    AboutpeoplewithlearningdisabilitiesPatientswithlearningdisabilitieswillneedspecialconsiderationtoensurethattheyreceivecareandtreatmentwhichmeetstheirspecificneedsandmaintainstheirsafety.

    TheNationalPatientSafetyAgency(2004)concludedthatpatientswithlearningdisabilitiesareparticularlyvulnerablewheninacutehospitalsettingsleadingtovaryingdegreesofharmasaresult.

    Frequentlypeoplewithlearningdisabilitieshaveanumberofcomplexneeds.Thesemayincludeimpairmentsinfunctionalskills,communicationandadditionalsensoryimpairments;asaresultanindividualmaybedisorientated,confusedoranxiousinasettingwhichisunfamiliar.

    Inaddition,individualswithmultipledisabilitiesorcomplexhealthproblemsarelikelytousehealthservices(includingacutehospitalservices)onaregularandfrequentbasis.

    Theimpactoflearningdisabilitywilloftenmean:

    o Thepersonwillbelessabletounderstandandretainneworcomplexinformation,

    o Theywillrequirethatyouusesimplelanguageandexplainthingsthatmaybeunfamiliar,

    o Theymaybeatgreaterriskofswallowingdifficulties,

    o Thepersonbelessabletocommunicatepainordiscomfort,

    o Mayrelyonotherstomeetsomeoralloftheirbasicneedsortomaintaintheirsafety.

    DutytopromotedisabilityequalityDisabilityDiscriminationAct(1995)placesadutyuponallserviceprovidersandpubicbodiestoensurethatpeoplewithdisabilitiescanaccessservicesonanequalandequitablebasis(generalduty).Serviceprovidersarerequiredtomakereasonableadjustmentstothewayinwhichaserviceorfunctioniscarriedouttoenabledisabledpeopletoaccessservicesandsupportonanequalbasis.

    TheActwasamendedin2005andnowrequirespublicbodiestakestepstoactivelypromotetherightsofpeoplewithdisabilities(specificduty).

    Inordertoachieveequalityofaccessforpeoplewithlearningdisabilitiesservicesmayneedtoprovidemorefavourablesupportortreatmenttodisabledpeopleinordertoachievethesameoutcomes(DisabilityRightsCommission,2007).

    Whatreasonableadjustmentsareneededwilldependuponanindividualsspecificdisabilityrelatedneeds.Thereforebothtimelyandcomprehensiveassessmentsofindividualsdisabilityrelatedandsafetyneedsiscrucialtoensuringthatpeoplewithdisabilitiesorcomplexneedsreceiveappropriatecareandsupport.

    Thefollowingareexamplesofreasonableadjustmentsthatmayberequiredbypatientswithlearningdisabilities.

    Considerofferingextendedordoubleappointmentswherethiswouldassisttosupporteffectivecommunication

    Considerofferingachoiceofappointmenttime,forexamplethefirstorlastappointmentsmaysuitparticularlyanxiouspatientsorthosewhofindsittinginbusywaitingareasdifficult

    Usethehospitalpassporttoensurethatyouhavegatheredinformationaboutapersonssafetyanddisabilityrelatedneeds(seethesectiononClinicaltools)

    Considerusingpicturesordiagramstoenhancepatientsunderstandingofdiseaseorplannedtreatments/procedures

    Ensurethatyouhaverecordedindividualsspecificaccessneedsandthenamesoffamilycarersorcommunityprofessionalswhocanprovidesupportoradditionalinformation.

    Reviewpatientinformationleafletsandensurethatstaffhaveaccesstoeasyreadleafletsforpatientswithlearningdisabilities

    Belowaresomeexamplesofreasonableadjustmentsmadeforpeoplewithlearningdisabilitiesorcomplexneeds

    WorkingwithHammersmith&FulhamCommunityTeamforPeoplewithaLearningDisability;thecentrehasdevelopedaspecialisedserviceforclientswithlearningdisabilities.

