• Prévalence – 4 à 32% chez les patients cirrhotiques– 8 à 9,7% chez les patients avec HTP non cirrhotique– 1.1% chez les patients avec hépatopathie chronique
non cirrhotique et sans HTP.– En France, 1% adultes transplantés
• Survie à 5 ans à 23% et médiane de survie à 24 mois avec SHP vs 63% et 87 mois groupe contrôle sans SHP (non transplantés).

– Déséquilibre ventilation perfusion : dilatation des vx précapillaires et capillaires (réduction ou
absence tonus vasculaire et inhibition de la VC° hypoxémique) augmentation du débit sanguin limitant les échanges
– Shunts intrapulmonaires vrais, augmentation du nbre de vx précapillaires et capillaires
– Anomalies de diffusion de l’O2 : Diffusion O2 alvéolaire vers le centre du vaisseau dilaté et donc vers l’hémoglobine des GR limitée, parfois augmentation de l’épaisseur de la paroi vasculaire
