ACADEMIC PROGRAM

School supplies - To achieve academic success, all students must come to class prepared to learn. Northridge Middle School students must

arrive in class with pencils, pens, paper, books and other needed materials, including homework assignments. Failure to bring appropriate materials

to class daily severely limits a student’s ability to learn and may result in lower grades. A list of school supplies may be obtained from the school

office.

Grades - Honor Roll is determined based on an average of all grades towards a total GPA. Students who received a 3.0 or higher

without any D’s or F’s will qualify for honor roll recognition each 9 weeks.

Fees – K-12 fees, will be approved by the Superintendent and Treasurer/CFO jointly to determine costs if applicable. For athletics,

participation fees will be assessed per sport per student for all student athletes. For more information, please refer to the Athletic

Department’s Handbook. Additional fees may be charged for lost or damaged books or other damage to school property. Students

with unpaid fees will not be issued a final grade card.

MID-TERM REPORTS Mid-term reports are not formally sent home at the Middle School. Your child’s teacher will be in contact with you if they feel like

there is a concern that they need to address. You may always contact the teacher if you have concerns of your own.

HOMEWORK/PROGRESS BOOK Homework is assigned to help students become independent learners and to improve skills. Students are expected to complete

assignments and return them to school. Computer generated work must be approved in advance by the teacher. Any form of

plagiarism, such as copying from another student’s work or other print resources, is forbidden and will result in loss of credit for all

students involved. Progress Book is available to all student’s families/guardians to view homework assignments, grades, and

announcements from teachers. Logins and passwords will be given to families/guardians and students, simply contact the office.

MAKE-UP WORK A student who has missed school due to an excused absence will be expected to make up all assignments. It is the responsibility of

the student to check with teachers and make arrangements for making up missed work. Students will be given an extra day per day

absent to make up work. Unexcused absence and truancy situations do not permit the student to make up any missed work, quizzes, or

tests.

CODE OF ACADEMIC INTEGRITY At Northridge Middle School, we believe that all students can and must learn in order to become successful in our society. We further believe that this success begins with personal and academic integrity, which is grounded in honesty, trust, fairness, respect, and responsibility. Absolute integrity is expected of all students and staff at Northridge. Violations—The following are examples of activities that violate the Code of Academic Integrity. This is not a definitive list:

1. Knowingly representing the work of others as one’s own 2. Not citing passages, information, or sources from any published material 3. Making up information to support laboratory or field work 4. Copying another student’s work—OR—allowing another to copy 5. Forging a signature 6. Changing answers and seeking credit after work has been graded

ARRIVAL AND DISMISSAL OF STUDENTS Arrival – Middle school students should not arrive at school before 7:10 a.m. The school assumes no responsibility for the safety of

students who arrive before the designated time. The school cannot be responsible for the safety or behavior of students on their way to

school unless they are riding a school bus. Students arriving after 7:30 a.m. should report to the office before going to their

classrooms.

Dismissal - The school day for middle school students ends at 2:09 p.m. Students must immediately leave the school grounds upon

dismissal unless they are involved in an authorized after school activity. The school assumes no responsibility for the safety of

students who fail to leave the school grounds promptly at 2:10 p.m. The school is not responsible for the safety or behavior of

students on their way home unless they ride a school bus. Once students leave, they may not return to school unless they are

supervised by school personnel.

Students are not permitted to leave the school grounds with older students or adults, other than custodial parents, unless

arrangements have been made with the school in advance. Any student going home with a friend must have a note giving permission

to do this from his/her parent and another note from the parent of the student with whom they plan to go home. Arrangements to go

home should be made before coming to school. Students will not be permitted to call home to arrange such visits.

Parents are asked not to schedule appointments for students during school hours or take students out of school early unless it is

absolutely necessary. If a student has to leave early, he/she should bring a note in the morning. The parent must then come to the

office to sign the student out at the prearranged time.

Bus students must ride their assigned buses to and from school unless prior arrangements have been made with the secretary or

principal. A written notice is preferred.

Emergency closings & delays - If the school must be closed or the opening delayed because of inclement weather or other conditions, the

school will notify local news stations, post the closing or delay on the school’s web page at www.northridge.k12.oh.us., and provide notification of

school closings or delays via a pre-recorded telephone message delivered to the phone number on file. Parents and students are responsible for

knowing about emergency closings and delays.

When school is dismissed because of bad weather, all after school and evening school activities are cancelled at the middle school.

SCHOOL ATTENDANCE AND ABSENCE GUIDELINES School attendance policy - The educational program offered by this District is predicated upon the presence of the student and

requires continuity of instruction and classroom participation. Attendance shall be required of all students enrolled in the schools

during the days and hours that the school is in session or during the attendance sessions to which s/he has been assigned.

In accordance with statute, the Superintendent shall require, from the parent of each student of compulsory school age or from an adult

student who has been absent from school or from class for any reason, a (written) statement of the cause for such absence. The Board

of Education reserves the right to verify such statements and to investigate the cause of each single absence or prolonged absence.

TYPES OF ABSENCES Excused

personal illness (a written physician’s statement verifying the illness may be required)

illness in the family necessitating the absence of the child

quarantine of the home

death in the family

necessary work at home due to absence or incapacity of parent(s)/guardian(s)

observation or celebration of a bona fide religious holiday

out-of-state travel (up to a maximum four (4) days per school year) to participate in a district-approved enrichment or

extracurricular activity (note: any classroom assignment missed due to the absence shall be completed by the student)

such good cause as may be acceptable to the Superintendent

Unexcused Failure to submit an excuse in writing within three (3) days upon return to school any absence not listed in Section 1 The Attendance Officer may be notified if:

Student has excessive absences (10% or more of the total days due)

Student is habitually or chronically truant

Written note - Any pupil not in attendance at school shall be required to present a written excuse from his parent or guardian stating

the reason for the absence upon returning to school. A telephone call does not substitute for the written note. Notes must be received

within three (3) days or the absence will be unexcused.