    ThePearlServicehasbeendesignedtospecificallymeettheuniqueneedsofpeoplewithlearningdisabilities.Thisincludesofferinganextendedappointment,usingeasyreadinformation&educationalmaterialsandadaptedcommunication.

    TheservicehasbeenrecognisedbytheDepartmentofHealthandHammersmith&FulhamPCT(TraceyHarrowAward)asanexampleofbestpracticewithregardstoachievingequalityofaccesstohealthcareforpeoplewithlearningdisabilities.

    Theoutpatientsdepartmentprioritisespatientswithlearningdisabilitiesandcomplexneeds.Itaimstoprovideapersonalisedservicetoindividualsandcarers.

    ThedepartmenthasdevelopedgoodworkingrelationshipswithWestminsterLearningDisabilityPartnershipandKensington&ChelseaCommunityTeamforPeoplewithalearningdisability.

    ThedepartmenthasdevelopedavideoforpeoplewithlearningdisabilitiesaboutcomingforanENTappointment.

    SupportingeffectivecommunicationCommunicationimpairmentsarecommonlyassociatedwithlearningdisabilities;whetherwithregardstoexpressivecommunicationorcomprehension.Itislikelythereforethatyouwillhavetoadaptyourcommunicationtomeettheneedsofpeoplewithlearningdisabilities.

    Thefollowingguidelinesmayhelptoensurethatyourcommunicationisappropriatetotheneedsofpeoplewithlearningdisabilities.

    o Ensurethatyourcommunicationisclear.Usesimplelanguageandkeepyoursentencesshort

    o Explainanydifficultorunfamiliarwords.Forexample:Iwillsendyouforanxrayitmaybebettertosay;wewillneedtotakeapictureofyourarm

    o Checkthattheindividualhasunderstood:CanyoupleasetellmeinyourownwordswhatIhavejustsaid

    o Givethepersontimetorespond

    o Usegesturestoemphasiseyourcommunication:pointtothepartofthebodyyouaretalkingabout

    o Usepicturesorobjectstodemonstratewhatyouaregoingtodobeforeyoudoit

    o ConsiderwhatEasyReadinformationmaybeavailable

    o Beawareofanyadditionaldisabilitiessuchashearingorvisualimpairment

    TotalCommunicationTotalCommunicationisaboutusinganumberofcommunicationmethodstogethertosupportpeoplewithcomplexneeds.Thismightincludeamixtureofspeech,gestureandaccessiblewritteninformationorpictures.

    SupportingadmissionsanddischargesTheTrusthasanadmissionanddischargeprocedureforpeoplewithlearningdisabilities(appendix1)whichisintendedtoensure:

    o Anydisabilityrelatedsupportneedsareidentified

    o Cliniciansaresupportedtoagreecareortreatmentplansandanyreasonableadjustmentsthatareneeded

    o Toensurethatadviceissoughtfromthespecialistcommunitylearningdisabilityteamorthepersonsmaincarer

    o Effectivedischargeplansaremadeinatimelymanner

    o Thatahealthactionplaniscompletedbythepersonscommunitysupportworker

    PreparingindividualsforadmissionsorclinicalinterventionsImprovedoutcomescanbeachievedforpatientswithlearningdisabilitiesorcomplexneedsbyeffectiveplanningandwilllikelyreducethenumberofappointmentsmissedoraborted.

    Considerationshouldbegiventowhatpreparationapatientwithlearningdisabilitiesorcomplexneedsmightrequireinorderachievinggoodoutcomesandapositivepatientexperience.

    Forexamplepatientsmaybeputateaseifsupportedtovisitthewardordepartmentaheadoftheirappointmentorhavetheopportunitytoseetheequipmentthatwillbeused.

    Patientswithlearningdisabilitiesarelikelytohavedifficultyunderstandingabstractconceptsorhavelimitedunderstandingofthetreatmentsbeingproposed.

    Additionally,manyindividualswillrequiresuchpreparationandinformationinordertomakeaninformeddecisionandtoconsenttotreatment.
