1. A Transformative Service“Patients love the program—they often tell me it’s a godsend,” says Tyler. Countless children of Tyler’s patients were certain they would have had to hire in-home care or place their parent in an assisted living facility if Peninsula was not able to package medications with Medicine-On-Time.

8. Wanted: ChampionsHow many staff members are necessary to launch Medicine-On-Time? Just one! You read that correctly—

Peninsula Pharmacy has one full-time staff member dedicated to Medicine-On-Time for production. Tyler has one or two staff aiding the one, primary Medicine-On-Time technician to help with actual packaging, running labels and processing scripts.

The success of Medicine-On-Time depends on your staff: “Staff buy-in is required, and it helps to have a technician champion Medicine-On-Time to learn and take on a lot of the product aspects,” said Tyler.

Managing Medicine-On-Time with other responsibilities can be accomplished with the same amount of staff. “If you manage your workflow correctly, it doesn’t take any more staff and staff time to accomplish what you need. You’re doing production of these things during the slowest times of the week,” adds Dr. Jenema.

9. Control Challenges What makes your business stand out from the others? For Tyler, it’s being able to control his workflow with Medicine-On-Time. Usually Mondays are a nightmare for independent pharmacists, but not for Peninsula Pharmacy. Mondays and Fridays are no different than any other day of the week. Tyler proceeded to give us a brief outline of a typical Peninsula workflow:

“About 40 percent of patients are on the adherence program. We know when their medications are coming, when they are due and when they’re coming to pick up their medications. The 40 percent of prescriptions filled are controlled. For example, we know patient X is coming in on Wednesday for 12 medications. We aren’t scrambling to fill huge orders. We know they’re coming in to get medications and prioritize time to process runs and complete refills. You can increase business without increasing staff, and don’t need a ton of people on Monday.”

Dr. Jenema leans on his synchronization, adherence and compliance packaging programs as the reason why his business is entirely unique from his competitors. Tyler can synchronize and package medications, allowing him to say his services are about the patient and ensuring the patient is taking their medications. Another way Dr. Jenema grows his business is by “always seeking out places to speak to caregivers or patients,” whether these places are local assisted living facilities or professional groups. He works with local senior centers at least once per year before Medicare enrollment, and in the spring after open enrollment when people are on new plans. Tyler typically presents to a group of social workers or case managers about what his pharmacy does, what their adherence and compliance programs offer and about all other aspects of Peninsula Pharmacy.

“Make sure you have an adherence program. Have a technician learn the program and start slow. We [Peninsula Pharmacy] started with one patient who was going to transfer medication elsewhere that had a compliance packaging program. “If you have robust adherence program, bring in Medicine-On-Time and transition patients to Medicine-On-Time from day one. Solidify those patients to be Medicine-On-Time patients for as long as they’re at home [not in an assisted living facility].”

“Identify one or a couple of patients and go from there. We [Peninsula Pharmacy] had 10 patients for the first six months, and then grew very quickly once we decided to expand the program.”

Judy owns Caring Together, Incorporated with her husband, Bob Gill since 2001. Caring Together is a Christian-based, client-centered organization headquartered in Augusta, Georgia that seeks personal care options tailored to individuals’ needs. Caring Together subcontracts with personal care homes, and Judy recommends Medicine-On-Time to affiliated homes and pharmacies. Persistence Pays Off “She can’t see you!” is what Mrs. Gill’s receptionist repeatedly stated when John Kalvelage wanted to discuss Medicine-On-Time’s offerings to Mrs. Gill. Despite the receptionist’s unwavering response, Mr. Kalvelage did not head for the door in defeat—he kept talking about the product and all of its innovative features. All he wanted was five minutes of her time.

Little did Mr. Kalvelage know that Judy heard every word about Medicine-On-Time. “I was desperately looking for this product,” said Mrs. Gill. In 1993, Judy was operating assisted living facilities in Georgia and South Carolina, and nobody had extensive experience in the personal care field. Judy specifically mentions the client’s photo feature on the calendar card: “How can you possibly give anyone the wrong medication? I’m so impressed with the upgrades! It’s so easy to get patients mixed up even when there are only a few.”

MAR CapabilitiesThe Medication Administration Record (MAR) records, by law, every dose caregivers administer to patients. Eight months.

That’s all the time it took for pharmacy owners Erik P. Johnson and Richard K. Ploude of Anawan Pharmacy in Rehoboth, Massachusetts to conceptualize, build-out and officially open the doors to Anawan on September 1, 2016.

Implementing Medicine-On-Time

What was the deciding factor for bringing Medicine-On-Time into your business?

ERIK: We were looking for a differentiator that would set us apart from “big box” stores.

RICH: Both Erik and I have experience in nursing home pharmacies. Medicine-On-Time is a way to keep an elderly person independent for a little longer before seeking nursing home care. If medication is the big thing that a loved one is looking at and saying “I just can’t handle their meds any longer, they need to be in full-time nursing care,” Medicine-On-Time allows them to stave off putting them into full-time nursing for a little longer-- if it’s really just the meds that are the problem.