Telephone call - Parents should call the school office (740 967-6671) before 7:45 A.M. to report a child’s absence. Please report

the child’s name, grade, and reason for absence. This will enable school personnel to know that the parent is aware that the student is

home. If you do not call the school, an attempt will be made to reach you. (SB321 - Missing Children’s Act)

Family vacations - Student vacations during time periods when school is in session are discouraged. However, recognizing the

educational value of a family field trip, excused absences will be granted with the following guidelines:

1. A written request, signed by the parent, must be given to the principal a minimum of five (5) days prior to the beginning

date of the family field trip.

2. Make-up work is required and is to be returned to the appropriate teacher within three (3) days after the student returns.

Missed tests are to be taken. It is the student’s responsibility to make arrangements with the teacher for make-up work before

leaving.

3. A Maximum of five (5) days of excused absences will be granted each school year for family field trips.

4. Additional days used for purpose of vacation must be authorized by the local Superintendent of Schools, in

accordance with Section 3301-51-13 of the Administrative Code.

Truancy - Unexcused absence from school (truancy) is not acceptable. Students who are truant will receive no credit for school

work that is missed. If a student is habitually or chronically truant and the student’s parent fails to correct the student’s attendance, a

complaint will be filed with the Judge of the Juvenile Court in compliance with State Law Length of absence: Half day—7:30 AM - 10:50 AM or 10:50 AM -2:10 PM Tardy— 7:30 AM - 8:20AM Students are expected to be on time to school and to class throughout the day.

Tardiness - A student who is not in his/her assigned location by 7:30 AM shall be considered tardy. Any student arriving late to

school is to report to the school office before going to class. Unexcused tardiness is not acceptable.

Absence on extra-curricular/night activity days - Students must be in attendance at least one half of a school day in order to

attend after school activities.

BEHAVIOR EXPECTATIONS Safety and security - All visitors must report to the office when they arrive at school. All visitors sign in and required to wear a

building pass while they are in the building. Staff are expected to question people in the building whom they do not recognize and

who are not wearing a building pass, and to question people who are “hanging around” the building after hours.

Students and staff are expected to immediately report to a teacher or administrator any suspicious behavior or situation that makes

them uncomfortable. All exterior doors are locked during the school day. Portions of the building that are not needed after the regular

school day are closed off. All District employees are to wear photo-identification badges while on district property. The Licking

County Sherriff’s Department and Northridge Local Schools work together to ensure the safety of all.

Safety order and discipline - The Board of Education and school administration recognizes the rights of students as individuals.

It also recognizes that these rights must be balanced with the safety, order and discipline of an effective educational process in a

school environment conducive to the healthy growth and development of all students. Teachers must be able to teach, supervise, and

conduct their educational program. Students should not lose their right to a good education because of the disruptive actions of

another student. In general, we all must consider the rights of others and assume the responsibilities that our rights place upon us.

Northridge Middle School has a zero tolerance policy relative to alcohol, drugs, violence and weapons on school property. When it

comes to the safety, order or discipline of the Middle School, the administration will take appropriate action to assure that students can

learn and teachers can teach. In addition, discipline is not in and of itself punishment. It is utilized to educate in the matter of self-

control and interaction with others to reduce disruption and thereby increase the student’s learning time. Discipline is an educational

service to which every child is entitled to under F.A.P.E. (Free and Appropriate Public Education Act).

Cafeteria expectations - Students in the cafeteria are expected to be safe, responsible, and respectful.

Northridge harassment policy/anti bullying message - The Board encourages the promotion of positive interpersonal

relations between members of the school community. Harassment and or aggressive behavior (including bulling/cyber bullying)

toward a student, whether by other students, staff, or third parties is strictly prohibited and will not be tolerated. This prohibition

includes physical, verbal, and psychological abuse, and any speech or action that creates a hostile, intimidating, or offensive learning

environment. The Board will not tolerate any gestures, comments, threats, or actions which cause or threaten to cause bodily harm or

personal degradation. Individuals engaging in such conduct will be subject to disciplinary action.

Students or parents may file reports of conduct that they consider to be bullying or harassment by using any of the following

procedures:

Reporting directly to a school staff member Calling our Bully/Safety Tipline at 1-800-927-8161

Logging onto the Northridge website http://www.northridge.k12.oh.us/ and filing a report with the appropriate form

Such reports should be reasonably specific including persons involved, number of times and places of the alleged conduct, the

target of the suspected harassment/bullying, and the names of any potential witnesses. Incidents will be investigated, and appropriate

actions will be taken, consistent with the due process rights of the student(s) alleged to have committed acts of harassment,

intimidation, and/or bullying.

STUDENT CODE OF CONDUCT Violation by a student of any one or more of the following rules on school grounds may result in disciplinary action, including suspension, emergency removal from class and/or expulsion. These rules are not meant to be all-inclusive; a building administrator may use other options if they are deemed more appropriate. The district has jurisdiction over its students during the regular school day and while going to and from school on district transportation. The district's jurisdiction includes any activity during the school day on school grounds, attendance at any school-related activity, regardless of time or location, and any school-related misconduct, regardless of time or location. 1. Disruption of Class/School: A student will not interfere with school purposes or with the orderly operation of the school by using,

threatening to use, or causing other persons to use violence, force, coercion, threats, intimidation fear, or through disruptive means. This includes being out of an assigned seat, inappropriate interruptions, not following school or classroom rules, etc.

2. Profanity and/or Obscenity: A student will not use profane, indecent, or obscene language, either orally or in writing toward any student, teacher or any other person. Included in this prohibition would be the use of obscene gestures, signs, pictures, publications, and/or clothing with obscene words/pictures/messages.

3. Insubordination: A student will not fail to comply with the directions of teachers, student teachers, school aides, bus drivers, principals, or other authorized school personnel. Teachers are authorized to make any rules to their classrooms which may aid in the efficiency of the class.

