.Astudentwillbepermittedtorescindhis/hernotificationinwritingandcontinuetheprogram,ifsochosen.However,ifthestudentsubsequentlydrops,thestudent’swithdrawaldateistheoriginaldateofnotificationofintenttowithdraw.Uponreceiptofthewithdrawalinformationtheschoolwillcompletethefollowing:

1.Determinethestudent’slastdateofattendanceasofthelastrecordeddateofacademicattendanceontheschool’sattendancerecord,and2.Performtwocalculationsa.Thestudent’sledgercardandattendancerecordarereviewedtodeterminethecalculationofReturnofTitleIV,HEAfundsthestudenthasearned,andifany,theamountofTitleIV,HEAfundsforwhichtheschoolisresponsible.ReturnsmadetothefederalfundsaccountarecalculatedusingtheU.S.DepartmentofEducation’sReturnofTitleIV,HEAfundsworksheets,scheduledattendanceandarebaseduponthepaymentperiod.b.Calculatetheschool’srefundrequirement(seeInstitutionalRefund/WithdrawalPolicycalculation).3.Thestudent’sgraderecordwillbeupdatedtoreflecthis/herfinalgrade.4.TheschoolwillreturntheamountforanyunearnedportionoftheTitleIV,HEAfundsforwhichtheschoolisresponsiblewithin45daysofthedatetheofficialnoticewasprovided.5.Ifapplicable,theschoolwillprovidethestudentwithaletterexplainingtheTitleIV,HEArequirements.Toinclude,a.TheamountofTitleIV,HEAassistancethestudenthasearned.Thisamountisbaseduponthelengthoftimethestudentwasenrolledintheprogrambasedonscheduledattendanceandtheamountoffundsthestudentreceivedb.Anyreturnsthatwillbemadetothefederalprogramonthestudent’sbehalfasaresultofexitingtheprogram.Ifastudent’sscheduledattendanceismorethan60%ofthepaymentperiod,he/sheisconsideredtohaveearned100%ofthefederalfundsreceivedforthepaymentperiod.Inthiscase,nofundsneedtobereturnedtothefederalfunds.c.Advisethestudentoftheamountofunearnedfederalfundsandtuitionandfeesthatthestudentmustreturn,ifapplicable.6.Supplythestudentwithaledgercardrecordnotingoutstandingbalanceduetotheschoolandtheavailablemethodsofpayment.Acopyofthecompletedworksheet,check,letterandfinalledgercardwillbekeptinthestudent’sfile.Intheeventastudentdecidestorescindhisorherofficialnotificationtowithdraw,thestudentmustprovideasignedanddatedwrittenstatementindicatinghe/sheiscontinuinghisorherprogramofstudy,andintendstocompletethepaymentperiod.TitleIV,HEAassistancewillcontinueasoriginallyplanned.Ifthestudentsubsequentlyfailstoattendorceasesattendancewithoutcompletingthepaymentperiod,thestudent’swithdrawaldateistheoriginaldateofnotificationofintenttowithdraw.

UnofficialWithdrawalAnystudentthatdoesnotprovideofficialnotificationofhisorherintenttowithdrawandisabsentnotmorethanfourteen(14)consecutivecalendardays,failstomaintainsatisfactoryacademicprogress,failstocomplywiththeschool’sattendanceand/orconductpolicy,doesnotmeetfinancialobligationstotheschool,orviolatesconditionsmentionedintheschoolcontractualagreement,willbesubjecttoterminationandconsideredtohaveunofficiallywithdrawn.Withintwoweeksofthestudent’slastdateofacademicattendance,thefollowingprocedureswilltakeplace:1.Theschoolwillmakethreeattemptstonotifythestudentregardinghis/herenrollmentstatus.2.Determineandrecordthestudent’slastdateofattendanceasthelastrecordeddateofacademicattendanceontheattendancerecord.3.Thestudent’swithdrawaldateisdeterminedasthedatethedayafterfourteen(14)consecutivecalendardaysofabsence.4.Notifythestudentinwritingoftheirfailuretocontacttheschoolandattendancestatusresultinginthecurrentterminationofenrollment.5.Theschoolcalculatestheamountoffederalfundsthestudenthasearned,and,ifany,theamountoffederalfundsforwhichtheschoolisresponsible.6.Calculatetheschool’srefundrequirement(seeInstitutionalRefund/WithdrawalPolicycalculation).7.TheFinancialAidDirectorwillreturntothefederalfundprogramsanyunearnedportionofTitleIVfundsforwhichtheschoolisresponsiblewithin45daysofthedatethewithdrawaldeterminationwasmade,andrecordonthestudent’sledgercard.8.Ifapplicable,theschoolwillprovidethestudentwitharefundletterexplainingTitleIV,HEAaidrequirements:a.TheamountofTitleIVaidthestudenthasearnedbaseduponthelengthoftimethestudentwasenrolledandscheduledtoattendintheprogramandtheamountofaidthestudentreceived.b.AdvisethestudentinwritingoftheamountofunearnedTitleIV,HEAaidandtuitionandfeesthathe/shemustreturn,ifapplicable.9.Supplythestudentwithafinalstudentledgercardshowingoutstandingbalanceduetheschoolandavailablemethodsofrepayment.10.Acopyofthecompletedworksheet,check,letter,andfinalledgercardwillbekeptinthestudent’sfile.

PostWithdrawIfyoudidnotreceiveallofthefundsthatyouhaveearned,youmaybedueapost-withdrawdisbursement.TheSchoolmayuseaportionorallofyourpost-withdrawdisbursementfortuitionandfees(ascontractedwiththeschool).Forallotherschoolcharges,theschoolneedsyourpermissiontousethepost-withdrawdisbursement.Ifyoudonotgivepermission,youwillbeofferedthefunds.However,itmaybeinyourbestinteresttoallowtheschooltokeepthefundstoreduceyourdebtattheschool.Thepost-withdrawaldisbursementmustbeappliedtooutstandinginstitutionalchargesbeforebeingpaiddirectlytothestudent.Bothgrantsandloansmustbedisbursedwithin180daysofthedateofdeterminationinapost-withdrawaldisbursement.
