Care Plan

Agreed with patient/ carer

All members of the team are involved in developing the care plan….

which is ultimately agreed and owned by the patient and reflects their own goals.

Working across health and social care, including the voluntary sector combines a wealth of information; ensuring gaps in care are identified.

Our Integrated Case

Management (ICM)

Approach:

Integrated health and social care into or coordinated close to the home

How do we all access the Care Plan?

Page 5: Technology Consultation: Shared Records, Telehealth and Patient-held Informtion

CHOC IT Progress• Use of EMIS Clinical Services as

live access to push and pull relevant information from the GP system

• One anticipatory care plan with key things which keep someone out of hospital

• Members of the multi-disciplinary team have access to enter data

• Flows immediately back into the GP EMIS system

• Can be viewed by out of hours staff

• Patient / carers / relatives also have information in printed form