4. Truancy: A student will comply with the compulsory school attendance laws. When a student is absent from school without a legal excuse, the student is truant or tardy. This includes unexcused absences from class or any other properly assigned activity. The truant officer may be notified.

5. Tardiness: Repeated tardiness is also considered a serious offense as it interrupts the educational process. 6. Public Display of Affection (PDA): Students will not engage in close physical contact. 7. Theft and Vandalism: A student will not steal, cause damage to, or destroy school or private property. Restitution, notification of law enforcement and/or disciplinary action may result. 8. Encouraging a Fight: Definition: Setting up a fight; verbally or physically encouraging a fight to start or continue; taunting. 9. Fighting: A student will not engage in a fight or any other type of physical altercation on school premises. 10. Intimidation and/or Threatening Behavior: Provocation that promotes unfriendly behavior that aggravates or irritates someone

such as name-calling, obscene language, and other abusive behavior is prohibited. Intimidation through direct or veiled verbal or nonverbal assaults or who exhibit belligerent, intimidating or threatening behavior. Threats, threatening behaviors, or other acts of violence executed off district property but directed at students, employees or other persons on district property or during district-sponsored activities are also in violation of the student code of conduct.

11. Dangerous Weapons, Instruments, and Objects: The Crime Control Act of 1990 makes it a federal offense for an individual to knowingly possess a firearm in a school zone or within a thousand feet of such grounds. The term "school zone" means the grounds of a public, parochial, or private school. A student shall be expelled from school if the student, on school property or while attending a school-sponsored or school-related activity on or off school property, uses, exhibits, or possesses a firearm. A student will not possess, handle, transmit, or conceal any object or substance which might be considered dangerous or capable of harming another person. These shall include but not be limited to firearms, knives, metal knuckles, straight razors, explosives (firecrackers), noxious irritation or poisonous gases, poisons, drugs (when shared with others), or other items.

12. Failure to Serve: A student will not fail to serve an assigned detention, or In-School Suspension. Transportation issues are not an excuse for missing.

13. Gambling: A student will not gamble on school premises or at school events. 14. Inappropriate Dress: The objective of this dress code is to provide an appropriate educational environment

while allowing students to dress comfortably within limits to facilitate learning. Students’ attire can have a positive or negative effect on the learning process, contribute to students’ success, and generate a safe and positive learning environment. We expect students to maintain the type of appearance that is not distracting to students, teachers, or the educational process of the school. Parents and children are equally responsible for the appearance of their child. There is appropriate and inappropriate attire for all of life’s activities. Please help your child adhere to these guidelines. Clothing should be clean, as should the body and hair. School is the place of “work” for all students. Students will be subject to the Northridge Middle School discipline policy. Students who violate the dress code may be warned the first time and appropriate alternative clothing secured. Additional violations may result in a phone call or conference with parents if appropriate attire cannot be arranged. Class work missed while arranging for appropriate clothing will receive a zero. The dress code is in effect during all extra-curricular activities.

A. Hats, sunglasses, coats/jackets, headgear, bandannas and other items normally worn outside are to be stored in book bags during the school day and not worn to class or other indoor activities. B. Clothing with “coined expressions” is prohibited. Trademark logos are acceptable, e.g. Nike, Hollister, Ohio State, etc C. No pajamas are to be worn. D. Clothing, personal articles, or tattoos with writing or symbols that make reference to sex, drugs, alcohol, profanity, tobacco, death, gangs, put downs, hate groups, violence or that are disruptive to the school process are prohibited. E. Shirts which offer a view of a student’s midriff at any point during school are prohibited for boys and girls. Shirts should be “Tuckable”. F. Tank tops and spaghetti strap tops are not permitted. Sleeveless shirts will be permitted but the shoulder area must be completely covered and the top should reach to the outer edge of the shoulder. No “cut-off” shirts are permitted. G. Shorts may be worn when the following guidelines are followed: a. Shorts must reach mid-thigh or below. b. Short shorts and tight shorts are not to be worn. H. Skirts must be mid-thigh or below. Slits in skirts must not come above mid-thigh. I. Pants must be worn above the hip with a belt if necessary. A shirt that covers the waistline does not excuse pants that are worn too low. Length of pants must not exceed the bottom of shoes. Writing across the buttocks is not permitted. J. Undergarments must never be seen. K. Any heavy chain on a student’s person used to secure wallets or worn as a belt, or worn as a necklace is prohibited. L. Pagers, cell phones, IPods, CD Players, and other electronic devices are prohibited. M. Book bags/backpacks may be used to transport books to and from school.. N. All students must wear shoes. Slippers and flip flops are prohibited. O. Ripped or torn clothing, including jeans, are prohibited. P. Students who violate the dress code will call their parent/guardian and sign in with office staff. Office staff will communicate with the referring teacher. Parents must pick up their child or deliver appropriate clothing. Q. Students who do not comply will change into something appropriate or be sent home. R. The administration has the final say as to the appropriateness of school attire. 15. Drugs and Alcohol: A student will not use, possess, transmit, conceal, sell or be under the influence of any alcoholic beverage,

narcotic drug, mind-altering substance, or regulated drugs, while on school property—including buses-- or while at any school-sponsored activity. Drug paraphernalia such as pipes, pacifiers, rolling papers, drain screens, “glow sticks” lighters/matches, or other drug paraphernalia as defined by the administration, in consultation with the SRO, are strictly prohibited.

16. Possession or Use of Tobacco: A student will not use or possess tobacco in any form in school buses, school buildings, on school grounds, or at any school-sponsored activities. A tobacco product in a student’s hand, whether lit or not, will result in suspension and will be treated as though it were being used. Effective March 2001, it is now prohibited by Ohio law for a minor to “use, consume, or possess cigarettes, other tobacco products, or papers used to roll cigarettes.” It is also now prohibited for a minor to assist, pay for or share in the costs of such products. A first offense results in (1) mandatory attendance at a smoking education program (if available) and/or; (b) a fine of up to $100. Upon any subsequent violations the court may (a) increase the fine; (b) impose up to 20 hours of community service; (c) suspend driving privileges for up to 30 days.

