Introduction

The t+ diabetes Solution

t+ Medical helps people manage their long term conditions and improve their quality of life with the aid of user-friendly technology.

The t+ diabetes management system will help you track your blood glucose levels and give you immediate feedback as to how you are managing your condition. By harnessing the computing power of your mobile phone, your diabetes diary is now transformed into a powerful self management tool.

How does t+ diabetes work?

The t+ diabetes system uses your mobile phone’s web browser capabilities to securely send blood glucose readings and other diary information you enter to a secure central computer.

The data is transmitted via the mobile internet known as GPRS. It does not use text (SMS) to send information as text messages can be unreliable and unsecure.

Instead of writing results down in a paper diary, all your diabetes information is quickly entered into your personal diary on your mobile phone.

Once you have transmitted your latest readings, you will receive immediate feedback on your phone with easy to follow colour coded charts informing you how you are doing. This is an important part of helping you to manage your condition based on your personal data.

The information you send can then be reviewed on your phone or on the internet and with your permission, by your Doctor or Nurse or via a monitoring centre.

The information and interpretation tools in your personal diabetes diary are available on your mobile phone or on-line whenever and wherever you want, giving you the freedom to live your life.

Why should I use t+ diabetes?

Closer monitoring of blood glucose and, where applicable, insulin dose, can assist in keeping your sugar levels within normal range helping to prevent problems associated with diabetes or its long term complications. This means that through simple advice and making some changes to the way you manage your diabetes, the risk of further complications can be reduced.

When you send a blood glucose reading to your diary through the t+ diabetes system, you can feel confident that your results are instantly available for your Doctor, Nurse or monitoring team to review when they choose. Clinicians monitoring your diabetes can also be alerted to any specific readings which may require you benefiting from additional support.

Notes on this Manual

This user manual covers the “Mobile Portal” part of the t+ diabetes system and its associated diary questions only.

Individual healthcare organisations may specify their own alternative diary questions to be used by their patients, in which case, the diary questions presented on the phone may not be as listed in this manual.

Despite this, controls and methods of use outlined in this manual will still apply and should be used as a basis for operation along with any specific documentation provided by such organisations or by t+ Medical themselves.

Getting Started

Installing t+ diabetes on your phone

Once you have registered an account with t+ diabetes, either through your local surgery or health organisation, you will be sent an SMS containing a “hyperlink” to the t+ diabetes secure website.

You will also receive a separate email confirming your log in credentials.

The t+ diabetes system will work on many types of phones as long as they have the ability to access the internet via a compatible browser.

The set-up of every phone differs slightly, however the following steps should be sufficient to guide you through the install process.

The steps outlined below are for a traditional phone type.

If you have a new style smart phone with a touch screen, the phone with automatically display the options required once the URL link is selected. Simply select the option as required.

Continue to follow the traditional phone instructions as the set-up of your smart phone will follow the same basic format.

Step 1 – Bookmarking the t+ diabetes site

An SMS message will be sent to your mobile phone once your t+ diabetes account has been registered.

Open the SMS message and then by using your mobile phone “joystick”, highlight the URL link - https://m.tplusdiabetes.com and press the “options” button on your phone.

From the “options” drop down list – go to; Add to bookmarks.

Name your new bookmark so you can easily select it in the future e.g. Diabetes, and press OK.

A bookmark will now be saved on your mobile phone’s web browser to be easily accessed the next time you need to log-in to the t+ diabetes mobile portal to submit your readings.

If you don’t receive an SMS message, please contact t+ support on 0800 389 3205 to investigate further.

Note: Different phone models and makes will all have slightly different methods of bookmarking a website. Please refer to your own phone’s user guide for more in depth instructions.

Step 2 – Connecting to the t+ diabetes site

Once your new “Diabetes” bookmark has been saved, select the “options” button again and this time select – Open Link.

Your phone will now attempt to access the internet to allow you to log in to the t+ diabetes system.

Please select YES to allow this connection to take place.

If for any reason you are unable to access the internet, please contact t+ support on 0800 389 3205 to investigate further.

After the t+ diabetes website has loaded, you will be required to enter your log-in credentials.

Your log-in credentials will have been supplied to you by your local surgery or health organization or sent via email to an address supplied by you.

If you do not as yet have these details, please contact t+ support on 0800 389 3205.

Step 3 – Logging In

Using your mobile phone’s “joystick”, select the box titled - Login Name, and enter your login name credential.

Then select the box – Password, and enter your login password credentials.

Select the – Remember me box

You are now set to log into the t+ diabetes Mobile Portal system.

