of community and our role as

members of that community. Our

hubs reach out to their community,

making vital connections which

enhance our customers' daily

experience.

We operate in partnership with our

customers in joining with our

community, enhancing our own

lives and those of the community.

We offer a staffing ratio of 1:4,

which allows for primarily

community based projects, and a

staffing ratio of 1:6, which is

primarily hub based projects. The

ratio determination is based upon

each customer's needs along with

Dept. of Mental Health funding.

customers, partnering with

HOW IT WORKS •••

Operating from our three

HUBs, each team

coordinates with their

local community

developing various

PROJECTSwhich meet the

needs of our customers

and the community.

Operating from each

customer's person

centered plan, projects

are identified (or created)

which will meet the needs

and wants of the

individual. To encourage

team work, the support

team will work with the

individual in identifying

other customers who

might be interested in

those projects and will

support the individual in

coordinating a team.

Within the framework of

the projects, on-going

training will occur in "real

time".