17. Supplements: Students will not use or possess nutritional supplements in any form in school buses, school buildings, on school grounds, or at any school-sponsored activities. This includes, but is not limited to, creatine and other muscle enhancement supplements, fat-burners/ weight loss supplements, vitamins, and any other performance enhancing supplement.

18. Trespassing or Loitering: A student will not be present in a school building or on school grounds at unauthorized times. 19. False Alarms: A student will not cause any false alarms or induce panic. 20. Violation of Law: A student will not violate any law or ordinance when on school property or while attending school-sponsored

activities. 21. Hazing: A student will not participate in hazing at any time. No student, including leaders of student organizations, shall plan,

encourage, or engage in hazing. 22. Cheating/Plagiarism: Presenting someone else’s work as one’s own in order to obtain a grade or credit is considered to be

cheating. This includes, but is not limited to copying others’ homework assignments, quizzes and/or tests and plagiarism. Students who violate this policy will receive a warning and zero credit for assignments

or work involved for the first offense. The second offense will result in suspension and zero credit and the third offense will result in suspension and zero credit with a possible recommendation for expulsion.

23. Forgery: A student will not falsify in writing the name of another person, times, dates, grades, addresses, or other data on school forms or correspondence directed to the school.

24. Electronic Devices: Electronic items (cameras, musical devices, game systems) should remain in the student’s book bag. The school is not responsible for stolen items.

25. Cell Phones: students are not permitted to use cell phones on school property during the school day. Cellular devices are to remain in book bags.

26. Bus Conduct: A student will not disobey school bus rules regarding conduct on school buses. 27. Failure to Comply: Students will follow directions that are reasonable and support the safety, order or discipline of the

classroom/school/activity. 28. Repeat Offender: A student will not repeatedly violate any school rule or accepted standards of school behavior. 29. Misconduct: A student will not engage in misconduct not otherwise defined.

DISCIPLINE PROCEDURES The following is a list of typical actions that a staff member might take in response to inappropriate behavior in the classroom or another activity during the school day: A. Conference with the student B. Classroom action - change of seat, etc. C. Miss recess or part of recess D. Written assignment - explanation of rule, written plan for improvement, etc. E. Have student come in before school, during lunch break or after school to discuss behavior and plan for better choices in the future F. Parent contact or conference G. Time out— remove student briefly from class to stop disruptions H. Contract with student I. Referral to the school administration J. After school detention — Any staff member may require that a student stay after school. The student is given a notice to take home at least 24 hours prior to the after school date, complete with the date to stay, reasons for the disciplinary action, the time the student should be picked up, and the name of the staff member assigning the after school detention. It is the parent’s responsibility to transport the child home. K. Suspension and expulsion—Major disciplinary actions such as suspension and/or expulsion from school are used in only the most serious disciplinary cases. The school principal may suspend a student out of school for up to a total of ten (10) school days for a specific offense. The superintendent of schools may expel a student from school for up to a total of eighty (80) days. This is used only in the most serious of cases and after much consideration. In –School Restriction or Out-of-School Suspension precludes participation in all school related functions and from being on the premises during the prescribed suspension. L. Emergency removal—If a student’s presence poses a continuing danger to persons or property, or an ongoing threat of disrupting the academic process taking place either in a classroom or elsewhere on the school premises, the Superintendent, principal or assistant principal may remove the student from any curricular or extracurricular activity or from the school premises. A teacher may remove the student from any curricular or extracurricular activity under the teacher’s supervision, but not from the premises.

Due process provisions for students - In all cases where disciplinary action is anticipated concerning a student, the student will

be given an opportunity at an informal hearing to state his or her case.

In case of an emergency removal from class or from school for more than twenty-four hours, a hearing will be scheduled within

seventy-two hours and the student will be given the opportunity to challenge the reasons for removal and any further action that may

be taken.

In case of suspension, a student will be given a written notice of the intention to suspend which will contain the reasons for the

suspension. Out-of-School Suspensions are considered the most severe of discipline procedures. Unlike In-School Suspensions, all

Out-of-School Suspensions carry with it the denial of credit for any missed work.

In the case of expulsion, the Superintendent will issue written notice of the intention to expel, which will include reasons for the

expulsion. In an expulsion, the student, parent, or representative has an opportunity to appear before a hearing officer designated by

the Superintendent to challenge the reasons for the expulsion.

Anti-harassment - It is the policy of the Board of Education to maintain an education and work environment which is free from all

forms of unlawful harassment, including sexual harassment. This commitment applies to all School District operations, programs, and

activities. All students, administrators, teachers, staff, and all other school personnel share responsibility for avoiding, discouraging,

and reporting any form of unlawful harassment. This policy applies to unlawful conduct occurring on school property, or at another

location if such conduct occurs during an activity sponsored by the Board. The Board will vigorously enforce its prohibition against

harassment based on sex, race, color, national origin, religion, disability, or any other unlawful basis, and encourages those within the

School District community as well as third parties, who feel aggrieved to seek assistance to rectify the problems. The Board will

investigate all allegations of harassment and in those cases where unlawful harassment is substantiated, the Board will take immediate

steps to end the harassment. Individuals who are found to have engaged in unlawful harassment will be subject to appropriate

disciplinary action.

Theft and personal valuables - Students should refrain from bringing large sums of money or other valuable articles to school.

The school is NOT RESPONSIBLE for the loss or theft of personal items. It is the student’s responsibility to maintain security for

his/her personal items.