We offer home delivery as one of our services, and are able to deliver Medicine-On-Time. We have a customer whose father who is on multiple medications and her mother is suffering from Alzheimer’s. It was very confusing for them to manage their own medication. She’d have to go over medications every other week to try and setup a pill box. Using Medicine-On-Time, we take care of it [medication management] for her and it’s managed on our [Anawan’s] end. They are very pleased with the service.

Note: Pharmacists expertly prepare oral, prescription medications into Medicine-On-Time. There is no extra work for the patient to have their medications packaged into Medicine-On-Time.

What’s your pharmacy’s focus? Retail, long-term care, facilities?

ERIK: Anawan partners with facilities so that when patients are discharged, or if they were on a short-term rehab visit and they are going home, they can go home on the Medicine-On-Time system. It also helps the nursing facilities with medication compliance after the patient leaves so they don’t end up back in the hospital.

RICH: With Medicine-On-Time, facilities don’t have to worry as much [as they normally would] about their bounce back Star Ratings from Medicare. One of these ratings measures if a patient is readmitted into the hospital or nursing home within a certain number of days from the discharge date. If bounce backs [non-adherence related readmission rates] happen, this affects the facility’s Star Ratings through the Medicare system. We [Anawan] tell them that if they send their patients home on the Medicine-On-Time system, you’ll know patients will be taking their medications the way the compliance system is setup during those 30 days, and this will help with your Star Ratings in the long run. That’s kind of the angle we’re gearing toward facilities, saying it’s a good idea to discharge patients on this program.

How long have you been using the Medicine-On-Time solution? ERIK: The first [Medicine-On-Time] patient came onboard in early October 2016.

How does your staff feel about Medicine-On-Time? Do you see efficiency within your overall workflow?RICH: The staff are Erik and I! But I primarily use the solution.

I’ve worked with Medicine-On-Time since its implementation in October 2016, and have quickly developed efficiency with the solution. I learned more about Medicine-On-Time from Nina Zingariello [the Regional Account Manager for Medicine-On-Time customers in the New England and Northwestern regions of the United States]. She demonstrated how to use Medicine-On-Time and provided helpful feedback and workflow efficiency advice.

Rich proceeded to tell us about his workflow efficiency method and how he maintains organization when filling Medicine-On-Time calendar cards:

For accuracy, Rich prefers to fill the calendar cards, or blister packs, in the order pills are listed on the label itself. He uses the filler tray and plastic tray inserts for different pill sizes (A, B, C, D, etc.) to fill batches of calendar cards. Rich says how filling calendar cards in the order pills are listed on the label saves time in the checking process. This way, Rich knows he accurately completed the filling process in a methodical manner.

Getting Out There: Marketing Medicine-On-Time

What type of patients are using the Medicine-On-Time solution?

ERIK: We’re marketing Medicine-On-Time from a perspective other than [patients] wanting to maintain independence later on in life. We [Anawan] have a patient that is learning how to become more independent early in life. She is a pre-teen, and her mother has several young children. The mother wants her daughter to be able to understand how she has to take her medications everyday. She needs to be on medications for the rest of her life, and she wants her daughter to have a solution to better monitor her medication regimen, along with an approach toward independence and taking her medications on a daily basis.

Note: Medicine-On-Time is great for youths to manage medications, such as when they go away to summer camps and need to have a daily routine for managing their vitamin and/or prescription intake. Medicine-On-Time gives everyone an additional level of confidence for managing multiple medications in an independent and concurrent manner.

What do you hope to accomplish in 2017?

RICH: Ideally, to make a profit and grow (laughs)! That would be phenomenal. Really, some of the goals we’re looking into is getting ready to launch a free vitamin program for children. The goal is to let people know that Anawan is in the area. We [Anawan] are going to partner with local elementary schools and send flyers home with children telling their parents that if they come into the pharmacy and provide their child’s name, address, and date of birth, they can go home with a free bottle of children’s vitamins. They can return to Anawan on a monthly basis to pick up a new supply of vitamins.

It’s a way to get our name out there as well as generate traffic. If people are walking in to get free vitamins, they will walk around the store and see what we have, and maybe check our pricing compared to where they’re going now. It’s a great way to give back to the community and get people to know that we are a pharmacy in town. No purchase is necessary, it’s just a “come in the door, give us your information and meet us” scenario. Erik and I are confident in ourselves that this [Anawan] will be a better pharmacy experience for you than what you are experiencing now.

Moving On: Growing and Learning from Mistakes

What advice would you offer to new pharmacy owners? RICH: To have another owner to talk to, to find out what mistakes they’ve made or what they would do differently. We’re members of NPSC (Northeast Pharmacy Service Corporation), and they were very helpful in guiding us through some of the things we did not know regulatory wise and about insurance contracts. We [Rich and Erik] weren’t out there blind, but there’s a difference between having someone advise you on how to do something than talking to someone who actually did it, went through it, and find out what their experience was with different insurance contracts. Having another owner to bounce subjects off of, or to talk about what merchandise they carry, can help you make the right business decision.

How did you resolve mistakes? RICH: Learn from mistakes, and hopefully they don’t cost you too much money in the long run. Just learn from them, move on and don’t make that mistake again.

