CCOHF is a grassroots community organization committed to improving oral health programs and services for Chicago-area residents through education, assessment, policy and program development , and collaboration.

CCOHF organized and convened a Chicago-area Oral Health Summit to chart a path that overcome disparities in oral health.

A systems-change Framework was used during the Summit to guide working groups and monitor resulting progress.

Chicago Community Oral Health Forum (CCOHF)

Chicago Framework was developed with input from important public health leaders in the Chicago area.

Over 60 community stakeholders convened at the Chicagoland Oral Health Summit (January, 2012) including:

Chicago Department of Public Health Illinois Department of Public Health American Dental Association Chicago Dental Society Advocacy Organizations Dental providers and administrators Area Foundations Public Health and Private Practice Providers

Summit working sessions were based on the Framework’s six objectives focusing on creating an actionable Oral Health Plan.

CCOHF organized/summarized activities suggested during the Summit and launched the first Chicago-Area Oral Plan (September, 2012).

CCOHF initiative “Leading Chicago to Oral Health Improvement” incorporates strategies to move the Plan towards implementation.

Initiative activities require active public/private collaboration to accomplish goals.

Chicago Area Oral Health Plan overarching goal: Improve access to dental services, oral health promotion and disease prevention for Chicago area residents

Monitor the use of the oral health care system

o Objective 1: Increase proportion of Chicago area residents who use the oral health care system in previous year Collection of baseline data in collaboration

with FQHCs/non-FQHCs in Cook County area (In progress).

o Objective 2: Increase proportion of Chicago area residents who had a preventive service in previous year Collection of baseline data in collaboration

with Chicago Department of Public Health School-Based Oral Health Program, Kennedy King College Dental hygienist program, FQHCs, and non-FQHCs (In progress)

Expand oral health infrastructure

o Objective 3: Increase number of agencies in Chicago area with an oral health prevention program and the number of agencies with a dental care component Collection baseline data (Accomplished) Release Chicago Area Oral Health Plan in collaboration with Chicago stakeholders

Develop and expand oral health promotion

o Objective 5: Educate the public about the importance of oral health as an essential component of general health. Develop CCOHF Oral Health Educator

Curriculum to be used in all Chicago Public Schools (Accomplished).

Pilot the 3rd phase of Community-Based Participatory Research Project in Englewood in collaboration with community partners(In Progress).

o Objective 6: Educate non-dental professionals about the importance of oral health as an integral component of comprehensive care. Implementing the Smiles for Life Curriculum

in three different settings:• A pilot in a large CHC educating the “primary

care team” using the SFL resources and tools (In Progress).

• A pilot incorporating the SFL Curriculum in City Colleges of Chicago educating students from health related careers (In Progress).

• Piloting SFL Curriculum in an Illinois Medicaid Care Coordination entity (Together4Health)

(In Progress). CCOHF advocacy efforts (On going).

Background

For the first time ever, oral health is one of the twelve national priority areas in Healthy People 2020 (HP2020) and also in the Illinois State Health Improvement Plan.

Two Institute of Medicine (IOM) oral health reports (2011) called to community leaders to act on behalf of the most vulnerable and underserved populations to prevent oral diseases and improve access to care.

Objectives

• To develop a collaborative public/private strategy to address disparities affecting Chicago-area residents using a systems-change Framework that aligns Chicago efforts to National and State initiatives for improving access to oral health care and education.

This project provides an overview of the development and implementation of the first Chicago Area Oral Health Plan.

CCOHF's coordinated efforts have mobilized partners and stakeholders in common goals: advocating for improved leadership and

strategy citywide stronger service and educational

infrastructure the responsible allocation of additional

resources to improve access to care in Chicagoland.

The project demonstrates the power of successful collaboration between public/private stakeholders to improve access to care.