Search and seizure - Administrators may search a student or his/her property (including vehicles, purses, knapsacks, gym bags,

etc.) with or without the student’s consent, whenever the administrator reasonably suspect that a search will lead to the discovery of

evidence of a violation of law or school rules. The extent of the search will be governed by the seriousness of the alleged infraction

and the student’s age. General housekeeping inspection of school property may be conducted with reasonable notice. Student’s desks

and/or lockers are the property of the District and students have no reasonable expectation of privacy in their contents or in the

contents of any other District property including tables or other containers. School authorities may conduct random searches of the

desks and/or lockers and their contents at any time without announcement. Unannounced and random canine searches may also be

conducted.

(Note: Signs accurately reflecting this policy MUST be placed in a conspicuous area.)

Additionally, students have NO reasonable expectation of privacy in their actions in public areas including but not limited to,

common areas, hallways, cafeterias, classrooms and gymnasiums. The District may use video cameras in such areas and on all

school vehicles transporting students to and from regular and extracurricular activities. Anything that is found in the course

of a search may be used as evidence of a violation of school rules or the law, and may be taken, held or turned over to the

police. The School reserves the right not to return items that have been confiscated.

Cell phones/wireless communications devices - Usage and possession Cell Phone/Wireless Communication Device is at the

discretion of building administration.

Bus safety/behavior expectations - For bus drivers to operate buses in a safe and efficient manner, it is necessary for students to

exercise self-control and use good judgment at all times.

Repeated misbehavior or disobedience will result in a suspension from the bus. Students waiting at any school in the district for bus transportation should arrive at the pick-up point no more than ten minutes or no less than five minutes before the scheduled arrival time of their bus. Students waiting for buses at other schools shall comply with the directions of all staff members at the other schools. When students are dropped of at the middle school they are to enter the school building immediately and go to their designated area.

Questions concerning student transportation may be directed to the Northridge transportation supervisor at 967-7533.

Additional building policies

Change of address or telephone number - The school must be able to contact parents if there is an emergency involving a

child. Please notify the district office (740 967-6631) any time a student has a change of address or telephone number. A change in a

parent’s work phone number should also be reported to the school.

Child abuse and neglect - Ohio law obligates school officials to report suspected child abuse or neglect to Children’s Services.

The school does not investigate such cases and makes no other decisions in this regard. All referrals are confidential.

Child custody - Parents are obligated to provide the school with up-to-date information regarding child custody. Copies of any

court orders, explaining or limiting custody, must be on file in the school office. A court official must sign such orders. The school

will be directed by this document rather than the wishes of one parent or the other.

Enrolling in school - In general, state law requires students to enroll in the school district in which their parent or legal guardian

reside unless enrolling under the District’s open enrollment policy.

New students under the age of 18 must enroll by their parent or legal guardian. When enrolling, parents must provide copies of the

following:

1. A Birth Certificate or similar document

2. Court papers allocating parental rights and responsibilities or custody (if appropriate)

3. Proof of residency

4. Proof of immunizations

When students enroll from another school, Northridge Local Schools will assist in obtaining school records from the previous

school. In addition, if a new student resides in the District with a grandparent and is the subject of a: (1) power of attorney designating

the grandparent as the attorney-if-fact; or (2) a caretaker authorization affidavit executed by the grandparent that provides the

grandparent with authority over the care, physical custody, and control of the child, including the ability to enroll the child in school,

consent in all school related matters, and discuss with the district the child’s educational progress, the student’s grandparent may

enroll the child in school on a tuition-free basis. However, in addition to the above-referenced documents that are typically required

for enrollment, the grandparent must provide the District with a duly executed and notarized copy of the power of attorney or care

takers authorization affidavit.

A student suspended or expelled by another public school in Ohio may be temporarily denied admission to the District’s schools

during the period of suspension or expulsion even if the student would otherwise be entitled to attend school in the District. Likewise,

a student expelled or otherwise removed for disciplinary purposes from a public school in another state and the period of expulsion or

removal has not expired may be temporarily denied admission to the District’s school during the period of expulsion or removal or

until the expiration of the period of expulsion or removal which the student would have received in the District had the student

committed the offense while enrolled in the district. Prior to denying admission, however, the Superintendent shall offer the student

an opportunity for a hearing to review the circumstances of the suspension or expulsion and any other factors the Superintendent

determines to be relevant.

Field trips - Field trips are academic activities that are held off school grounds. There are also other trips that are part of the

School’s co-curricular and extra—curricular program. No minor student may participate in any school-sponsored trip without parental

consent and a current emergency medical form on file in the office. Medications normally administered at school will be administered

while on field trips. The Student Code of Conduct applies to all field trips. Students who violate school rules may lose the privilege

to go on field trips. While the District encourages students to participate in field trips, alternative assignments will be provided for any

student whose parents does not give permission for the student to attend. Attendance rules apply to all field trips. A walking field trip

permission form is provided to parents to complete at the beginning of the year. Permission for walking field trips is needed for

weekly trips to the Public Library and visits to various places in the village.

Fire/tornado/safety drills - The School complies with all fire safety laws and will conduct fire drills in accordance with State law.

The School conducts tornado drills during the tornado season following procedures prescribed by the State. The alarm system for

tornadoes consists of a public address announcement. Teachers will provide specific instructions on how to proceed in the case of fire

or tornado and will oversee the safe, prompt and orderly student movement to safe areas. Safety drills will be conducted at least once per school year. Teachers will provide specific instruction on the appropriate procedures to follow in situations where students must be secured in their building rather than evacuated. These situations can include a terrorist threat, a person in possession of a deadly weapon on school property, or other acts of violence.

Lost and found - The lost and found area is in the main hallway. Students who have lost items should check there and may retrieve

their items if they give proper description. Unclaimed items will be given to charity at the close of the school year.

Lunch program - Students may buy a school lunch or pack a lunch. The menu is sent home monthly, and posted on the website.

Lunch prices are set annually by the board of education. Free or reduced price lunches are available to students who qualify for them.

