This induction pack is to help you as a learner to familiarise yourself with the team. It provides you with the basics about the team and should help you to plan to meet your learning needs.

We recommend that all learners contact the team in good time prior to placement to identify who will be supervising your learning and confirm start times/hours.

During your placement you should have a minimum of a preliminary, midpoint and final meeting with whoever is supervising your learning. You should also undertake some form of leaning analysis then produce a learning contract and or action plan that provide structure and direction to the time you are with us. Your supervisor will be able to help draw this together but you must take a lead in negotiating your plan. Some examples of these tools are provided but you may wish to use those produced by your education provider.

Please ensure that you present any documents regarding your placement with our staff on the first day of your placement. This should include any assessment document, learning outcomes you hope to achieve and any type of ongoing record of achievement.

On completion of your placement you will be asked to provide an evaluation. Please take time to complete this openly and honestly as it is only with your feedback can we improved the quality of these valuable learning experiences.

We hope you find the time on your placement enjoyable and productive.

Service Description: The service is for adults from the age of 16 to end of life who experience a full range of functional mental health and associated social problems.Where appropriate joint working will occur with other internal and external services where individuals who have dual diagnosis; either learning disability or substance misuse.Services are provided to those who require mental health assessment and care form specialist mental health practitioners. A substantial number of service users who receive a service from the teams will have time limited issues and shall be referred back to their GP after intervention or when their condition has improved.Significant numbers of service users may remain involved with the team for on-going treatment, care and monitoring for several years. This will be subject to regular multidisciplinary review through the Care Co-ordination process confirming the need for ongoing support at this level and to promote improvement in the individual’s condition or quality of life through the process of recovery.

Team philosophy: The service delivery is multidisciplinary in structure, the team consists of practitioners and clinicians with a range of professional backgrounds from both Health and Social Care agencies. Whilst individual staff are operationally managed locally by the Operational Team Leader, professional lines of accountability and support exist within all staff groups.Services are provided through a Single Point of Access and a local Duty System these are operating Monday to Thursday 8.30 am to 5.00 pm and 8.30 am to 4.30 pm on Fridays. Bank Holiday excluded.

your time with us, to fully integrate with our team. The focus of your time with us should be on the

development of your clinical skills and knowledge underpinning those skills. Your supernumerary status is

assured, and the team recognise your learning needs, and will help you to identify learning opportunities.

You are expected to participate in all aspects of clinical care during your time with us, developing your clinical skills, whilst maintaining and enhancing the high standards of care we strive to deliver.

Discipline specific learning opportunities:

Student Nurses will have an opportunity to spend time with all the different disciplines / staff groups within our team but will spend the majority of time with their Nurse Mentor.Students will be able to follow the whole patient journey, from receiving referral, assessments, interventions by our team and other teams or services as required and through to discharge.

Local links and networks: There are links with the primary mental health service, Chase Emotional Wellbeing, GPs and other primary community services. Day opportunities are provided Mind, Quest and Richmond Fellowship. A crisis house provided by Richmond Fellowship is within our locality.There are links with other Mental Health teams; Crisis/home treatment, Early intervention team, Liaison Psychiatry etc. Three members of the assertive outreach team are based within the CMHT.

Types of treatments interventions offered within the service

Screening of new referrals, prioritising those needing crisis, urgent or routine assessment based on risk and need. Assessment of clients new to the service or have been previously seen. Formulation of a plan of care and undertaking a risk assessment. Liaison with a wide range of other disciplines and agencies to meet the service users needs. Interventions from a wide background of physical, psychological and social theories.

Housekeeping Park House is located close to Cannock bus station so is easy to get to by public transport. There are limited free parking spaces on site. The CMHT is located on the first floor. There is a lift for anyone with mobility issues. There are disabled toilets.There is a small kitchen with kettle, fridge and microwave oven for staff and student use. There are various food outlets in the town centre a short walk away. For a small monthly charge beverages are provided.