Using your phone’s joystick, Select the Login icon and the main t+ diabetes “diary” screen will be displayed.

You can now choose to submit a new Diary Record, view Messages from your Nurse or Health Organisation, review your transmitted Readings or browse our Educational pages.

Submitting a Diary Record

The diary entry page should be displayed by default when you log-in to the t+ diabetes system. If not, select the “Diary” tab from the t+ diabetes heading bar.

Today’s date and time will be pre-set allowing you to quickly upload your current diary record.

If you wish to enter a reading for an earlier Blood Glucose test, simply select the time/date box you wish to amend (using your mobile phones “joystick”), and then select the appropriate date and time for the reading you wish to submit.

Depending on what your local surgery or health organisation wishes you to record, you may be asked to input the following diary details; Questions 1 & 2 are always asked.

1. The Date of your Diary Reading

2. Your last Blood Glucose Reading

3. Confirm if the BG reading is Fasting or Normal

4. How many hours ago did you last eat

5. Your Blood Pressure Reading

6. How much Exercise will you do today

7. Your Weight

8. How much Insulin you administered.

9. Any personal Notes you wish to record

Simply scroll down through the set questions using your phone’s joystick and answer them appropriately.

Use you phone’s keypad to input the numbers you need e.g. Blood Glucose is 6.2 mmol/L

Note: On some handsets you will need to double click the 1 button to get a decimal point for your BG reading 6.2

Continue to scroll down through the set of questions answering them accordingly. You can the select the “Notes” box and input a personal note relating to the data you have inputted

Note: You can only mark readings as fasting if they have been recorded in the last four days, including today.

Once you have entered a Note, you can now save your diary record.

Select the SAVE button.

A confirmation message will be displayed on your phone.

t+ Secure Messages

If your readings are being monitored by your local surgery or health organisation, your Nurse or GP can securely message you via the t+ diabetes system as required.

When you first log into the t+ diabetes system to transmit your BG readings, check to see if you have received a message – Msg. (1)

If you do have a message, select the message – Msg. (1) tab from the t+ diabetes heading bar.

A list of unread messages will be displayed. Using your phone’s joystick, select each message and read. Once you have read the message and noted any requirements requested by your Nurse or GP select “back to message list”.

The message box will now be empty and Msg. (0) will be displayed.

Educational Pages

The t+ diabetes system has a library of educational information relating to diabetes that you can browse while logged into your account.

If you wish to browse the educational pages, select the Edu. tab from the t+ diabetes heading bar. Simply scroll through the educational pages that are of interest to you.

Note: Browsing the Educational pages on your mobile phone uses packet data. Network providers charge for the amount of data you download, so please check with your provider as to how much this will cost. Alternatively log on to your account via the website to browse the educational pages for free.

Reviewing your readings

Once you have entered a diary, your t+ diabetes system provides you with instant feedback. You can navigate from one feedback screen to the next by pressing the option you wish to review:

Blood Glucose Summary

View Blood Glucose Readings

View Blood Pressure Readings Your BG Summary view gives you an easy to read “traffic light” bar split into four coloured portions. If the arrow points to the green portion then your average blood glucose is in good control. If the arrow points to orange or red then your average blood glucose may be too high. If you are in the blue section this indicates your average blood glucose may be too low.

The BG Summary view will list your average BG and if recorded what your average BP reading is, along with the last BG and BP readings recorded.

As well as the BG Summary view, you can review you Blood Glucose and Blood Pressure readings as a traditional table.

The BG readings are colour coded allowing you to easily distinguish the readings as Normal, Hypo, Borderline or Hyper.

Readings that have beenMarked as Fasting will appear in the View BG Readings table with a (F) next to the reading.

Help and Further Information

I cannot log-in

The t+ diabetes web portal is reliant on your own mobile phone’s network signal. If there is no mobile signal, you will not be able to log-in to submit your readings.

The t+ diabetes system will also automatically lock your account if you have attempted to log-in using incorrect credentials more than 3 times.

If you are unable to log-in to your account using the credentials supplied, please contact support on 0800 389 3205 – Mon to Fri 9am to 5:30pm GMT to investigate further.

I am unable to submit my readings

The t+ diabetes web portal is reliant on your own mobile phone’s network signal.

If you attempt to submit a diary record and you lose network signal, the system will automatically log you out. You would be required to log back in and re-enter your reading.

The View BG Reading list will help you confirm if your last reading was uploaded before the network signal was lost.

I am unable to browse the web using my phone

The t+ diabetes web portal requires web access. Please contact your mobile phone network provider to ensure your phone is enabled to access the web or refer to your handset manual.