Forms for applying for free and reduced price lunches are sent home with students at the beginning of the year.

Conferences - Conferences are scheduled in the fall and spring of each year for the purpose of encouraging parents and teachers

meeting to discuss the progress of their student/s. Appointment notices are sent home prior to the conference date. We encourage all

parents to contact their child’s teacher in advance regarding appointments.

Teachers are encouraged to request conferences on an “as needed” basis, and parents are always welcome to initiate a conference. If a

parent wishes to request a conference s/he should:

1. Phone the office and request a return the call from the teacher. It would be helpful to note the intent of the call so the teacher may be

better prepared to return the call.

2. Send a note to the teacher requesting a conference.

Impromptu conferences during the day cannot permitted, as classroom instruction must not be interrupted.

STUDENT RECORDS

Student cumulative records are kept in the school office. These records are confidential and protected by Family Education Rights

and Privacy Act (FERPA). Student records may be inspected by natural parents, adoptive parents, and foster parents given temporary

custody as well as certified school personnel and, in some instances, individuals employed by legal or social agencies Eligible persons

may inspect and have copies of school records within seven days following a written request to do so. Northridge School Board

policy restricts the release of student records to anyone except these individuals and agencies. (See Board Policy 8830)

A divorce or change in custody does not change the rights of a natural parent to inspect his/her child’s school records, receive a report card, or have a teacher conference unless these rights were specifically taken away by a court procedure. It is the obligation of the custodial parent to make the school aware that such proceedings have occurred. Step-parents do not have access to school records, report cards, or conferences unless these rights are conferred on them by the natural parent.

TEXTBOOKS AND MATERIALS

All basic textbooks are loaned to students for use during the school year. It is the student’s responsibility to keep textbooks clean

and in good condition. Students will be charged for lost or damaged textbooks and library books. Each student should write his/her

name in the front of the book and keep the book covered.

VISITORS Parents are always welcome at Northridge Middle School and are encouraged to visit the school. With prior arrangement, parents are encouraged to visit the school and their child’s classroom. It is necessary to make advance notice of one to several days with the teacher, or principal, prior to the visitation. WE TAKE YOUR CHILD’S SAFETY VERY SERIOUSLY. Therefore, all visitors, including parents, must ALWAYS check in first at the school office before gaining admittance to the classroom. We consider this procedure to be very important for the safety of the students and the security of our school. Conferences with teachers or the administration should be scheduled in advance. Unauthorized visitors on school property will be considered trespassing and will be reported to the proper authorities. Students are not permitted to bring friends or relatives to school with them as guests. All Visitors Must Report To The Office Upon Arrival At School

VOLUNTEERS We are successful because of our volunteers! We welcome you to volunteer at our school and during school related events. Volunteers will follow Board Policy and Procedures. An application to volunteer must be completed and all BCI checks must be completed, and Board approved before volunteers may begin volunteering their time. All Volunteers must report to the office and wear a visitor’s badge.

WITHDRAWAL FROM SCHOOL

No student under the age of 18 is allowed to withdraw from school without the written consent of his/her parents and in compliance with State law. A student who otherwise withdraws from school shall be reported to juvenile judge of the county. Parents must notify the Principal about plans to transfer their child to another school. School records, including disciplinary records of suspension and expulsion, will be transferred to the new school within 14 days of the parents’ notice or request. All school owned property must be returned and all personal articles removed from desks. Any outstanding fees should be paid at the time of

withdrawal.

HEALTH PROGRAM

Injury and illness - All injuries must be reported to a teacher or the office. If the injuries are minor, the student will be treated and

may return to class. If medical attention is required, the Office will follow the School’s emergency procedures and attempt to make

contact with the student’s parents.

A Student who becomes ill during the school day should request permission to go to the office. An appropriate adult in the office will

determine whether the student should remain in school or go home. No student will be released from school without proper parental

permission.

All students must be fever free and vomit free for 24 hours before returning to school.

Immunizations - Students must be current with all immunizations required by law, including but not limited to poliomyelitis,

measles, diphtheria, rubella, pertussis, tetanus, and mumps, or have an authorized exemption from state immunization requirements.

Students who start kindergarten during or after the 2006 school year must be immunized against chicken pox. For the safety of all

students, the school principal may remove a student from school or establish a deadline for meeting State requirements if a student

does not have the necessary immunizations or authorized exemption. Any questions about immunizations or exemptions should be

directed to the school nurse.

Proper hand washing procedure - Hand washing: protects health and prevents disease. According to the US Center for Disease

Control, “Hand washing is the single most important means of preventing the spread of infection.” The Northridge High School

recommends students and staff wash their hands: •After touching ears, nose, mouth, hair.

•Any contact with infected or otherwise unsanitary areas of the body. •After use of handkerchief or tissue. •Hand contact with unclean equipment or work surfaces. •Hand contact with soiled clothing, shop rags, or any other material that is soiled. •Handling raw food – partially cooked meat or poultry. •Handling money. •Eating. •After using the restroom.

Emergency medical authorization - Each student is required to have on file at the school office an Emergency Medical

Authorization, which gives direction to the school staff in case of an emergency. These are sent home at the beginning of each school

year and should be filled out and returned to school promptly. The school should be notified of any changes immediately. In the

event of a medical emergency, every effort will be made to contact the parent or guardian. If this is not possible, the emergency

medical authorization procedure will be used.

