To register as a vendor please complete the following:
1. Vendor Commitment Form
2. Vendor Attendee Registration Form
3. Return all completed forms to the Sponsorship Chair, Sue Jones
([email protected] or FAX—419-692-1500), no later than
March 17th. This will reserve your spot and ensure that you receive
recognition in our early promotional materials.
Payments do not need to be submitted with the forms, but payment must
be received prior to April 6th to guarantee your event sponsorship or ex-
hibitor table.

NEIAHU and NAIFA Fort Wayne will be jointly hosting their annual Super CE Day at Hotel Fort Wayne on April 28, 2016. Last year we had more than 125 agents and industry professionals attend this event.
We have several sponsorship and exhibitor opportunities listed below. Please select the level of your choice. Complete and return your Commitment Form and the Vendor Attendee Reg-istration Form by email or fax to reserve your space. Forms must be returned no later than March 17, 2016. Payments do not need to be submitted with the forms, but payment must be received prior to April 6, 2015. Space is limited!