McCarthy's PharmacyMeet Jim Myers of McCarthy’s Pharmacy

Medication adherence shouldn’t prevent patients from enjoying their summer, especially if they’re headed to camp. Medicine-On-Time simplifies the medication management process for everyone involved, assuring camp directors and parents alike that medications are properly packaged based on the needs of the patient.

Meet McCarthy’s Pharmacy Jim Myers, Manager of McCarthy’s Pharmacy remembers when his summers at McCarthy’s were a bit different. 22 years ago, Jim began his career with the pharmacy as a cashier after school. He knows the pharmacy and the community well and wanted to offer something different that would also bring in extra business during the summer months.

In 2013, the pharmacy began to offer their patients the opportunity to do something they may otherwise never have the chance to do; attend summer camp. “The patients love it, adherence is close to 100%,” says Myers. The pharmacy now packages medications for 500 campers each summer and they are not stopping there. Offering patients this service allows the pharmacy to gain additional exposure to many other facilities.

They get the word out about their service any way they can and have added an additional 300 patients on top of the 500 summer camp patients they service during those specific months. “The concept of MOT allows med passes easier and more accurate for the facilities we fill for.” McCarthy’s has five staff members dedicated to the Medicine-On-Time System. “We sync the medications and then transition to the MOT system.”

You currently have 75 patients on MOT, how has Medicine-On-Time helped grow your pharmacy business?

Medicine-On-Time fills a service gap in our pharmacy and allows us to offer a unique solution in compliance packaging that truly simplifies and improves the medication management process.

You currently market the pharmacy to Physician groups and have found success with this effort, can you breakdown your process to get the Physicians and their staff on board?

The way we market to physicians is simple:

• ALIGN with the nurse practitioners and COMMUNICATE the benefit to their patient’s outcomes by improving their patient’s medication compliance

• FOLLOW UP with a luncheon at this point and present to the entire staff

• OFFICIALLY ON-BOARD the office staff with the MOT program, it is very important to EXECUTE a CE with the office every quarter since there will be inevitable turnover within the office

Cascade Pharmacy has set a goal of 200 MOT patients this year. Can you tell us some additional tactics you use to marketing MOT to your community?

“From a tech perspective, we are using our website, facebook, and google Adwords. Internally, we are utilizing our pharmacy software system (Pioneer Rx) to identify current patients that would benefit from MOT and reaching out to them by phone or approaching them during their next refill. The final way we get the MOT word out is through networking with our Dr.’s Offices and at local senior events within the community. This has been very beneficial for our growth. This can be difficult to do with time being the constraint, but it is a must and can be done.”

How many patients did you begin with and how many do you have now?

We began with around 50 patients and now our patient count stretches over 1,000 and still growing.

Medicine-On-Time offers customization based on your pharmacy’s needs and the needs of your patients. How do you use this to your advantage?

It’s a powerful system that helps my staff get work done quickly. We have an “assembly line” of techs. If there is a challenge they face, it can be handled right at the source because they are already heavily involved. This efficiency has allowed the business to grow in other areas.

How do you handle prescription changes within the facilities you service?

Patient’s medication changes happen. MOT allows my staff to add, edit or delete a prescription easily. For new prescriptions, we make a new card and then add the new prescription to the full card during the next cycle. We work closely with the nurses at each facility. By doing that, we have built relationships and they take us with them if they leave and go to another facility.

Reach out to the MOT staff if you have questions. Everyone is very accommodating and reachable. Invest in your people and allow them to really learn the system. When they need help, use the MOT Customer Support team to your advantage. They are your first-line of attack and available if you need them.

We just filled our One millionth prescription!

How are you marketing your Medicine-On-Time solution to your community?

Many different avenues are utilized in marketing to our close community. We use bag stuffers, home care organizations, VNA, case workers, and of course word of mouth. I have been to Independent Living facilities where they enjoy having topic talks with their residents. Medicine-On-Time currently has a terrific table-top display promoting conversation that can go near your point of sale area. For us we made it fun for our employees—whoever gets the most leads generated gets to have breakfast with their pharmacist of choice!

What are some tactics you use to grow your business?

We read Medicine-On-Time informational emails and pick and choose what works for our area. You must stay in constant touch with your referral people to share any updates and new approaches to introducing Medicine-On-Time.

How has Medicine-On-Time helped grow your pharmacy business? How do you market to physicians and healthcare groups/facilities?

Marketing Medicine-On-Time to providers/facilities is done along with other marketing efforts that I may be doing. We always keep brochures in the information racks in sitting areas, we make sure the RNs are familiar with the program, and we let the office staff know how to go about talking to the patient and that our pharmacy will do all the work! The patient just gets this nice and organized calendar each month—possibly even delivered to their doorstep if they live within city limits! How easy is that?!

The main improvement we have seen is in the compliance adherence area. This is an important issue because not only does the Medicine-On-Time patient add to your business but many times family members, and other potential customers that come through referrals, start coming to Wilcox.

We all love the laser print! It’s hard to believe the difference from the original dot matrix printing job. Customer response has been so positive with the change to laser. Clear, easy-to-read printing and the pictures of the medication are great too.