Medication - The administration of medication at school is addressed, in detail, in Ohio law and Board of Education policy. The

following guidelines must be followed in the Northridge Local School District:

1. All medication to be administered at school will be secured, supervised, and dispensed by a designated person. Medication must be

sent to school in the manufacturer’s container or the one in which it was prescribed. Medication must be delivered to the Principal’s

office by the student’s parent or guardian or by another responsible adult at the parent or guardian’s request. Students may not bring

medication to school. Students may carry emergency medications for allergies and /or reactions, or asthma inhalers during school

hours provided the student has written permission from a parent or physician and has submitted the proper forms. In the case of

epinephrine auto injectors (“epi pens”), in addition to written permission and submission of proper forms, the parent or student must

provide a backup dose to the school nurse. Students are strictly prohibited from transferring emergency medication, epi pens, or

inhalers to any other student for their use or possession. Medication may be conveyed to school directly by the parent or transported

by transportation personnel (bus driver and/or bus aide) at parental request. This should be arranged in advance. A student may not

possess or self-administer medication unless such need is specifically directed in writing by the student’s physician.

2. The appropriate form must be filed with the respective building principal before the student will be allowed to begin taking any

medication during school hours or to use an inhaler to self-administer asthma medication. Such forms must be filed annually and as

necessary for any change in the medication

3. Any changes in original information or instructions about administering medication will require a revised written request and the

appropriate signatures of parent/guardian and/or physician.

The parents shall have sole responsibility to instruct their child to take the medication at the scheduled time.

The principal will maintain a log noting the personnel designated to administer medication, as well as the date and the time of day

that administration is required. This log will be maintained along with the prescriber’s written request and the parent’s written release.

Any unused medication unclaimed by the parent will be destroyed by school personnel when a prescription is no longer to be

administered or at the end of a school year. Any student who distributes medication of any kind or who is found in possession of unauthorized medication is in violation of the School’s Code of Conduct and will be disciplined in accordance with the drug-use provision of the Code. These guidelines refer to both prescription drugs and non-prescription medications. Unless you have complied with the guidelines listed above, please do not send aspirin, cough medicine or other non-prescription drugs to school with a student. If at all possible, you should plan to give medicine to your child at home.

Chronic medical problems - Parents are responsible for providing the school with information about allergies, insect sting

reactions, and other medical problems. This information should be placed on the Emergency Medical Authorization Form and the

student information record card. It is also a good idea to report this information to the school office by note or telephone.

Control for casual-contact communicable diseases - Because a school has a high concentration of people, it is necessary to

take specific measures when the health or safety of the group is at risk. The School’s professional staff may remove or isolate a

student who has been ill or has been exposed to a communicable disease or highly-transient pest, such as lice. Specific communicable

disease include diphtheria, scarlet fever, strep infections, whooping cough, mumps, measles, rubella and other conditions indicated by

the Local and State Health Departments.

Any removal will be limited to the contagious period as specified in the district’s administrative guidelines.

Control of non-casual-contact communicable diseases - The School district has an obligation to protect staff and students

from non-casual-contact communicable diseases. When a non-casual-contact communicable disease is suspected, the student's health

will be reviewed by a panel of resource people, including the County Health Department. The school will protect the privacy of the

person affected and those in contact with the affected person. Students and staff will be permitted to remain in school unless there is

definite evidence to warrant exclusion. Non-casual-contact communicable diseases include sexually transmitted diseases, AIDS, ARC-AIDS Related Complex, HIV, Hepatitis B, and other diseases that may be specified by the State Board of Health. As required by Federal law, parents will be requested to have their child’s blood checked for HIV and HBV when the child bleeds at school and students or staff members are exposed to the blood. Any testing is subject to laws protecting confidentiality.

Control of blood-borne pathogens - The School District seeks to provide a safe educational environment for students and take

appropriate measures to protect those students who may be exposed to blood-borne pathogens in the school environment and/or during

their participation in school related activities. While the risks of students being exposed to blood-borne pathogens may be low,

students must assume that all body fluids are potentially infectious and must take precautions to follow universal procedures in order

to reduce such risks and minimize and/or prevent the potential for accidental infection.

COMPUTER/INTERNET USE Computers are located in classroom and the computer lab. In order to use a computer, students must return the Internet User Agreement.

NORTHRIDGE LOCAL SCHOOL DISTRICT

COMPUTER NETWORK AND INTERNET ACCEPTABLE USE POLICY AND AGREEMENT The Northridge Local School District is pleased to make available to students access to interconnected computer systems within the District and to the Internet, the world-wide network that provides various means of accessing significant educational materials and opportunities. In order for the School District to be able to continue to make its computer network and Internet access available, all students must take responsibility for appropriate and lawful use of this access. Students must understand that one student’s misuse of the network and Internet access may jeopardize the ability of all students to enjoy such access. While the School’s teachers and other Staff will make responsible efforts to supervise student use of network and Internet access and while the district employs a content filter, students must cooperate in exercising and promoting responsible use of this access. Below is the Computer Network and Internet Acceptable Use Policy and Agreement (“Policy and Agreement”) of the School District and the Data Acquisition Site that provides Internet access to the School District. Upon receiving, signing, and returning this Policy and Agreement as the students have

been directed, each student will be given the opportunity to enjoy Internet access at School and is agreeing to follow the Policy. The School District cannot provide access to any student who fails to sign and submit the Policy to the School as directed with the signatures of the student and his/her parents or guardians. Listed below are the provisions of your agreement regarding computer network and Internet use. If you have any questions about these provisions, you should contact the School. If any user violates this Policy and Agreement, the student’s access will be denied, if not already provided, or withdrawn and he/she may be subject to additional disciplinary action. I. Personal Responsibility

By signing this Policy and Agreement, you are agreeing not only to follow the rules in this Policy and Agreement, but are agreeing to report any misuse of the network to the School Office. Misuse means any violations of this Policy or any other use is not included in the Policy, but has the effect of harming another or his or her property. II. Purpose and Use A. The School District is providing access to its computer networks and the Internet for only educational purposes. If you have any doubt about whether a contemplated activity is educational, you may consult with the person(s) monitoring the Internet to help you decide if a use is appropriate. B. Netiquette. All users must abide by rules of network etiquette, which include: 1. Be polite. Use appropriate language. No swearing, vulgarities, suggestive, obscene, belligerent or threatening language. 2. Be safe. In using the computer network and Internet, do not reveal personal information such as your home address and telephone number. Do not arrange a face-to-face meeting with someone you meet on the computer network or Internet. Among uses that are considered unacceptable and constitute a violation of this Policy and Agreement are: 3. Uses that are offensive to others. Don’t use access to make ethnic, sexual preference or gender-related slurs or jokes. 4. Uses that violate the law or encourage others to violate the law. Don’t transmit offensive or harassing messages; offer for sale or use any substance the possession or use of which is prohibited by the Board of Education Adopted Student Conduct Code; view, transmit or download pornographic materials or materials that encourage others to violate the law; intrude into the networks or computers of others; and download or transmit confidential, trade secret information, or copyrighted materials. Even if materials on the networks are not marked with the copyright symbol, you should assume that all materials are protected unless there is explicit permission on the materials to use them. 5. Uses that cause harm to others or damage to their property. For example, do not engage in defamation (harming another’s reputation by lies); employ another’s password or some other user identifier that misleads message recipients into believing that someone other than you is communicating or otherwise using his/her access to the network or the Internet; upload a worm, virus, trojans, time bombs, or other harmful programming or vandalism. 6. Uses that jeopardize the security of student access and of the computer network or other networks on the Internet. For example, don’t disclose or share your password with others; impersonate another. 7. Uses that access controversial or offensive materials. All users and their parents/guardians are advised that access to the electronic network may include the potential for access to materials inappropriate for school-aged pupils despite the use of a content filter. Every user must take responsibility for his or her use of the computer network and Internet and stay away from these sites. Parents of minors are the best guide to materials to shun. If a student finds that other users are visiting offensive or harmful sites, he or she should report such use to the person monitoring the Internet usage. 8. Uses that are commercial transactions. Students may not sell or buy anything over the Internet. You should not give others private information about you or others, including credit card numbers and social security numbers. Rev. 8/02 III. Privacy Network and Internet access is provided as a tool for your education. The School District reserves the right to monitor, inspect, copy, review and store at any time and without prior notice any and all usage of the computer network and Internet access and any and all information transmitted or received in connection with such usage. All such information files shall be and remain the property of the School District and no user shall have any expectation of privacy regarding such materials. IV. Failure to Follow Policy and Breach of Agreement

THE USER’S USE OF THE COMPUTER NETWORK AND INTERNET IS A PRIVILEGE, NOT A RIGHT. A user who violates this Policy and breaches his/her Agreement, shall at a minimum, have his or her access to the computer network and Internet terminated, which the School District may refuse to reinstate for the remainder of the student’s tenure in the School District. A user breaches his/her Agreement not only by affirmatively violating the above Policy, but also by failing to report any violations by other users that come to the attention of the user. Further, a user violates this Policy and Agreement if he or she permits another to use his or her account or password to access the computer network and Internet, including any user whose access has been denied or terminated. The School District may take other disciplinary action. V. Updates The School District makes no warranties of any kind, either express or implied, in connection with its provision of access to and use of its computer networks and the Internet provided under this Policy and Agreement. It shall not be responsible for any claims, losses, damages or costs (including attorney’s fees) of any kind suffered, directly or indirectly, by any user or his or her parent(s) or guardian(s) arising out of the user’s use of its computer networks or the Internet under this Policy and Agreement. Policy and Agreement, users are taking full responsibility for his or her use and the parent(s) or guardian(s) are agreeing to indemnify and hold the School, the School District, the Data Acquisition Site that provides the computer and Internet access opportunity to the School District and all of their administrators, teachers and staff harmless from any and all loss, costs, claims or damages resulting from the user’s access to its computer network and the Internet, including but not limited to any fees or charges incurred through purchases of goods or services by the user. The user or, if the user is a minor, the user’s parent(s) or guardian(s) agree to cooperate with the School in the event of the School’s initiating an investigation of a user’s use of his or her access to its computer network and the Internet,

whether that use is on a School computer or on another’s outside the School District’s network.

VI. Updates Users may be asked from time to time to provide new or additional registration and agreement information, which must be provided by the user if he or she wishes to continue to receive service. If after you have provided your agreement information, some or all of the information changes, you must notify the School Office. Do not assume these changes have taken place until you receive notification from the school office. VII. Pictures of school sponsored activities on Web Pages

The School District reserves the right to publish pictures of school sponsored activities on School District sponsored web pages as long as last names of students are not published in correlation with the pictures. VIII. Term of Agreement

All K-3 student forms will remain on file and in effect each continuing year until student has withdrawn from that building. All students in grade 4-5 will complete a new form when moving to the intermediate building and will remain on file and in effect each continuing year until student has withdrawn from that building. Students will be required to complete a new form when enrolling in grades 6-12 which will remain in effect each continuing year until student graduates or withdraws from the Northridge Schools. A parent/guardian may change their student’s user status at any time by a written notice submitted to the school office/media center.

Parent or Guardian Network Usage Agreement As the parent or legal guardian of this student, I have read, understand and agree that my child or ward shall comply with the terms of the Northridge Local District’s Acceptable Use Policy and Agreement for the students’ access to the Northridge Local School District’s computer network and the Internet. I understand that access is being provided to the students for educational purposes only. However, I also understand that it is impossible for the School to restrict access to all offensive and controversial materials and understand my child’s or ward’s responsibility for abiding by the Policy and Agreement. I am therefore signing this Policy and Agreement and agree to indemnify and hold harmless the School, the School District and the Data Acquisition Site that provides the opportunity to the School District for computer network and Internet access against all claims, damages, losses and costs, of whatever kind, that may result from my child’s or ward’s use of his or her access to such networks or his or her violation of the foregoing Policy and Agreement. Further I accept full responsibility for supervision of my child’s or ward’s use of his or her access account if and when such access is not in the School setting. I hereby give permission for my child or ward to use the building-approved account to access the School District’s computer network and the Internet.
